Özet
Amaç: Bu çalışma posterior torakolomber spinal füzyon operasyonlarında klasik Total İntravenöz Anestezi (TİVA) yöntemi ile bu yönteme lidokain ve ketamin infüzyonlarının eklenmesiyle oluşturulan multimodal anestezi protokollerinin postoperatif ağrı yönetimi, opioid tüketimi ve cerrahi iyileşme parametreleri üzerindeki etkilerini karşılaştırmayı amaçlamaktadır. Gereç ve Yöntem: Etik kurul onayı sonrası çalışmaya katılmayı kabul eden, hastanemiz Beyin ve Sinir Cerrahisi kliniğinde posterior torakolomber spinal füzyon operasyonu geçiren 72 hasta çalışmaya dahil edildi. Hastalar, grup S, klasik TİVA yöntemi (propofol ve remifentanil) uygulanan hastalar; grup K, klasik TİVA'ya ketamin eklenen hastalar; grup L ise klasik TİVA'ya lidokain eklenen hastalar olmak üzere 3 gruba ayrıldı. İntraoperatif dönemde ortalama kan basıncı, kalp atım hızı ve kullanılan anestezik ilaçların dozları kaydedildi. Postoperatif dönemde ise visual ağrı skoru, opioid tüketimi, ilk ambulasyon zamanı, gaz-gaita çıkışı ve postanestezik bakım ünitesinde kalış süresi ayrıca bulantı-kusma ve sedasyon skorları not edildi. Bulgular: Postoperatif opioid tüketimi açısından, ketamin ve lidokain infüzyonu eklenen grupların, klasik TİVA grubuna göre daha düşük opioid ihtiyacı gösterdiği tespit edildi (p<0.001). Gruplar arasında intraoperatif kullanılan propofol miktarında istatistiksel olarak anlamlı fark vardı. Ketamin kullanılan grupta propofol dozu, diğer gruplara kıyasla belirgin şekilde daha yüksek bulundu (p=0.001). İntraoperatif ve postoperatif hemodinamik parametreler açısından gruplar arasında anlamlı bir farklılık tespit edilmedi. Cerrahi iyileşme parametreleri, sedasyon skoru ve bulantı-kusma oranları açısından da gruplar arasında istatistiksel olarak anlamlı bir fark gözlenmedi. Tartışma: Çalışmamızda Multimodal anestezi protokollerinin klasik TİVA yöntemi ile karşılaştırıldığında postoperatif opioid tüketimini anlamlı ölçüde azalttığı, özellikle ağrı yönetiminde etkili olduğu ancak, postoperatif iyileşme parametrelerinde anlamlı bir iyileşme sağlamadığını gözlemledik. Literatürde multimodal anestezi protokollerinin opioid tüketimini azalttığı ve yan etkileri sınırladığına dair çalışmalar mevcuttur. Bununla birlikte, bazı çalışmalarda ketaminin anestezi derinliği ve bispektral indeks(BİS) monitörizasyonu üzerinde yanıltıcı etkiler oluşturabileceği ve propofol dozunun gereksiz yere artırılmasına neden olabileceği gösterilmiştir. Sonuç: Çalışmamızda lidokain ve ketamin eklenen protokollerde opioid ihtiyacı azaldı, ancak iyileşme parametreleri üzerinde anlamlı bir fark tespit edilemedi. Ayrıca ketamin grubunda intraoperatif kullanılan total propofol dozunun klasik TİVA grubuna kıyasla anlamlı daha yüksek olduğu bulundu.

Aim: This study aims to compare the effects of multimodal anesthesia protocols, which include the classical Total Intravenous Anesthesia (TIVA) method with the addition of lidocaine and ketamine infusions, on postoperative pain management, opioid consumption, and surgical recovery parameters in patients undergoing posterior thoracolumbar spinal fusion surgeries. Materials and Methods: Following ethical committee approval, 72 patients who agreed to participate in the study and underwent posterior thoracolumbar spinal fusion surgery in our hospital's Neurosurgery clinic were included. Patients were divided into three groups: group S (control), which received classical TIVA (propofol and remifentanil); group k, which received classical TIVA with the addition of ketamine; and group L, which received classical TIVA with the addition of lidocaine. During the intraoperative period, mean arterial pressure, heart rate, and doses of anesthetic drugs used were recorded. Postoperatively, visual pain score, opioid consumption, time to first ambulation, gas and stool passage, duration of stay in the post-anesthesia care unit, as well as nausea-vomiting and sedation scores were also documented. Results: In terms of postoperative opioid consumption, it was observed that the groups receiving ketamine and lidocaine infusions had significantly lower opioid requirements compared to the classical TIVA group (p<0.001). There was a statistically significant difference in the amount of propofol used intraoperatively among the groups. The ketamine group showed a significantly higher propofol dosage compared to the other groups (p=0.001). No significant differences were found among the groups regarding intraoperative and postoperative hemodynamic parameters. Additionally, there were no statistically significant differences in surgical recovery parameters, sedation scores, and nausea-vomiting rates among the groups. Discussion: Our study observed that multimodal anesthesia protocols significantly reduced postoperative opioid consumption compared to classical TIVA, particularly in pain management; however, there was no significant improvement in postoperative recovery parameters. Literature supports that multimodal anesthesia protocols can reduce opioid consumption and limit side effects. Nonetheless, some studies have indicated that ketamine may produce misleading effects on anesthesia depth and bispectral index (BIS) monitoring, leading to unnecessary increases in propofol dosage. Conclusion: In our study, the addition of lidocaine and ketamine in the protocols resulted in reduced opioid requirements; however, no significant differences were found in recovery parameters. Furthermore, it was noted that the total propofol dosage used intraoperatively in the ketamine group was significantly higher compared to the classical TIVA group.
