Özet

Konjenital penil kurvatur (KPK), doğuştan gelen ve genellikle ergenlik döneminde fark edilen bir penil eğrilik durumudur. Bu durum, penisin normal cinsel fonksiyonunu olumsuz etkileyebilir ve hastaların yaşam kalitesini düşürebilir. KPK'nin patofizyolojisi tam olarak anlaşılmamış olup, tunika albuginea dokusunda fibrozis ve genetik faktörlerin önemli rol oynayabileceği düşünülmektedir. Bu çalışmada, RHOA, ROCK1 ve ROCK2 genlerinin KPK'deki rolünü incelemek amaçlanmıştır. Çalışmamıza 15 KPK hastası ve 37 kontrol grubu (Peyronie hastalığı ve erektil disfonksiyon hastası) hastası dahil edilmiştir. Hastalardan alınan tunika albuginea doku örnekleri, gen ekspresyon seviyelerini belirlemek için kantitatif polimeraz zincir reaksiyonu (qPZR) yöntemi kullanılarak analiz edilmiştir. İstatistiksel analizler, gen ifadelerinin gruplar arasındaki farklılıklarını belirlemek amacıyla yapılmıştır. KPK grubunun yaş ortalaması kontrol grubunun yaş ortalamasından daha düşük bulunmuştur (p<0,001). Erektil disfonksiyon, hipertansiyon ve diyabetes mellitus kontrol grubunda daha yüksek oranda gözlenmiştir (p<0,05). ROCK2 gen ifadesi, KPK hastalarında kontrol grubuna göre anlamlı derecede yüksek bulunmuştur (p=0.010). Diğer yandan, RHOA ve ROCK1 gen ifadeleri gruplar arasında anlamlı bir fark göstermemiştir. Çalışmamızın bulguları, ROCK2'nin KPK gelişiminde önemli bir rol oynayabileceğini ve bu genin gelecekteki terapötik müdahaleler için potansiyel bir hedef olabileceğini düşündürmektedir. Ek olarak RHOA ve ROCK1 gen ifadelerinin KPK üzerinde belirgin bir etkisi olmadığı görülmektedir. Çalışmamızın bulguları, KPK'nin moleküler mekanizmalarının daha iyi anlaşılmasına ve tedavi stratejilerinin geliştirilmesine katkı sağlayabilir.

Congenital penile curvature (CPC) is a congenital condition typically noticeable during adolescence, characterized by penile curvature. This condition can negatively impact the normal sexual function of the penis and reduce the quality of life for patients. The pathophysiology of CPC is not fully understood, but fibrosis and genetic factors in the tunica albuginea tissue are thought to play significant roles. This study aims to investigate the roles of the RHOA, ROCK1, and ROCK2 genes in CPC. Our study included 15 CPC patients and 37 control group patients (patients with Peyronie's disease and erectile dysfunction). Tissue samples of the tunica albuginea obtained from the patients were analyzed using quantitative polymerase chain reaction (qPCR) to determine gene expression levels. Statistical analyses were performed to identify differences in gene expressions between the groups. The mean age of the CPC group of patients was found to be lower than the mean age of the control group (p<0.001). Erectile dysfunction, hypertension, and diabetes mellitus were observed at higher rates in the control group (p<0.05). ROCK2 gene expression was found to be significantly higher in CPC patients compared to the control group (p=0.010). On the other hand, RHOA and ROCK1 gene expressions did not show significant differences between the groups. The results of our study suggests that ROCK2 may play an important role in the development of CPC and that this gene could be a potential target for future therapeutic interventions. Additionally, the expressions of RHOA and ROCK1 genes do not appear to have a significant impact on CPC. Our findings may contribute to a better understanding of the molecular mechanisms underlying CPC and the development of treatment strategies.
