Özet
İnfertilite, seksüel aktif çiftlerin yaklaşık %10-20'sini etkiler ve yaklaşık %30-50 erkek kaynaklı nedenler ile ortaya çıkmaktadır. Testislerinde spermatogonyal kök hücre (SKH) olan infertil erkek hastalarda spermatogenezin in vitro koşullarda sağlanarak SKH'lerden fonksiyonel sperm elde edilmesi kendi biyolojik çocuklarına sahip olmak isteyen çiftler için umut verici deneysel yaklaşımlar arasındadır. Özellikle çocukluk çağı kanser hastalarında gonadotoksik kanser tedavisi öncesi SKH içeren dondurulan testis biyopsilerinden in vitro spermatogenez ile sperm eldesi kanser hücre kontaminasyonunu engellemesi açısından oldukça önemlidir. Üç boyutlu (3B) in vitro kültür sistemleri hücre-hücre ve hücre ektraselüler matriks (ESM) etkileşimlerini destekleyerek in vivo ortamı taklit eden sistemlerdir. Bu sistemler ile testiküler germ ve somatik hücrelerin tekrar organize olarak organoid oluşturması veya testis parçalarının organotipik kültürü ile SKH'lerin kök hücre olma özelliklerini ve farklılaşmalarını destekleyecek niş ortamının sağlanması ile SKH'lerin fonksiyonel sperme farklılaşması desteklenebilir. Testis organoid ve organotipik kültür sistemleri in vitro spermatogenezi desteklemekle birlikte bu deneysel yaklaşımların en önemli limitasyonlardan biri kültür besiyerlerinin içerikleri tam olarak bilinmeyen serum veya ksenojik büyüme faktörleri ile desteklenmesidir. Trombositten zengin plazmanın (PRP) testiküler hasar sonrası spermatogenez ve testis yapılanmasında rejeneratif etki gösterdiği ve in vitro çalışmalarda PRP'nin SKH'lerin proliferasyonunu ve kök hücre olma özelliklerini sürdürdüğü bilinmektedir. Aktive PRP testis organoidlerinde SKH havuzunu koruyabilir ve in vitro spermatogenezi destekleyebilir. Bu tez çalışmasında 6 günlük prepubertal C57BL/6 heterojen testiküler hücreler aktive PRP ile desteklenen kültür besiyerinde ultra düşük bağlanma (UDB) ve agaroz temelli hava sıvı interfaz (AT-HSİ) kültür yöntemi ile 6 hafta kültüre edilmiş ve aktive PRP'nin in vitro spermatogeneze etkisi 1, 3, 4 ve 6. haftalarda histomorfometrik ve immunohistokimyasal olarak değerlendirilmiştir. %10 PRP ile desteklenen testis organoidlerinde SKH havuzu 6 hafta korunmuş ve 3, 4, ve 6. haftalarda %10 KSR ve %5 FBS+%5 KSR ile desteklenen testis organoidlere göre ID4(+) SKH sayısı daha yüksek bulunmuştur. %10 PRP ile desteklenen testis organoidlerinde SCP3(+) spermatosit saysı 1. haftadan 6 haftaya kadar sabit kalmış ve 4. ve 6. haftalarda 10 KSR ve %5 FBS+%5 KSR ile desteklenen testis organoidlere göre SCP3(+) spermatosit sayısı daha yüksek gözlemlenmiştir. %10 KSR, %5 FBS+%5 KSR ve %10 PRP ile desteklenen testis organoidlerinde 3. haftada ACR(+) yuvarlak spermatidlerin varlığı ortaya konmuş ve 4. haftada %10 PRP ile desteklenen testis organoidlerinde ACR(+) spermatid sayısının %10 KSR ile desteklenen testis organoidlerine göre daha yüksek olduğu görülmüştür. Sonuç olarak, bu tez çalışmasında aktive PRP'nin in vitro spermatogeneze olumlu etkisi ortaya konduğundan, PRP'nin kişinin kendi kanından basit teknik ile elde edilerek potansiyel immunolojik reaksiyon ve viral kontaminasyon riski oluşturmadan infertilite tedeavisinde güvenli bir kültür platform olarak kullanılabileceği saptanmıştır.

Infertility affects 10-20% of sexually active couples and male-related factors contribute about 30-50% of these cases. Obtaining functional sperm from spermatogonial stem cells (SSC) by in vitro spermatogenesis in infertile male patients is the promising experimental approache for couples who want to have their own biological children. Especially in childhood cancer patients, since spermatogenesis has not started, obtaining sperm from frozen testicular biopsies before treatment by in vitro spermatogenesis is very important in terms of eliminating cancer cell contamination. Three-dimensional (3D) in vitro culture systems mimic in vivo environment by cell-cell and cell extracellular matrix (ECM) interactions. Germ cells and somatic cells can reorganize to form testis organoid or culture of testicular pieces support differentiation of SSCs into functional sperm by 3D culture systems. Although testicular organoid and organotypic culture systems support in vitro spermatogenesis, one of the most important limitations of these experimental approaches is that the culture media are supplemented with undefined nature of serum and/or xenogenic growth factors. It is known that PRP has a regenerative effect on spermatogenesis and testicular regeneration after testicular damage and PRP maintains the proliferation and function of SSCs in vitro. Activated PRP can preserve the SSC pool in testicular organoids and support spermatogenesis in vitro. In this thesis study, 6-day-old prepubertal C57BL/6 heterogeneous testicular cells were cultured in culture medium supplemented with activated PRP by ultra-low attachment (UDB) and agarose-based air-liquid interphase (ALİ) culture methods for 6 weeks, and the effect of activated PRP on in vitro spermatogenesis was evaluated histomorphometrically and immunohistochemically at 1, 3, 4 and 6 weeks. In testicular organoids supported with 10% PRP, the SSC pool was preserved for 6 weeks and the number of ID4(+) SSCs was found to be higher than in testicular organoids supported with 10% KSR and 5% FBS+5% KSR at 3, 4 and 6 weeks. In testicular organoids supported with 10% PRP, the number of SCP3(+) spermatocytes remained constant from week 1 to 6 and higher than in testicular organoids supported with 10% KSR and 5% FBS+5% KSR at 4 and 6 weeks. In testicular organoids supported with 10% KSR, 5% FBS+5% KSR and 10% PRP, the pres ACR(+) round spermatids was observed at week 3 and the number of ACR(+) spermatids was higher in testicular organoids supported with 10% PRP compared to testicular organoids supported with 10% KSR at week 4. In conclusion, the overall data demonstrated the positive effect of activated PRP on in vitro spermatogenesis. Thus, activated PRP obtained from the person's own blood with a simple technique can be used as a safe culture platform in the the teratment of infertility without risk of potential immunological reaction and viral contamination.
