Özet
Küresel epidemi olarak bahsedilen metabolik sendrom (MetS), insülin direnci, yüksek kolesterol seviyesi ve hipertansiyon gibi birkaç faktörün bir arada bulunmasıyla isimlendirilen multifaktöriyel bir durumdur. MetS'nin erkek üreme sistemi üzerine olumuz etkilerini araştıran çalışmalar da bulunmaktadır. MetS'nin etki yolaklarından birisi endoplazmik retikulum stresininin (ERS) tetiklenmesidir. ERS, yanlış katlanmış ya da katlanmamış proteinlerin birikimi sonucu protein homeostazının bozulmasıyla meydana gelmektedir. ERS'ye cevap olarak PERK, IRE1 ve ATF6 genlerinin rol oynadığı katlanmamış protein yanıtı (UPR) yolakları aktifleşmektedir. UPR aktivasyonu sonucu stresin ağırlığına bağlı olarak ya ERS ortadan kaldırılarak homeostaz sağlanmakta ya da apoptoz indüklenmektedir. Diğer yandan, yapılan çalışmalarda D vitamini (VitD) uygulamasının MetS'nin olumsuz etkilerini düzeltici etki gösterdiği saptanmıştır. Bu tez çalışmasının amacı MetS modeli oluşturulan sıçanların testis dokusunda VitD'nin ERS üzerine etkisinin incelenmesidir. Çalışmada dört gruba ayrılan (sağlıklı kontrol grubu (SK), sağlıklı+VitD uygulanmış grup (SKD), MetS'li grup (MS) ve MetS+VitD uygulanmış grup (MSD)) sıçanlar 15. hafta sonunda sakrifiye edildi. Testis dokularından total RNA izolasyonu ve cDNA sentezi yapıldı. PERK, IRE1, ATF6, ATF4, CHOP, GRP78, ERP57, CYP19A1 ve VDR genlerinin ekspresyon analizi kantitatif gerçek zamanlı polimeraz zincir reaksiyonu (qRT-PCR) yöntemi ile yapıldı. Bilgimiz dahilinde, MetS olgularında testis dokularında gelişen ERS'nin infertilite ve VitD ilişkisini araştıran bir çalışma bulunmamaktadır. Bu tez çalışmasından elde edilen bulgular, VitD'nin IRE1 yolağı üzerinden ERS'yi baskıladığı ve PERK ve CYP19A1 seviyesindeki değişimlerle testiküler disfonksiyonu düzelttiği yönündedir. Sonuçlar, MetS kaynaklı erkek infertilitesinde VitD takviyesinin bir tedavi ya da tedaviye ek olarak önemli bir katkı sağlayabileceğini öne sürmektedir.

Metabolic syndrome (MetS), a global epidemic, is a multifactorial condition characterised by a combination of several factors such as insulin resistance, high cholesterol levels and hypertension. There are also studies investigating the adverse effects of MetS on the male reproductive system. One of the pathways of MetS is the induction of endoplasmic reticulum stress (ERS). ERS occurs when protein homeostasis is disrupted by the accumulation of misfolded or unfolded proteins. In response to ERS, unfolded protein response (UPR) pathways involving PERK, IRE1 and ATF6 genes are activated. As a result of UPR activation, depending on the severity of stress, either ERS is eliminated and homeostasis is maintained or apoptosis is induced. On the other hand, studies have shown that vitamin D (VitD) administration has a corrective effect on the negative effects of MetS. The aim of this thesis study was to investigate the effect of VitD on ERS in testicular tissue of rats with MetS model. The rats divided into four groups (healthy control group (SK), healthy+VitD treated group (SKD), MetS group (MS) and MetS+VitD treated group (MSD)) were sacrificed at the end of 15th week. Total RNA isolation and cDNA synthesis were performed from testicular tissues. Expression analyses of PERK, IRE1, ATF6, ATF4, CHOP, GRP78, ERP57, CYP19A1 and VDR genes were performed by quantitative real-time polymerase chain reaction (qRT-PCR). To the best of our knowledge, there is no study investigating the relationship between ERS in testicular tissues of MetS patients and infertility and VitD. The findings of this thesis suggest that VitD suppresses ERS via the IRE1 pathway and improves testicular dysfunction through changes in PERK and CYP19A1 levels. The results suggest that VitD supplementation may make an important contribution as a treatment or adjunct to treatment in MetS-induced male infertility.
