Özet

Amaç: Bu çalışmanın amacı sistemik inflamatuar yanıt indeksinin geç preterm erken membran rüptürünü öngörmedeki rolünün belirlenmesidir. Gereç ve Yöntem: Çalışmamız T.C. S.B.Ü. Dr. Sami Ulus Kadın Doğum, Çocuk Sağlığı ve Hastalıkları Eğitim ve Araştırma Hastanesi'nde Kliniği'nde 1 Ocak 2016-1 Ocak 2022 tarihleri arasında geç preterm erken membran rüptürü olan hastalarda birinci ve üçüncü trimester hemogram sonuçlarından elde edilen sistemik inflamatuar yanıt indeksinin (nötrofil x monosit/ lenfosit) geç preterm erken membran rüptürünü öngörmedeki rolünün değerlendirilmesi amacıyla yapılmış olan retrospektif tanımlayıcı kesitsel bir çalışmadır. Çalışmaya belirtilen tarihler arasında hastanemizde doğum yapmış, dahil olma kriterlerini karşılayan toplam 200 hasta dahil edilmiş, 34-37.gebelik haftası arasında steril spekulum ile incelenmesi sonucu vajende amniyotik sıvı göllenmesi ve ya Amnisure testi ile vajinal sıvı gelişi tespit edilmiş(amniyon membran rüptürü olan) hastalar çalışma grubunu, çalışma grubuna dahil edilen her hasta ile aynı zaman diliminde poliklinik kontrolüne gelen 34-37 gebelik haftaları arasında, suları gelmemiş olan, ilk ve üçüncü trimesterde hemogramları bakılmış olan bir hasta rastgele seçilerek kontrol grubuna (n=100) dahil edilmiştir. Gruplar birinci ve üçüncü trimester SIRI değerleri açısından karşılaştırılmıştır. Bulgular: Gruplar arasında yaş, gravida, parite, doğum haftası, doğum ağırlığı, 1. ve 5.dakika Apgar değerleri, çalışma durumu, eğitim ve eş eğitim düzeyi, gelir seviyesi ve doğum şekli açısından istatistiksel olarak anlamlı farklılık saptanmamıştır (p>0,05). Çalışma grubunda 1. ve 3. trimester hemoglobin değerlerinin anlamlı düzeyde düşük, nötrofil değerlerinin ise anlamlı düzeyde yüksek olduğu bulunmuştur (p<0,05). Çalışma grubundakilerin 1.trimester SIRI değerlerinin, anlamlı düzeyde daha yüksek olduğu belirlenmiştir (Z=-3,880; p<0,001). Optimal 1.trimester SIRI cut-off değeri %75,0 sensivite ve %54,0 spesifite ile ≥3,3592 olarak tespit edilmiştir (AUC=0,659; p<0,05). Çalışma grubunda 1. trimester NLO ve MLO ortalamalarının anlamlı seviyede yüksek olduğu (Z=-3,246, p=0,001; Z=3,129, p=0,002) ancak 3. trimester NLO ve MLO ortalamalarının benzer olduğu saptanmıştır (Z=-0,892, p=0,372; Z=-0,172, p=0,863). Lojistik regresyon analizi sonucunda; 1.trimester SIRI parametresinin, hastalık durumunu etkileyen önemli bir parametre olduğu tespit edilmiş (p<0,05), 1.trimester SIRI değeri 1 birim arttığında, hastalık riskinin %8,3 artacağı gözlenmiştir (OR=0,924). Sonuç: PPROM olan gebelerde ilk trimester SIRI, NLO ve MLO ortalamalarının anlamlı düzeyde yüksek bulunması, bu markerların PPROM' u öngörmede etkili birer biyobelirteç olarak kullanılabileceğini göstermektedir. Hem yenidoğanda kalıcı pek çok sekel oluşturabilecek olması, hem uzamış yenidoğan yoğun bakım ünitesi yatış süreleri hem de maternal ciddi komplikasyonları göz önünde bulundurulduğunda PPROM gelişebilecek gebelerin erken tespitinin son derece önemli olduğu ve literatürde SIRI ile PPROM ilişkisinin henüz yeterince yer bulmamış olması nedeniyle konunun, hasta sayısı daha fazla, çok merkezli, prospektif, randomize kontrollü, geniş çalışmalarla desteklenmesi gerektiği düşüncesindeyiz. Bu konuda SIRI gibi kolay bakılabilecek yeni markerlara ihtiyaç olduğu ve konunun geniş kapsamlı, çok merkezli, prospektif, randomize kontrollü çalışmalar ile desteklenmesinin önemli olduğu düşüncesindeyiz.

