How does pain feel? Verifiability and symptom endorsement in malingered, exaggerated, and genuine physical symptoms

Malingering is defined as intentional fabrication or gross exaggeration of symptoms for external benefit. The current study has investigated whether pure malingering, in which reported symptoms are nonexistent, and partial malingering, in which existent symptoms are exaggerated, could be differentiated by applying the Verifiability Approach (VA) and the Self-Report Symptom Inventory (SRSI). The logic behind the VA is that deceivers' statements contain more non-verifiable information, whereas truth tellers' accounts include more verifiable details. The SRSI taps into over-reporting by including a mix of genuine symptoms and implausible complaints (pseudosymptoms). Thus, we checked if pure malingerers, partial malingerers, and truth tellers can be differentiated in their statement (non)verifiability and symptom endorsement. Participants (N = 124) allocated to three conditions (pure malingerers vs. partial malingerers vs. truth tellers) were instructed to provide symptoms reports and fill out the SRSI. Findings revealed that malingerers overall produced lengthier statements and included more verifiable details in their statements than truth tellers. The groups did not differ regarding non-verifiable details. Truth tellers endorsed less symptoms of the SRSI than partial malingerers, but not than pure malingerers. Pure malingerers and partial malingerers did not differ mutually in statements' (non)verifiability nor in symptom endorsement. Overall, the study findings revealed that, when compared with truth tellers, partial malingerers (i.e., exaggerators) exhibited stronger over-reporting tendencies than pure malingerers. However, when mutually compared, the differentiation between fully fabricated complaints and exaggerated symptoms is limited using the VA and the SRSI.
