Özet
Giriş ve Amaç: Kanser cerrahisinde kullanılan analjezik yaklaşımlar immünitede ve kanser çıktılarında değişikliklere neden olmaktadır. Rejyonel veya sistemik analjezi yaklaşımlarının kanser çıktılarına etkileri üzerine farklı görüşler vardır. Bu çalışmada, evre 1 ve 2 meme kanserinde Erektör Spina Plan Bloğu (ESP), morfin ve non-steroid antiinflamatuar ilaç (NSAİİ) temelli analjezik yaklaşımların; metabolomik profil, Vasküler Endotelyal Büyüme Faktörü (VEGF), kortizol, C-reaktif Protein (CRP), sistemin immün inflamatuar indeks (SIII), nötrofil lenfosit oranı (NLR) ve platelet lenfosit oranına (PLR) etkilerini araştırdık. Gereç ve Yöntem: Çalışma prospektif gözlemsel olarak Aralık 2020-2021 tarihleri arasında meme kanseri ameliyatı olan 90 hastada gerçekleştirildi. 87 hasta değerlendirildi. Hastalar analjezi yöntemlerine göre ESP, morfin ve NSAİİ olarak üç grupta değerlendirildi. Hastaların demografik verileri, cerrahi tipi, American Society of Anesthesiologists (ASA) sınıflaması, ek hastalıkları, kullandıkları ilaçlar, transfüzyon uygulamaları, cerrahi süreleri kaydedildi. Preoperatif dönemde ve postoperatif 1, 2, 12 ve 24. saatlerde Numeric Rating Scale (NRS) kullanılarak ağrı skorları kaydedildi. Preoperatif (T0), postoperatif 1. saat (T1) ve 24. saat (T24) rutin olarak çalışılan tetkikler kaydedildi ve SIII, NLR ve PLR değerleri hesaplandı. Kortizol, CRP, VEGF çalışıldı ve metabolomik analiz yapıldı. Bulgular: Demografik bulgular ve ek hastalıklar açısından gruplar arasında fark yoktu. Prostaglandinler, araşidonik asit ve katekolamin metabolizmasında görevli metabolitler NSAİİ ve morfin gruplarında ESP grubuna göre yüksek ölçüldü (p<0.05, fold change (FC)>1.5). T24 anında NSAİİ grubunun VEGF'si, ESP'den yüksekti (p<0.05, FC=1.39). T24 anında NSAİİ grubunda kortizol düzeyi ESP grubundan yüksekti (p<0.05). T24'te morfin grubunun CRP'si hem ESP hem NSAİİ grubundan yüksek ölçüldü (p<0.05). Morfin grubunun postoperatif SIII ölçümleri ESP grubundan yüksekti (p<0.05). T24 anında morfin grubunun NLR'si ESP grubundan yüksek hesaplandı (p<0.05). T24 anında morfin grubunun PLR'si hem ESP hem NSAİİ grubundan yüksek hesaplandı (sırasıyla p<0.01, p<0.05). Sonuç: Metabolomik profilleme ile abartılı immün yanıtlarda rol oynayan araşidonik asit, katekolamin, prostaglandinler, morfin grubunda ESP grubuna göre belirgin yüksek bulundu. En ılımlı VEGF yükselişinin ve en düşük kortizol değerlerinin ESP grubunda görülmesi dolaşıma serbestlenen tümör hücrelerinin (circulating tumor cells, CTC) anjiyogenezis riskinin azaltılması ve immün homeostazisi korumak açısından ESP bloğun yararlı olabileceğini düşündürmektedir. Morfin grubunun CRP düzeyleri diğer gruplardan yüksek seyrettiğinden, morfinin immüniteyi olumsuz etkilediği düşünülebilir. SIII, NLR, ve PLR oranlarına bakıldığında, yine en yüksek değerler morfin grubunda gözlenmiştir. Sonuç olarak, ESP blok meme kanseri cerrahisi analjezisinde immün yanıtların kontrolü ve tümörogenezis riski açısından avantajlı, morfin ise dezavantajlı bulunmuştur. Kronik ağrı riski yüksek meme kanseri gibi cerrahilerde; morfin yerine hem yeterli analjeziyi sağlayacak, hem de immun sistemi modüle ederek metastaz, rekürrens gibi kötü çıktıları azaltacak rejyonel blok seçeneklerinin kullanılması tavsiye edilmektedir.

Background and Aim: Analgesic techniques that are implemented in cancer surgery alter immune responses and thus cancer outcomes. There are different opinions on the effects of regional or systemic analgesia approaches on cancer outcomes. In this study, we investigated the effect of different analgesic preferences, such as Erector Spina Plan Block (ESP), morphine, and non-steroidal anti-inflammatory drugs (NSAID) based analgesia, on metabolomic profile, Vascular Endothelial Growth Factor (VEGF), cortisol, C-reactive Protein (CRP), immune-inflammatory index (SIII), neutrophil-lymphocyte ratio (NLR) and platelet lymphocyte ratio (PLR) in stage 1 and 2 breast cancer surgery. Materials and Methods: This prospective observational study was carried out on 90 patients who underwent breast cancer surgery between December 2020-2021. 87 patients were evaluated. Patients were categorized into three groups according to analgesia methods as ESP, morphine, and NSAID. Demographic data of the patients, type of surgery, American Society of Anesthesiologists (ASA) classification, comorbidities, drugs used, transfusion practices, and duration of surgery were documented. Pain scores were recorded using the Numeric Rating Scale (NRS) at preoperative period, postoperative 1th, 2nd, 12th and 24th hours in the postoperative period. Preoperative (T0), postoperative hour 1 (T1), and hour 24 (T24) tests were listed and SIII, NLR, and PLR values were calculated. Cortisol, CRP, VEGF, and metabolomic analysis were performed. Results: There was no difference between the groups in terms of demographic findings and comorbidities. Prostaglandins and metabolites involved in arachidonic acid and catecholamine metabolism were higher in the NSAID and morphine groups compared to the ESP group (p<0.05, FC>1.5). VEGF levels of the NSAID group at T24 were higher than ESP (p<0.05, FC=1.39). At T24, the cortisol level in the NSAID group was higher than in the ESP group (p<0.05). At T24, the CRP of the morphine group was higher than both the ESP and NSAID groups (p<0.05). Postoperative SIII measurements of the morphine group were higher than the ESP group (p<0.05). At T24, the NLR of the morphine group was higher than the ESP group (p<0.05), and the PLR of the morphine group was higher than both the ESP and NSAID groups (p<0.01, p<0.05). Conclusion: Metabolomics in arachidonic acid, and catecholamine metabolism, which play a key role in immune responses were significantly higher in the morphine group than in the ESP. The fact that the most moderate VEGF elevation and the lowest cortisol levels were observed in the ESP group suggests that ESP block is beneficial in maintaining immune homeostasis and reducing the risk of angiogenesis and, thus may improve oncological outcomes. Since the CRP, SIII, NLR, and PLR of the morphine group were highest, one can suggest that morphine impairs immunity. In conclusion, we think that ESP block is favorable in terms of modulating immune responses and reducing the risk of tumorigenesis in breast cancer surgery analgesia comparing morphine based analgesic approaches. In surgeries such as breast cancer with a high risk of chronic pain; we recommend applying ESP block which will both provide adequate analgesia and modulate the immune system since it may reduce poor outcomes such as metastasis and recurrence.
