Özet

Obezite, aşırı veya anormal yağ dokusu birikimi ile giden kronik, sistemik, multifaktöriyel etyolojiye sahip bir hastalıktır. Sınıflaması Vücut Kitle İndeksi (VKİ) kullanılarak yapılır ve VKİ'nin 30 kg/m 2' den fazla olması obezite olarak tanımlanır. Artan prevalansıyla birlikte neredeyse her kültür ve etnik uygarlığı etkileyerek modern sağlık sistemlerine yük getirmektedir. Bu nedenlerle obezitenin tedavisi giderek önem kazanmaktadır. Obez bireylerin multidisipliner yaklaşım ile yönetilmesi gerekmektedir. Bariatrik prosedürler önemli miktarda kilo kaybını elde etmek ve sürdürmek için obezitenin tedavisindeki yerleşik ve en etkili yöntem olmaktadır. Sleeve gastrektomi (SG), midenin yaklaşık %80'inin, tübüler bir yapı oluşturacak şekilde uzaklaştırıldığı bir restriktif metabolik cerrahi prosedürdür. Obezite gelişimi ve epigenetik mekanizmalar arasındaki ilişki son yıllarda yapılan çalışmaların ilgi noktası olmuştur. Uzun kodlanmayan RNA'lar (Long non-coding RNA; Lnc-RNA), epigenetik değişikliklerde görev alan sinyal düzenleyicileridir. Bu tez çalışmasında, diyet ile indüklenen obezite ve non-alkolik yağlı karaciğer hastalığı (NAFLD) ve non-alkolik steatohepatit (NASH) gelişen sıçanlarda, Lnc-RNA ailesinde yer alan HULC (Highly up-regulated in liver cancer ), MALAT1 (Metastasis associated lung adenokarsinom transcript 1) ve NEAT1 (Nuclear paraspeckle assembly transcript 1)'in sleeve gastrektomi öncesi ve sonrasındaki ekspresyon durumları; bu markerların lipogenez ve inflamasyon ile ilişkisi incelenmiştir. Sağlıklı sıçanlar ile karşılaştırıldığında HULC, MALAT1 ve NEAT1'in ekspresyonunun obez olan deney gruplarında anlamlı olarak yüksek olduğu saptanmıştır. Obezite sonucu NAFLD gelişen ve laparotomi yapılan deney grubunda belirteçlerin preoperatif ve postoperatif ekspresyon durumunun değişmediği görülmüştür. Obezite sonucu NAFLD gelişen ve sleeve gastrektomi yapılan grupta kilo kaybı ile birlikte postoperatif analizlerde NEAT1'in ekspresyonunun anlamlı olarak düşüş gösterdiği bulunmuştur. Obezite sonucu NASH gelişen ve sleeve gastrektomi yapılan grupta MALAT1 ekspresyonun değişmediği, NEAT1 ve HULC ekspresyonun ise postoperatif anlamlı düşüş gösterdiği saptanmıştır. Literatürde Lnc-RNA'ların ekspresyonlarının up-regüle ya da down-regüle olmasının kanser dahil birçok hastalıkta etkili olabileceğine yönelik çalışmalar bulunmaktadır. Bu çalışma sonucunda obezite sonucu gelişen NAFLD ve NASH hayvan modelinde Lnc-RNA ailesinden olan HULC, MALAT1 ve NEAT1'in yüksek ekspresyon profili sergilediği gösterilmiştir. Bu sonuç HULC, MALAT1 ve NEAT1'in lipogenezis ve inflamasyon ile ilişkili Lnc-RNA'lar olduğunu doğrulamaktadır. NEAT1 ekspresyonunun sleeve gastrektomi yapılan grupta normal seviyelere gerilemiş olması ilk kez bu çalışma ile gösterilmiş olup, gelecekte hedefe yönelik tedavi stratejilerinin geliştirilebilmesi ve toplum tabanlı taramalarda belirteç olarak kullanılabilmesi için yapılacak çalışmalara katkısı olacağı düşünülmektedir.

Obesity is a chronic, systemic, multifactorial disease with excessive or abnormal fat tissue accumulation. Its classification is made using Body Mass Index (BMI) and a BMI of more than 30 kg/m 2 is defined as obesity. With its increasing prevalence, it affects almost every culture and ethnic civilization, placing a burden on modern healthcare systems. For these reasons, the treatment of obesity is becoming increasingly important. Obese individuals need to be managed with a multidisciplinary approach. Bariatric procedures are the established and most effective method of treating obesity to achieve and maintain significant weight loss. Sleeve gastrectomy is a restrictive metabolic surgery procedure in which approximately 80% of the stomach is removed to form a tubular structure. The relationship between obesity development and epigenetic mechanisms has been the focus of studies in recent years. Long non-coding RNAs (Lnc-RNA) are signaling regulators involved in epigenetic changes. In this thesis study, the expression status of HULC, MALAT1 and NEAT1, which are in the Lnc-RNA family, before and after sleeve gastrectomy in rats that developed diet-induced obesity and non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH); The relationship of these markers with lipogenesis and inflammation was examined. Compared to healthy rats, the expression of HULC, MALAT1 and NEAT1 was found to be significantly higher in the obese experimental groups. It was observed that the preoperative and postoperative expression status of the markers did not change in the experimental group who developed NAFLD as a result of obesity and underwent laparotomy. In the group that developed NAFLD as a result of obesity and underwent sleeve gastrectomy, it was found that the expression of NEAT1 decreased significantly in postoperative analyzes with weight loss. It was found that MALAT1 expression did not change in the group that developed NASH as a result of obesity and underwent sleeve gastrectomy, while NEAT1 and HULC expression showed a significant postoperative decrease. There are studies in the literature that up-regulation or down-regulation of the expression of Lnc-RNAs may be effective in many diseases, including cancer. As a result of this study, it was shown that HULC, MALAT1 and NEAT1, which are members of the Lnc-RNA family, exhibited a high expression profile in the animal model of NAFLD and NASH, which developed as a result of obesity. This result confirms that HULC, MALAT1 and NEAT1 are Lnc-RNAs associated with lipogenesis and inflammation. The fact that NEAT1 expression decreased to normal levels in the sleeve gastrectomy group was shown for the first time in this study, and it is thought that it will contribute to future studies to develop targeted treatment strategies and use it as a marker in population-based screenings.
