Özet

IV ÖZET Bu çalışma İran'da eğitim veren hastanelerdeki tıbbi dokümantasyon bölümlerinin yönetimsel sorunlarını ortaya çıkarmaya ve çözüm önerilerinde bulunmayı amaçlamaktadır. Tıbbi Dokümantasyon Bölümünün yönetim ve organizasyonunda hasta, hastane personeli, eğitim kurumları, devlet kuruluşları ve bunlara benzer kullanıcı gruplarının şimdi ki ve gelecekteki ihtiyaçlarına cevap verebilmek için yasal, yönetimsel, eğitimsel, standardizasyon ve personel gibi çeşitli konulardaki sorunların ortadan kaldırılması gerekmektedir. Bu konuda yaptığımız çalışmalarla İran'da eğitim veren hastanelerdeki tıbbi dokümantasyon bölümlerinin tüm araştırmacılara ve ttp öğrencilerine yeterince hizmet vermemesinin nedenleri incelenmiştir. Nitelikli personel eksikliği, yeterli standartların olmaması ve gereken bilginin tam olarak toplamamasının sorunlara neden olduğu ortaya çıkarılmıştır. Konu ile ilgili mevcut basılı kaynakların incelenmesi çalışmanın temelini oluşturmuştur. Araştırma ile ilgili anket, ve görüşmeler yapılmış ve böylece çalışmaya yön. veren asıl yöntem betimleme yöntemi olmuştur. Çeşitli yollarla elde edilen bulguların analizi tıbbi dokümantasyon yöneticilerinin bölümlerine etkin bir şekilde yönetemediklerini, nitelikli personel sayısının az olduğunu ve tıbbi dokümantasyon standartlarının ihtiyaçları karşılayamadığım göstermiştir. 3u konu ile ilgili olarak şunlar önerilmiştir: - Sağlık ve Tıp Eğitimi Bakanlığının, Araştırma ile ilgili Bölümünce tıbbi dokümantasyonun standardizasyon ve kanuni yönleri üzerinde çalışacak komite olusturulmalıdırYüksek Lisans düzeyinde tıbbi dokümantasyon bilimi için öğretmenler yetiştirecek eğitim programları hazırlamalıdır. Dünya' da uygulanmakta olan standartlar ve eğitim programları incelenerek personelin informal eğitimi için programlar planlanmalıdır. İran Tıbbi Dokümantasyon Derneği mümkün olan en kısa zamanda kurulmalıdır.

11 ABSTRACT The purpose of this study was to find out the impor tant problems of management of the medical record de partment, in teaching hospitals of Iran and provide re commendations for solving problems. The organization and management of medical records to meet present day and future requirements of the patient health care providers, medical and social researchers, government agencies, and educational institutions requires I the-. - fixing of responsibility for medical record management; the production of records plan; the develop ment of ; guidelines and standards; the establishment of a retention and disposal program; evaluation of records media; the provision of adequate and appropriate storage systems, and input from staff with professional knowledge and experience. The primary study in this field has shown that the medical record department, of teaching hospitals of Iran are not able to serve researchers and medical students sufficiently. The most important cause of this deff iciency is the lack of effective management in those departments. There is not enough skilled per sonnel in this field and the standard of medical records is generally poor, their content is usually inadequate and it is difficult to extract information quickly. Collection of data for this study were involved in sending questionnaires to the managers of the medical record departments and the hospitalrs administrators, direct observations, visiting hospitals and reviewing the existing literature. The survey method was used '.throughout the study., Analysis ' of replies to questionnaires which were distributed among the managers of the medical record departments shows that they are not sufficiently successful to fulfill the management activities in theirIll departments. Analysis of replies to questionnaires which were distributed among the hospital's administrators shows that the managers of the medical record departments have not enough skill to manage those departments. Hospitals' administrators believe that they need a manager who is university graduated in this field, while most of the managers are highschool diploma holders. The analysis of interviews and observations shows that the standard of the medical record is generally poor, and their content is usually inadequate and it is difficult to extract information from them quickly. It is recommended that: t A council for legal aspects of medical record and a council for standardization of medical record be establised by the Vice Minister of Research, Ministry of Health and Medical Education. - To establish an educational program for training teachers of medical record science at post graduate level. - Reviewing the existed educational program in this field and improve it 3 level to the standard which exist elsewhere in the world and planning a pro gram for informal education of personnel. - To estabish the Iranian Medical Record Association as soon as possible.
