Özet
Amaç: Birinci trimester fetal hepatik arter ve duktus venozus doppler incelemesi ile gestasyonel diyabetes mellitus ve makrozomiyi öngörmede prediktif değerin bulunması ve hepatik arter doppler parametlerinin neonatal sonuçlar ile korelasyonu değerlendirilecektir. Metod: Çalışmada 3 hasta grubu incelencektir. 1.grup pregestasyonel diyabet (tip1 ve tip 2 dm). 2. Grup kontrol grubu hastaların içinden gestasyonel diyabet tespit edilen hastalar. 3. grup rutin kontroldeki komplike olmayan gebe hastaları içerecektir.US incelemelerinin tümü, 3.5 MHz konveks dönüştürücülü GE VOLUSON E6 ile 11-14 ve 18-22. gebelik haftalarında yapılacaktır. Bu haftalarda fetal hepatik arter ve duktus venozus tepe sistolik hız (PSV), diyastol sonu hız, pulsatilite indeksi, direnç indeksi kayıt edilecektir.Kliniğimize başvuran hastalarımıza rutin gebelik kontrolünün bir parçası olarak ogtt testi yapılacaktır. IADPSG önerdiği klavuza gore 24-28. haftalarda 75 g glikoz kullanılarak açlık durumunda OGTT yapılır. Tanı, ≥ 92 mg/dl (≥ 5,2 mmol/l) veya 1 -saat ≥ 180 mg/dl (≥ 10 mmol/l) veya 2-saat ≥ 153mg/dl (≥ 8,5 mmol/l) durumlarından birinin sağlanması durumunda GDM tanısı konulur. Bulgular: Çalışmaya 11 pregestasyonel DM, 21 gestasyonel DM ve 83 sağlıklı gebe olmak üzere toplam 115 kadın dahil edilmiştir. Pregestasyonel grupta olanların yaş ortalaması 27,64±7,66 olarak, gestasyonel DM olanların yaş ortalaması 29,95±4,46 olarak ve kontrol grubunda olanların yaş ortalaması 27,39±6,09 olup aralarında anlamlı farklılık görülmemiştir (p=0,118). Gruplar arasında BMI açısından anlamlı farklılık görülmüş olup bu farklılık kontrol grubu ile diğer iki grup arasındaki farktan kaynaklanmış olup kontrol grubunun BMI değeri daha düşüktür (p<0,001). Gruplar arasında doğum kilosu açısından anlamlı farklılık görülmüştür ve bu farklılık kontrol grubu ile diğer iki grup arasındaki farktan kaynaklanmıştır (p<0,001). Gruplar arasında gravida (p=0,491), parite (p=0,465), yaşayan (p=0,346) ve abortus (p=1,000) sayısı açısından anlamlı farklılık görülmemiştir (Tablo 6.). Pregestasyonel DM grubunda olanların 1'i (%9,1) exitus olmuş iken diğer iki grupta exitus olan yoktur ve gruplar arasında exitus varlığı açısından anlamlı farklılık görülmemiştir (p=0,096)(Tablo 7).Gruplar arasında hafta 1 (p=0,456) ve hafta 2 (p=0,643) açısından anlamlı farklılık görülmemiştir (Tablo 8).Gruplar arasında HA-PSV değerinin ilk ölçümü açısından anlamlı farklılık görülmüş ve bu farklılık sadece gestasyonel grup ile kontrol grubu arasındaki farktan kaynaklanmıştır (p=0,001). Gruplar arasında DV-PI (p<0,001) ve DV-S (p<0,001) değerinin ilk ölçümü açısından anlamlı farklılık görülmüştür ve bu farklılık kontrol grubu ile diğer iki grup arasındaki farktan kaynaklanmıştır. Gruplar arasında DV-RI değerinin ilk ölçümü açısından anlamlı farklılık görülmüştür ve bu farklılık Pregestasyonel DM grubu ile diğer iki grup arasındaki farktan kaynaklanmıştır (p=0,015)(Tablo 9). Gruplar arasında HA-PI (p<0,001), HA-PSV (p<0,001), DV-PI (p<0,001), DV-RI (p<0,001) ve DV-S (p<0,001) değerinin ikinci ölçümü açısından anlamlı farklılık görülmüştür ve bu farklılık kontrol grubu ile diğer iki grup arasındaki farktan kaynaklanmıştır (Tablo 10).Gruplar arasında HAPI (p<0,001), HAPSV (p<0,001), DVPI (p<0,001) değerinin değişimi açısından anlamlı farklılık görülmüştür ve bu farklılık kontrol grubu ile diğer iki grup arasındaki farktan kaynaklanmıştır. Gruplar arasında DVRI değerinin değişimi açısından anlamlı farklılık görülmüştür ve bu farklılık gestasyonel DM grubu ile kontrol grubu arasındaki farktan kaynaklanmıştır (p<0,001). Gruplar arasında DVS değerinin değişimi açısından anlamlı farklılık görülmüştür ve bu farklılık pregestasyonel DM grubu ile gestasyonel DM grubu arasındaki farktan kaynaklanmıştır (p=0,042)(Tablo 11). Tüm katılımcıların HAPI, HAPSV ve DVS değeri sonrasında öncekine göre anlamlı şekilde artmış iken; HARI, DVPI ve DVRI değeri ise anlamlı şekilde düşmüştür (p<0,001)(Tablo 12). Çeşitli değerlerin gestasyonel DM'yi predikte edebilmeleri ROC analizi ile araştırılmış ve cut-off değerleri belirlenmiştir. HA-PSV için 10,69 değeri cut-off olarak alındığında %47,6 sensitivite, %96,4 spesifite saptanmış ve iyi bir belirleyici olduğu görülmüştür. DV-PI için 0,94 değeri cut-off olarak alındığında %100 sensitivite, %100 spesifite saptanmış ve iyi bir belirleyici olduğu görülmüştür. DV-S için 28 değeri cut-off olarak alındığında %85,7 sensitivite, %88 spesifite saptanmış ve iyi bir belirleyici olduğu görülmüştür (Tablo 13, Şekil 1-3). Doğum kilosu ile HAPSV, DVPI ve DVS arasında negatif yönde anlamlı bir korelasyon görülmüştür (Tablo 14). Sonuç: Pregestasyonel ve gestasyonel diyabetle komplike gebelik sahibi olmak ve makrozomik yenidoğan sahibi olmak özellikle takipsiz gebeliklerde, gebelik seyri ve doğum eylemi sırasında pek çok komplikasyonu içermektedir. Bu yüzden birinci trimesterda ölçülebilecek objektif bir Doppler parametresinin tespiti pregestasyonel ve gestasyonel diyabeti ve komplikasyonlarını yönetmek açısından yol gösterici olabilir. Çalışmamızın örneklem büyüklüğünün az olması ve neonatal sonuçlar ile korelasyonlarının değerlendirilmemiş olması kısıtlılıklarıdır. Erken haftalarda meydana gelen HA ve DV akım değişikliklerinin fetal makrozomi ve gestasyonel diyabet öngörüsünde kullanılabilirliğinin değerlendirilmesi amacıyla daha büyük hasta grubu ile yapılan çalışmalarla bu bulguların desteklenmesi gereklidir. Çalışmamız diyabet ile komplike gebelerde fetal hepatik arter ve duktus venozus akımlarının gestasyonel diyabet ve pregestasyonel diyabet Doppler parametrelerinin incelendiği literatürdeki ilk çalışmadır. Literatürde bu alanda daha önce çalışma bulunmamakla birlikte bulgularımız bu alanın gelecek vaadettiğini göstermektedir. Daha geniş örneklem büyüklüğü ve ek çalışmalarla bulgularımız desteklenmelidir.