Aim: To determine the role of the systemic inflammatory response index, used as an inflammatory marker, in predicting late preterm premature rupture of membranes in pregnant women diagnosed with late preterm premature rupture of membranes. Materials and Methods: Our study was designed as a retrospective analytical cross-sectional study. It is a retrospective descriptive cross-sectional study conducted at Dr. Sami Ulus Maternity and Children's Training and Research Hospital, to evaluate the role of the systemic inflammatory response index (neutrophil x monocyte/lymphocyte) obtained from the first trimester and third trimester hemogram results in predicting late preterm premature rupture of membranes in patients with late preterm premature rupture of membranes. Our hospital is a tertiary education and research hospital. In the research, the files of patients who gave birth at Dr. Sami Ulus Maternity and Children's Training and Research Hospital Gynecology and Obstetrics Clinic, between January 1, 2016 and January 1, 2022 will be scanned. Those who were diagnosed with late preterm premature rupture of membranes between these dates, met the inclusion criteria, were between the ages of 20-40, had a singleton and live pregnancy, gave birth in our hospital, had a routine hemogram checked during their first and third trimester controls, had amniotic fluid pooling in the vagina or Amnisure test. Patients with vaginal fluid discharge (amniotic membrane rupture) and pregnant women who do not have active infection and do not meet the exclusion criteria will constitute the study group (n = 100). Neutrophil x monocyte / lymphocyte ratio (systemic inflammatory response index) will be calculated based on the hemogram values (a routinely requested examination in pregnancy follow-up) examined in the first and third trimesters of the patients in the study group. For each patient included in the study group, a patient between 34-37 weeks of gestation who came to the policlinic routine gestational controls, whose waters had not broken, whose hemograms were checked in the first and third trimesters, who met the inclusion criteria and who did not meet the exclusion criteria, was randomly selected for control. will be included in the group (n=100). The principles stated in the Declaration of Helsinki were followed in the study. The groups will be compared in terms of first trimester and third trimester SIRI values. Findings: There is no statistically significant difference between the groups in terms of age (years), gravida, parity, week of birth, birth weight, Apgar 1st minute and Apgar 5th minute values (p>0.05). The groups are similar in terms of the specified characteristics. When both groups were compared in terms of employment status, education level, spouse's education level, income level, type of birth and gender of the baby, no statistically significant relationship was found. The groups are independent and homogeneous in terms of the specified characteristics. There is no statistically significant relationship between groups and CS indications (p>0.05). A statistically significant difference was detected in terms of 1st trimester SIRI values according to the groups (Z=-3.880; p<0.001). It was determined that the 1st trimester SIRI values of the study group were significantly higher than those in the control group. There is no statistically significant difference in terms of 3rd trimester SIRI values between groups (p>0.05). A statistically significant difference was detected in terms of 1st trimester NLR and MLR values according to the groups (p <0.05). It was determined that the 1st trimester NLR and MLR values of the study group were significantly higher than those in the control group. There is no statistically significant difference in terms of 1st trimester PLR, 3rd trimester NLR, MLR and PLR values according to the groups (p>0.05). A statistically significant difference was detected in terms of 1st trimester hemoglobin and neutrophil values according to groups (p <0.05). It was determined that the first trimester hemoglobin values of the study group were significantly lower than those in the control group. In addition, it was determined that the 1st trimester neutrophil values of the study group were significantly higher than those of the control group. There is no statistically significant difference between the groups in terms of 1st trimester lymphocyte, monocyte, 3rd trimester hemoglobin, lymphocyte, monocyte and neutrophil values (p>0.05). The optimal 1st trimester SIRI cut-off value was determined to be ≥3.3592 with 75.0% sensitivity and 54.0% specificity (AUC=0.659; p<0.05). As a result of the logistic regression analysis performed with the 1st trimester SIRI parameter that is likely to affect the disease state; It was determined that the 1st trimester SIRI parameter was an important parameter affecting the disease status (p<0.05). When the 1st trimester SIRI value increases by 1 unit, the risk of disease will increase by 8.3% (OR = 1.083). There is no statistically significant relationship between the 1st and 3rd trimester SIRI values of the study group and the duration of NICU stay (p>0.05). There is no statistically significant relationship between the 1st and 3rd trimester SIRI values of the control group and the duration of NICU stay (p>0.05). Results: As a result, in our study, the 1st Trimester SIRI parameter was found to be an important parameter affecting late preterm premature membrane rupture (p<0.05). When the 1st trimester SIRI value increases by 1 unit, the risk of disease will increase by 8.3% (OR = 1.083). SIRI; It is a newly identified marker related to inflammation. It has a simple, low-cost and easily accessible calculation that can be obtained from the total blood count. It is observed that there are still few studies on its use in the department of gynecology and obstetrics. It is thought that more meaningful results can be obtained with prospectively planned studies on the subject with a larger patient population.
