Özet
Ortognatik cerrahi daha çok ortodontik tedavi ile giderilemeyen maloklüzyon problemleri ile ilgilenir. Maloklüzyon problemleri genel olarak genetik sebeple ortaya çıksa da enfeksiyon, travma gibi bazı klinik durumlar da ilgili kemik patolojileri sebebiyle maloklüzyon görülebilmektedir. Yine distraksiyon osteogenezinin kraniyofasiyal bölgede uygulanmaya başlamasıyla ortognatik cerrahi uygulanım alanlarını arttırmıştır. Ortognatik cerrahide tarih boyunca çeşitli yöntemler uygulanmışsa da, günümüzde maksilla için altın standart kabul edilen yöntem LeFort I osteotomisidir. Yapılan cerrahiler ile maksillaya uygulanan yeni hareket sonucunda hastaların hava yollarında belirgin değişiklikler oluşabilmektedir.Hava yollarını etkileyen cerrahilerin koku alma duyusuna etkilerine yönelik literatürde çok sayıda çalışma bulunmakta olup, bu çalışmamızda ortognatik cerrahi özelinde bahsi geçen olası etkiler incelenmiştir. Bu çalışmada anabilim dalımız bünyesinde Ekim 2021 – Ağustos 2022 tarihlerinde ameliyat edilen 21 ortognatik cerrahi hastasının, ameliyat öncesi ve ameliyat sonrası 1. ve 6. ayda olmak üzere yapılan testlerle, koku alma duyusunun etkilenme durumu değerlendirilmiştir. Çalışma eşik değer ve identifikasyon testleri üzerinden yapılmıştır. Çalışma toplumun her kesimince karşılaşma ihtimali yüksek olan kahve, vanilya ve kakao kokuları üzerinden yürütülmüş olup bu kokulardan eşik değer bakımından yapılan istatistiksel çalışmalarda vanilya ve kakao kokuları özelinde istatistiksel açıdan anlamlı bir iyileşme sağlandığı görülürken, kahve kokusunda da artış tespit edilse de bu durum istatistiksel olarak anlamlı bulunmamıştır. Kokuların identifikasyon testlerinde ise istatistiksel anlamlı bir değişiklik olmamıştır.

Orthognathic surgery mostly deals with malocclusion problems that cannot be resolved with orthodontic treatment. Although malocclusion problems generally occur due to genetical reasons, some clinical conditions such as infection and trauma can also cause malocclusion due to related bone pathologies. Also, with the implementation of distraction osteogenesis in the craniofacial region, orthognathic surgery has increased its application areas. Although various methods have been used in orthognathic surgery throughout history, the method accepted as gold standart for maxialla today is LeFort I osteotomy. As a result of the new movement applied in the maxilla with the surgeries performed, significant changes can occur in the airways of the patients. There are many studies in the literature on the effects of surgeries affecting the airways on the sense of smell, and in this study, the possible effects mentioned in the orthognatic surgery were evaluated. In this study, the state of being affacted by the sense of smell of 21 orthognathic surgery patients who underwent surgery in our department between October 2021 and August 2022, was evaluated with the test performed before the operation and at the 1st and 6th months after the operation. The study was carried out on threshold value mean scoring and identification tests. The study was carried out on the odors of coffee, vanilla and cocoa, which are more likely to be encountered by all the segments of the society, and statistical studies made in terms of threshold value from these scents showed a statistically significant improvement in vanilla and cocoa scents. Although an increas was detected in the smell of coffee, this situation was not found statistically significant. There was no statistically significant change in the identification tests of the odors.