Aim: The predictive value of first trimester fetal hepatic artery and ductus venosus Doppler examination in predicting gestational diabetes mellitus and macrosomia and the correlation of hepatic artery Doppler parameters with neonatal outcomes will be evaluated. Method. Three patient groups will be examined in the study. Group 1 pregestational diabetes (type 1 and type 2 DM). Group 2: Patients diagnosed with gestational diabetes among the control group patients. Group 3 will include uncomplicated pregnant patients on routine follow-up. All US examinations were performed on days 11-14 and 18-22 with GE VOLUSON E6 with 3.5 MHz convex transducer. It will be done during the pregnancy weeks. During these weeks, fetal hepatic artery and ductus venosus peak systolic velocity (PSV), end-diastolic velocity, pulsatility index, and resistance index will be recorded. Our patients who apply to our clinic will have an OGTT test as part of their routine pregnancy control. 24-28 according to the guideline recommended by IADPSG. OGTT is performed in a fasting state using 75 g glucose for weeks. Diagnosis is made from ≥ 92 mg/dl (≥ 5.2 mmol/l) or 1-hour ≥ 180 mg/dl (≥ 10 mmol/l) or 2-hour ≥ 153mg/dl (≥ 8.5 mmol/l). If either one is met, GDM is diagnosed. Bulgular: A total of 115 women, including 11 pregestational DM, 21 gestational DM and 83 healthy pregnant women, were included in the study. The average age of those in the pregestational group was 27.64±7.66, the average age of those with gestational DM was 29.95±4.46, and the average age of those in the control group was 27.39±6.09, and there was no significant difference between them (p = 0.118). . There was a significant difference between the groups in terms of BMI, and this difference resulted from the difference between the control group and the other two groups, and the BMI value of the control group was lower (p<0.001). There was a significant difference in birth weight between the groups, and this difference was due to the difference between the control group and the other two groups (p <0.001). There was no significant difference between the groups in terms of gravida (p=0.491), parity (p=0.465), number of survivors (p=0.346) and abortion (p=1.000) (Table 6.). While one of the patients in the pregestational DM group (9.1%) had exitus, there was no exitus in the other two groups and there was no significant difference between the groups in terms of the presence of exitus (p = 0.096) (Table 7). Week 1 between the groups (p = 0.456) and week 2 (p=0.643) (Table 8). There was a significant difference between the groups in terms of the first measurement of HA-PSV value, and this difference was due only to the difference between the gestational group and the control group (p=0.001). There was a significant difference between the groups in terms of the first measurement of DV-PI (p<0.001) and DV-S (p<0.001) values, and this difference was due to the difference between the control group and the other two groups. There was a significant difference between the groups in terms of the first measurement of the DV-RI value, and this difference resulted from the difference between the Pregestational DM group and the other two groups (p = 0.015) (Table 9). In terms of the second measurement of HA-PI (p<0.001), HA-PSV (p<0.001), DV-PI (p<0.001), DV-RI (p<0.001) and DV-S (p<0.001) values between the groups A significant difference was observed and this difference resulted from the difference between the control group and the other two groups (Table 10). While HAPI, HAPSV and DVS values of all participants increased significantly after compared to before; HARI, DVPI and DVRI values decreased significantly (p<0.001) (Table 12). The ability of various values to predict gestational DM was investigated by ROC analysis and cut-off values were determined. When the value of 10.69 was taken as cut-off for HA-PSV, 47.6% sensitivity and 96.4% specificity were found and it was found to be a good predictor. When the value of 0.94 was taken as the cut-off for DV-PI, 100% sensitivity and 100% specificity were determined and it was found to be a good predictor. When the value of 28 was taken as cut-off for DV-S, 85.7% sensitivity and 88% specificity were found and it was seen to be a good predictor (Table 13, Figure 1-3). A significant negative correlation was observed between birth weight and HAPSV, DVPI and DVS (Table 14). Conclusion: Having a pregnancy complicated by pregestational and gestational diabetes and having a macrosomic newborn involves many complications during the course of pregnancy and labor, especially in uncontrolled pregnancies. Therefore, the determination of an objective Doppler parameter that can be measured in the first trimester may be guiding in terms of managing pregestational and gestational diabetes and its complications. The limitations of our study are the small sample size and the fact that its correlations with neonatal outcomes were not evaluated. These findings need to be supported by studies conducted with a larger patient group in order to evaluate the usability of HA and DV flow changes occurring in the early weeks in predicting fetal macrosomia and gestational diabetes. Our study is the first study in the literature to examine the gestational diabetes and pregestational diabetes Doppler parameters of fetal hepatic artery and ductus venosus flows in pregnant women complicated with diabetes. Although there is no previous study in this field in the literature, our findings show that this field is promising. Our findings should be supported with larger sample sizes and additional studies.
