Özet
Amaç: Çalışmamızda kanser hastalarında nütrisyonel durumdan ciddi bir şekilde etkilenen albümin ve mutlak lenfosit sayısı kullanılarak hesaplanan prognostik nütrisyonel indeksin (PNI)'nın Hodgkin lenfoma hastalarında prognoz üzerine etkisini araştırmayı amaçladık. Yöntem: Son 10 yılda hastanemiz hematoloji kliniğine başvuran Hodgkin Lenfoma tanılı 95 hasta çalışmamıza dahil edildi. Hastaların PNI değeri: PNI=albümin (g/L) + 0.005 x mutlak lenfosit sayısı (mm3) formülü ile hesaplandı. İstatistiksel değerlendirmede kategorik verilere sahip iki grubun karşılaştırılmasında Pearson ki kare testi ve Sperman Rank korelasyon katsayısı kullanıldı. Sayısal verilere sahip iki grubun karşılaştırılmasında normal dağılıma uyanlar için Student T Testi, uymayanlar için Mann Whitney U testi, PNI için cut-of belirlemede ROC curve analizi kullanıldı. Bulgular: HL tanısı alan 95 hastanın ortanca yaşı 42 olup (18-77), 61'i (%64) erkek, 34'ü (%36) kadındı. Düşük PNI skorlu grupta OS 60 ay (45-74 ay), yüksek PNI skoru olanların ise OS 115 ay (108-122 ay) olup istatistiksel anlamlı şekilde düşük PNI skoruna sahip olanlarda sağkalım daha kısaydı (p=0.0001). PFS açısından incelendiğinde de düşük PNI Skorlu grupta PFS 56 ay (40-73 ay), Yüksek PNI grubunda 114 ay (107-121 ay) olup istatistiksel anlamlı şekilde düşük PNI skoruna sahip olanlarda PFS daha kısa bulundu (p=0.0001). PNI skoru ve IPS evresi OS açısından multivariete analiz ile değerlendirildiğinde istatistiksel anlamlı şekilde OS için prediktör oldukları görüldü (p=0.05, p=0.002). PFS açısından bakıldığında ise; PNI skoru ve IPS skoru istatistiki anlamlılığını koruyordu, (p=0.003, p=0.05). Sonuçlar: Çalışmamızda Hodgkin lenfomalı düşük PNI skoruna sahip hasta grubunda daha düşük Toplam Sağkalım (OS) ve Progresyonsuz Sağkalım (PFS) değerleri olduğu bulundu. PNI skorunun OS ve PFS için prognozu öngörmede etkisi gösterildi.

Aim: In our study, we aimed to investigate the effect of prognostic nutritional index (PNI) on the prognosis of patients with Hodgkin's lymphoma, which is calculated using albumin and absolute lymphocyte count, which are significantly influenced by nutritional status in cancer patients. Methods: A total of 95 patients diagnosed with HL who were admitted to the hematology clinic of our hospital in the last 10 years were included in the study. The PNI value of the patients was calculated using the formula: PNI = albumin (g/L) + 0.005 x absolute lymphocyte count (per mm³). For statistical analysis, the Pearson chi-square test and Spearman rank correlation coefficient were used to compare two groups with categorical data. When comparing two groups with numerical data, Student's T-test was used for those that fit the normal distribution and Mann Whitney U-test for those that did not. ROC curve analysis was used to determine the cut-off for PNI Results: The median age of the 95 patients diagnosed with Hodgkin lymphoma was 42 years (18–77), with 61 (64%) being male and 34 (36%) female. In the low PNI score group, OS was 60 months (45–74 months), while in the high PNI score group, it was 115 months (108–122 months). Survival was significantly shorter in patients with a low PNI score (p=0.0001). Regarding PFS, the low PNI score group had a PFS of 56 months (40–73 months), whereas the high PNI score group had a PFS of 114 months (107–121 months), showing that PFS was also significantly shorter in patients with a low PNI score (p=0.0001). When PNI score and IPS stage were evaluated through multivariate analysis for their impact on OS, both were found to be statistically significant predictors of OS (p=0.05, p=0.002). Similarly, for PFS, both PNI score and IPS stage remained statistically significant (p=0.003, p=0.05). Conclusion: In our study, it was found that the patient group with low PNI scores in Hodgkin lymphoma had lower Overall Survival (OS) and Progression-Free Survival (PFS) rates. The impact of the PNI score on predicting prognosis for OS and PFS was demonstrated.
