Özet

Amaç: Timpanik membran perforasyon tamirinde kullanılan tek kat kartilaj greft ile çift kat kartilajın greft kullanımın işitme ve greft başarısı açısından karşılaştırılması Gereç ve Yöntem: Eylül 2020- Eylül 2022 yılları arasında kronik otitis media tanısıyla timpanoplasti yapılan hastalar prospektif olarak değerlendirildi. İşitme başarısı açısından 195 hasta; greft başarısı açısından ise 211 hasta çalışmaya dahil edildi. Kolesteatoma nedeniyle opere edilen, kanal wall down timpanomastoidektomi, butterfly timpanoplasti, kronik mukozal hastalık nedeniyle intakt kanal wall timpanomastoidektomi operasyonu uygulanan hastalar çalışma dışı tutuldu. Hastalar postoperatif 1 yıl takip edildi. İşitme ve greft başarısı 6. ay otomikroskopik muayene ve saf ses odyometri sonuçları ile değerlendirildi. Her iki grup işitme ve greft başarısı açısından karşılaştırıldı. Ayrıca perforasyon boyutu ve middle ear risk index (MERI) skorlarının greft başarısı üzerindeki etkileri değerlendirildi. İstatistiksel analizler için NCSS (Number Cruncher Statistical System) 2020 Statistical Software (NCSS LLC, Kaysville, Utah, USA) programı kullanıldı. Shapiro Wilks test, Student t-test, Paired Sample t-test, Ki-Kare test, Fisher's Exact test, Fisher Freeman Halton test ve Box Plot grafiklerden yararlanıldı. Sonuçlar %95'lik güven aralığında, anlamlılık p<0.05 düzeyinde değerlendirildi. Bulgular: Çift kat greftleme ile yapılan 87, tek kat greftleme ile yapılan 108 tip 1 timpanoplasti hastası işitme ve greft başarısı açısından incelendi. Yaş ortalaması 35.58 (±13.80) olan 18-65 yaş aralığında 78 erkek ve 117 kadın hasta bulunmaktaydı. Çift kat greftleme grubunda preoperatif hava yolu ortalama 38.59 (±14.18) dB, kemik yolu ortalama 13.93 (±12.06) dB iken; postop 6.ay hava yolu ortalama 25.34 (±14.99) dB'dir. Tek kat greftleme grubunda preop hava yolu 34.93 (±12.81) dB, kemik yolu ortalama 8.98 (±6.88) iken; postop 6.ay hava yolu 20.50 (±8.82) dB'dir. Çift kat greftleme grubunda postop hava yolu değeri istatiksel olarak anlamlı yüksektir (p=0,005). Preop ve postop hava kemik aralığı çift katta 11.40 (±8.61) dB işitme kazancı var iken; tek katta 12.95 (±10.88) dB işitme kazancı olup gruplar arasında işitme kazancı açısından istatiksel olarak anlamlı fark yoktur. Çift kat greft grubu greft başarısı 84/87 (%96.6) iken, tek kat greft grubunda 103/108 (%95.4) olup gruplar arasında fark yoktur (p=0,679). Farklı timpanoplasti tipleri ve MERI skoru ≥4 olan hastaları da içeren 43 erkek, 60 kadın hastadan oluşan 103 hastaya yapılan çift kat greftleme tekniğinin kullanıldığı grubun greft başarısı ise 99/103'tür (%96.1). Sonuç: Çift kat greftleme ile yapılan timpanoplastinin tek kat greftleme ile yapılan timpanoplastiye greft başarısı açısından avantajı yoktur. İşitme başarısı açısından ise çift kat greftleme grubunun hava yolu iletim sonuçları tek kat grefleme grubundan anlamlı olarak daha yüksektir.

Aim: Comparison of single layer cartilage graft and double layer cartilage graft used in the repair of tympanic membrane perforation in terms of hearing and graft success. Materials and Methods: Patients who underwent tympanoplasty with the diagnosis of chronic otitis media between September 2020 and September 2022 were evaluated prospectively. In terms of hearing success, 195 patients; In terms of graft success, 211 patients were included in the study. Patients who were operated for cholesteatoma, who underwent canal wall down tympanomastoidectomy, butterfly tympanoplasty, and intact canal wall tympanomastoidectomy for chronic mucosal disease were excluded from the study. The patients were followed up for 1 year postoperatively. Hearing and graft success were evaluated with the 6th month otomicroscopic examination and pure tone audiometry results. Both groups were compared in terms of hearing and graft success. In addition, the effects of perforation size and middle ear risk index (MERI) scores on graft success were evaluated. NCSS (Number Cruncher Statistical System) 2020 Statistical Software (NCSS LLC, Kaysville, Utah, USA) program was used for statistical analysis. Shapiro Wilks test, Student t-test, Paired Sample t-test, Chi-Square test, Fisher's Exact test, Fisher Freeman Halton test and Box Plot graphics were used. The results were evaluated at the 95% confidence interval and the significance level was p<0.05. Results: 87 patients with double-layer grafting and 108 patients with type 1 tympanoplasty with single-layer grafting were evaluated in terms of hearing and graft success. There were 78 male and 117 female patients between the ages of 18-65 with a mean age of 35.58 (±13.80). In the double-layer grafting group, the mean preoperative air conduction was 38.59 (±14.18) dB, and the mean bone conduction was 13.93 (±12.06) dB; Postop 6th month air counduction average is 25.34 (±14.99) dB. In the single layer grafting group, the preoperative air conduction was 34.93 (±12.81) dB, and the mean bone conduction was 8.98 (±6.88); Postop 6th month air conduction is 20.50 (±8.82) dB. The postoperative air conduction value was statistically significantly higher in the double-layer grafting group (p=0.005). While there is 11.40 (±8.61) dB hearing gain in preop and postoperative air bone opening in double layer; 12.95 (±10.88) dB hearing gain in a single layer and there is no statistically significant difference between the groups in terms of hearing gain. The graft success rate in the double-layer graft group was 84/87 (96.6%), and the single- layer graft group was 103/108 (95.4%) and there was no difference between the groups (p=0.679). The graft success rate of the group using the double layer grafting technique was 99/103 (96.1%) in 103 patients, consisting of 43 male and 60 female patients, including patients with different tympanoplasty types and MERI score ≥4. Conclusion: Tympanoplasty with double-layer grafting has no advantage over single-layer tympanoplasty in terms of graft success. In terms of hearing success, the air conduction results of the double-layer grafting group were significantly higher than the single-layer grafting group.
