Özet
Amaç: Karın ağrısı, sık görülen bir acil servis başvuru nedenidir. Acil servislere geçmeyen karın ağrısıyla başvuran hastaların yönetiminin nasıl olacağı hakkında yeterli çalışma yoktur. Çalışmamızın amacı acil serviste geçmeyen karın ağrısı nedeniyle tekrarlı batın bilgisayarlı tomografisi (BT) çekilen hastaların özelliklerini tanımlamak, BT bulgularını değerlendirerek yüksek ve düşük riskli hasta gruplarını belirlemek ve görüntüleme sonuçlarının hasta yönetimine etkisini göstermektir. Gereç ve Yöntem: Bu retrospektif, tanımlayıcı çalışmaya 01.06.2021-01.06.2023 tarihlerinde Dokuz Eylül Üniversitesi Hastanesi acil servisinde karın ağrısı nedeniyle bir hafta içinde tekrarlı batın BT görüntüleme yapılan hastalar dahil edildi. Hastaların demografik özellikleri, komorbidleri, iki BT arasındaki süre, fizik muayeneleri, laboratuvar verileri, kontrast madde kullanımı, tanıları ve sonlanımları incelendi. Radyolojik değişimlere göre hastalar; Grup 1 (radyolojik değişim olmayan), Grup 2 (iyileşme olan), Grup 3 (kötüleşme olan) ve Grup 4 (yeni patolojik bulgu saptanan) olarak gruplandırıldı. BT bulgularında değişim olmayan veya iyileşme olanlar Grup 1+2 ve kötüleşme veya yeni patolojik bulgu saptananlar Grup 3+4 kabul edildi. Bulgular: Çalışmaya dahil edilen 180 hastanın %51'i erkek ve yaş ortalaması 58±20 idi. Radyolojik değişime göre hastaların %51,7'si Grup 1 (93/180), %10,5'i Grup 2 (19/180), %10'u Grup 3 (18/180) ve %27,8'i Grup 4 (50/180) olduğu saptandı. Yeni ya da kötüleşen BT bulguları olan Grup 3+4 hastaların oranı %37,8 idi. Grup 1+2 ve Grup 3+4 hastaların yaş ve cinsiyetleri arasında fark yoktu (p=0,222, p=0,816). Grup 3+4'teki hastalarda hipertansiyon varlığı Grup 1+2'den daha fazla idi (p=0,002). Grup 3+4 hastalar, Grup 1+2 hastalara göre acil servise daha erken başvurduğu halde gruplar arasında istatistiksel fark yoktu (77 saat ve 96 saat, p=0,308). İlk başvurudaki BT'lerin 112'sinde (%62), ikinci başvuruda 103'ünde (%57) IV kontrast kullanıldığı saptandı. Kontrast kullanım kombinasyonu açısından Grup 1+2 ve Grup 3+4 arasında fark yoktu (p=0,568). İkinci başvuruda FM'de akut batın bulgusu saptanan hastaların çoğunluğu (%64) Grup 3+4'e dahildi (p<0,001). İlk başvurusunda akut batın bulgusu olmayıp, ikinci başvurusunda akut batın bulgusu saptanan hastaların Grup 3+4 olma oranı, her iki başvurusunda da akut batın bulgusu olmayan hastalardan daha yüksekti (sırasıyla %82,6 ve %32,4, p<0,001). Her iki grupta da ikinci başvurudaki CRP ortancası ilk başvuruya göre daha yüksekti (Grup 1+2: p=0,016 ve Grup 3+4: p=0,005). Ayrıca ikinci başvuruda Grup 3+4'ün CRP ortancası Grup 1+2'den daha yüksekti (p=0,001). Tekrarlanan batın BT'de yeni ya da kötüleşen patoloji saptama oranı (Grup 3+4) %37,8 bulundu. Tüm hastalar değerlendirildiğinde tekrarlayan BT sonrası en sık patoloji saptanan sistem intestinal sistemdi (%28). Grup 3+4'teki hastaların en sık tanıları ise ince bağırsak obstrüksiyonu (%19) ve kolesistit (%13) idi. İlk başvuruda NSKA düşünülen 52 hastanın %58'si, maligniteye sekonder ağrı düşünülen 34 hastanın %32'si ikinci başvuruda farklı tanılar aldı. NSKA düşünülen ve ikinci başvuruda tanısı değişen 30 hastanın en sık tanıları ince bağırsak obstrüksiyonu (%13) ve mezenter iskemi (%13) idi. Maligniteye sekonder ağrı düşünülen ve ikinci başvuruda tanısı değişen 11 hastanın en sık tanıları ise kalın bağırsak obstrüksiyonu (%36), perforasyon (%27) ve ince bağırsak obstrüksiyonu (%9) idi. Çalışmaya dahil edilen 180 hastanın %80,5'inin ilk başvurudan sonra taburcu edildiği, %10,6'sının servise yatırıldığı, %3,3'ünün acil cerrahiye alındığı ve %5,6'sının tedaviyi reddederek acil servisten ayrıldığı tespit edildi. İlk başvurusunda taburcu olan 145 hastanın, 73'ü (%50,3) ikinci başvurusunda da taburcu edilirken, 50'si (%34,5) servise yatırıldı, 14'ü (%9,7) acil cerrahiye gönderildi, 6'sı (%4,1) tedavi terk oldu ve 2 (%1,4) hasta ölümle sonuçlandı. İlk başvuruda taburcu olan hastaların %44'ünde sonlanımın değiştiği saptandı. Sonuç: Acil serviste bir hafta içinde geçmeyen karın ağrısıyla tekrarlı BT çekilen hastaların %37,8'inde yeni veya daha kötü bulgular elde edilmiştir. Tekrarlı BT çekimleri sayesinde hastaların %44'ünün sonlanımı değişmiştir. Özellikle intestinal patolojilerden şüphelenilen hastalarda tekrarlı BT görüntülemesi yapılması yararlı olabilir. Geçmeyen karın ağrısı ile başvuran hastalarda tekrarlı BT görüntüleme yapılması hastaların yönetimini değiştirebilir.

