Özet
Plastik Cerrahi pratiğinde oldukça fazla uygulanan ve talep gören operasyonlardan olan rinoplastide amaç hem kozmetik hem de estetik olarak tatmin edici bir sonuç ortaya koymaktır. Küçültme amaçlı olarak başlayan rinoplasti serüveni son dönemde burun ucunun ışık ve gölgelenmesini öne çıkarmaktadır. Bülböz burun ucu deformitesi rinoplastide en sık karşılaşılan ve zorlu deformiteler arasında yer alır. Literatürde bu deformitenin çözümüyle ilgili geniş greft içeren öneriler bulunmaktadır. Yaptığımız literatür çalışması sonucunda karşılaşmadığımız için önerdiğimiz bu teknikle plastik cerrahinin temel prensiplerinden olan benzeri benzerle onar prensibine uygun olarak hem deformitenin çözümüne katkı sağlanması hem de yeni dönor saha komorbiditelerinden kaçınılması hedeflenmektedir.Çalışmamızda bülböz burun ucu deformitesi olan 65 hastaya kaudal turn under flep uygulanmıştır. Kaudal turn under flep; sefalik eksizyon sonrası kalan lateral krusun kaudalinden hazırlan mukoza devamlılığı korunan kıkırdak flebini içermektedir. Bu flep kalan lateral krusun mukozal yüzüne sütüre edilerek hem bülbözite deformitesinin çözümüne katkı sağlanmakta hem de eksternal nazal valv desteklenmektedir. Çalışma tasarımında hastaların ameliyat öncesi ve ameliyat sonrası 3.aylarında 9 farklı pozda fotoğrafları çekilmiş, NOSE ve Utrecht Rinoplasti Değerlendirme anketi uygulanmıştır. Ayrıca çalışmadan bağımızsız 4 plastik cerrah tarafından ameliyat sonrası 3.ay fotoğrafları üzerinden Görsel Analog Skala ve Nazal Pinc Değerlendirme Testi uygulanmıştır.Takiplerine gelmeyen 15 hastanın çalışmadan çıkarılması sonrası 50 hasta üzerinden yapılan istatistiksel analiz sonucunda Kaudal Turn Under flep uygulanan hastaların ameliyat öncesine göre estetik ve fonksiyonel olarak belirgin bir ilerleme görüldüğü sonucuna varılmıştır. Kaudal turn under flep bülböz burun ucu deformitesinin çözümüne sağladığı katkıyla estetik olarak, eksternal nazal valve sağladığı destekle de fonksiyonel olarak rinoplastide etkili bir teknik olarak kullanılabilir.

ABSTRACT The aim of rhinoplasty, which is one of the most applied and demanded operations in Plastic Surgery practice, is to provide a satisfactory result both cosmetically and aesthetically. The evolution of rhinoplasty, which began with the purpose of reduction, has recently been emphasized the light and shading of the tip of nose. Bulbous nasal tip deformity is one of the most frequently used and challenging deformities in rhinoplasty. In the literature, there are suggestions for the solution of this deformity, including large grafts. Since we did not encounter this technique as a result of the literature study we have done, it is aimed to contribute to the solution of the deformity and to avoid new donor area comorbidities,to contribute to the solution the principle of repair like with like, which is one of the basic principles of plastic surgery. In our study, Caudal Turn-under Flap was applied to 65 patients with bulbous nasal tip deformity. Caudal turn under flap; It consists of a cartilage flap, which is prepared from the caudal of the lateral crus remaining after cephalic excision, with the continuity of the mucosa preserved. This flap is sutured to the mucosal surface of the remaining lateral crus, contributing to the resolution of the bulbous tip deformity and supporting the external nasal valve. In the study design, the patients were photographed in 9 different poses before and after the surgery, and the NOSE and Utrecht Rhinoplasty Evaluation Questionnaire was applied. In addition, Visual Analogue Scale and Nasal Pinch Evaluation Test were applied on the postoperative 3rd month photographs by 4 plastic surgeons independent of the study. As a result of the statistical analysis performed on 50 patients after the exclusion of 15 patients who did not come for follow-up, it was concluded that patients who carried out to Caudal Turn Under flap showed a significant improvement in aesthetics and functionality compared to preoperatively. The caudal turn-under flap can be used as an effective technique in rhinoplasty aesthetically with its contribution to the solution of bulbous nasal tip deformity, and functionally with the support it provides to the external nasal valve.