Purpose: Abdominal pain is a common reason for emergency department visits. There is insufficient research on the management of patients presenting to emergency departments with persistent abdominal pain. The aim of our study is to describe the characteristics of patients undergoing repeated abdominal computed tomography (CT) scans due to persistent abdominal pain in the emergency department, to identify high and low-risk patient groups by evaluating CT findings, and to demonstrate the impact of imaging results on patient management. Materials and Methods: This retrospective, descriptive study included patients who underwent repeated abdominal CT imaging within one week due to abdominal pain in the emergency department of Dokuz Eylül University Hospital between June 1, 2021, and June 1, 2023. The demographic characteristics, comorbidities, time interval between two CT scans, physical examinations, laboratory data, contrast agent usage, diagnoses, and outcomes of the patients were examined. Based on radiological changes, patients were categorized into Group 1 (no radiological changes), Group 2 (improvement), Group 3 (deterioration), and Group 4 (new pathological findings detected). Patients with no change or improvement in CT findings were considered as Group 1+2, while those showing deterioration or new pathological findings were considered as Group 3+4. Results: Of the 180 patients included in the study, 51% were male, with a mean age of 58±20 years. According to radiological changes, it was determined that 51.7% of the patients were in Group 1 (93/180), 10.5% were in Group 2 (19/180), 10% were in Group 3 (18/180), and 27.8% were in Group 4 (50/180). The rate of patients with new or worsening CT findings in Group 3+4 was 37.8%. There was no difference in terms of age and gender between Group 1+2 and Group 3+4 patients (p=0.222, p=0.816). The presence of hypertension was higher in Group 3+4 patients compared to Group 1+2 (p=0.002). Although patients in Group 3+4 presented to the emergency department earlier than Group 1+2, there was no statistical difference between the groups (77 hours and 96 hours, p=0.308). In 112 of the initial CT scans (62%), IV contrast was used, whereas in 103 of the subsequent scans (57%), IV contrast was used. There was no difference between Group 1+2 and Group 3+4 in terms of contrast agent usage combinations (p=0.568). The majority of patients (64%) with acute abdominal findings during the second visit in the physical examination (FM) were categorized into Group 3+4 (p<0.001). The rate of being in Group 3+4 for patients who did not have acute abdominal findings during their initial visit but developed these findings during their second visit was higher compared to those who did not have acute abdominal findings in both visits (82.6% and 32.4% respectively, p<0.001). The median CRP levels during the second visit were higher in both groups compared to the levels during the first visit (Grup 1+2: p=0.016 and Group 3+4: p=0.005). Additionally, the median CRP level of Group 3+4 during the second visit was higher than that of Group 1+2 (p=0.001). The rate of detecting new or worsening pathology on repeated abdominal CT scans (Group 3+4) was found to be 37.8%. When all patients were evaluated, the most commonly detected pathology after repeated CT scans was related to the intestinal system (28%). Among patients in Group 3+4, the most frequent diagnoses were small bowel obstruction (19%) and cholecystitis (13%). Of the 52 patients initially considered as non-specific abdominal pain (NSAP), 58% received different diagnoses during their second visit, and among the 34 patients with pain suspected to be secondary to malignancy, 32% received different diagnoses during their second visit. Among the 30 patients initially considered as NSAP and whose diagnosis changed during their second visit, the most frequent diagnoses were small bowel obstruction (13%) and mesenteric ischemia (13%). For the 11 patients with pain suspected to be secondary to malignancy and whose diagnosis changed during their second visit, the most frequent diagnoses were colonic obstruction (36%), perforation (27%), and small bowel obstruction (9%). Among the 180 patients included in the study, it was determined that 80.5% were discharged after the initial visit, 10.6% were admitted to the hospital, 3.3% underwent emergency surgery, and 5.6% refused treatment and left the emergency department. Among the 145 patients discharged after the initial visit, 73 (50.3%) were discharged during their second visit, 50 (34.5%) were admitted to the hospital, 14 (9.7%) were referred for emergency surgery, 6 (4.1%) abandoned treatment, and 2 (1.4%) patients resulted in death. It was found that the outcome changed in 44% of patients discharged after the initial visit. Conclusion: New or deteriorating findings were observed in 37.8% of patients who underwent repeated CT scans within one week due to persistent abdominal pain in the emergency department. The outcome changed in 44% of patients due to repeated CT scans. Repeated CT imaging, especially in patients suspected of intestinal pathologies, could be beneficial. Performing repeated CT imaging in patients presenting with persistent abdominal pain may alter patient management.
