Özet

Amaç: Bu çalışma diz artroplasti ameliyatlarında istenmeyen perioperatif hipotermi insidansı ve ilişkili risk faktörlerinin incelenmesi amacıyla yapıldı. Yöntem: Tanımlayıcı tipteki bu çalışma, 18.10.2021-04.01.2023 tarihleri arasında, Ege bölgesinde bulunan bir üniversite hastanesinin Ortopedi ve Travmatoloji Klinik' inde yürütüldü. Araştırmanın örneklemini, Ortopedi ve Travmatoloji Kliniğinde, diz artroplasti ameliyatı geçiren araştırmaya katılmaya gönüllü olan 18 yaş ve üzeri 227 hasta oluşturdu. Araştırmanın verileri araştırmacılar tarafından hazırlanan sosyo-demografik özellikler ve istemeyen perioperatif hipotermi (İPH) durumuna ilişkin soruların yer aldığı veri toplama formu kullanıldı. Bu formda 136 soru bulunmaktadır. Bu formda demografik özellikleri içeren 26 soru, ameliyat öncesi hasta verilerine ilişkin 27, ameliyat sırasında cerrahi işlem ile ilgili 41 soru, ameliyat sonrası işlem ile ilgili 42 soru bulunmaktadır. Araştırma verilerinin analizinde ise“Statistical Package for the Social Sciences (SPSS) 26.0 programı”kullanıldı. Tanımlayıcı istatistikler birim sayısı (n), yüzde (%), ortalama±standart sapma ve medyan değerleri olarak verildi. Sayısal değişkenlere ait verilerin normal dağılımı Shapiro Wilk normallik testi ile değerlendirildi. İki grubun karşılaştırılmasında verilerin normal dağılması durumunda bağımsız örneklerde t test, verilerin normal dağılmaması halinde Mann Whitney U Testi kullanıldı. Sayısal değişkenler arasındaki ilişkiler Spearman korelasyon katsayısı ile değerlendirildi. Buşgular: Araştırmaya katılan DA ameliyatı geçiren hastaların ameliyat sırasında %60.4' ünde, ameliyat sonrasında %20.7'sinde İPH insidansı olduğu bulundu. Ameliyat öncesi dönemde İPH gözlemlenmedi. Bu araştırmaya katılan hastaların %50.7'sinin 65-74 yaş aralığında, %82.8'inin kadın, %67.4'ünün obez olduğu, %86.8'inin kronik hastalığının olduğu ve bunların % 75.6'sında HT, %43.1'ünde DM, %33.6'sinde kalp hastalığı olduğu saptandı. Bu hastaların %51,1'i ASA II ve %45.8'i ASA III'ti. Charlson Komorbidite İndeksi (CCİ) puanı hastaların %65.2'sinde 3 ve altı puan idi. Bu çalışmada ameliyat sırasında İPH gelişme durumu ile bilateral diz artroplasti, uygulanan, ≥1000 ml IV sıvı infüzyonu uygulanma durumu, ameliyathane salonu ortam ısısı, kan kaybı miktarı, irrigasyon sıvısı miktarı, aspiratördeki ortalama sıvı hacmi ve irrigasyon hacmi farkı, aspiratördeki drenaj hacmi, insizyondan primer kapatmaya kadar geçen süre ve anestezi indüksiyonu süresi arasında istatistiksel olarak anlamlı fark bulundu (p<0.05). Ameliyat sonrası İPH'nin HT, tiroid fonksiyon bozukluğu, CCİ puanı 4 ve üzerinde olan, ameliyat sonrası bakım ünitesi ortam ısısı, kan ürünü kullanım durumu, kan kaybı miktarı, anestezi indüksiyonu süresi istatistiksel olarak anlamlı fark bulundu (p<0.05). Ameliyat sırası İPH gelişme durumunun anestezi indüksiyon sürenin 1 dk artışının ameliyat sırasında İPH gelişme riskini 1.016 kat, uygulanan IV sıvı miktarındaki 1000 ml artışın İPH gelişme riskini 1.001 kat arttırdığı belirlendi. Ameliyathane oda sıcaklığının 1°C artışının ise ameliyat sırasında İPH gelişme riskini 0.804 kat azalttığı saptandı. Ameliyat sonrası hipotermi gelişme durumu HT rahatsızlığı olan hastalarda 3.856 kat kat, tiroid fonksiyon bozukluğunun bulunmasının 3.181 kat fazladır. Ameliyat öncesi globülin değerinin 1 birimlik artışı 2.869 kat daha fazla artırdığı saptandı (p<0.05). Sonuç: İPH diz artroplasti ameliyatları sırasında ve sonrasında yüksek olduğu bulundu. Ameliyathane salonu ortam ısı İPH için en önemli risk olduğu saptandı.

Objective: This study is a descriptive study conducted to investigate the incidence of inadvertent perioperative hypothermia and associated risk factors in knee arthroplasty surgeries. Methods: Method: This descriptive study was conducted between 18.10.2021 and 04.01.2023 in the Orthopedics and Traumatology Clinic of a university hospital in the Aegean region. The sample of the study consisted of 227 patients aged 18 and over who voluntarily agreed to participate and had undergone knee arthroplasty surgery at the Orthopedics and Traumatology Clinic. The data for the study were collected using a data collection form developed by the researchers, which included questions on socio-demographic characteristics and the status of inadvertent perioperative hypothermia (IPH). This form contains 136 questions. This form contains 136 questions. This form includes 26 questions about demographic characteristics, 27 questions about preoperative patient data, 41 questions about the surgical procedure during surgery, and 42 questions about the postoperative procedure. In the analysis of the research data, the“Statistical Package for the Social Sciences (SPSS) 26.0 program”was used. Descriptive statistics were given as unit number (n), percentage (%), mean±standard deviation and median values. The normal distribution of the data belonging to numerical variables was evaluated with the Shapiro Wilk normality test. In the comparison of two groups, if the data were normally distributed, the t-test was used in independent samples, and if the data were not normally distributed, the Mann Whitney U Test was used. The relationships between numerical variables were evaluated with the Spearman correlation coefficient. Findings: In the study, 60.4% of the patients who underwent DA surgery had IPH during surgery and 20.7% after surgery. IPH was not observed in the preoperative period. It was determined that 50.7% of the patients participating in this study were between the ages of 65-74, 82.8% were female, 67.4% were obese, 86.8% had chronic diseases, 75.6% of whom had HT, 43.1% had DM, and 33.6% had heart disease. In these patients, 51.1% were ASA II and 45.8% were ASA III. The Charlson Comorbidity Index (CCI) score was 3 or less in 65.2% of the patients. In this study, the development of IPH during the surgery and bilateral knee arthroplasty, ≥1000 ml IV fluid infusion, the ambient temperature of the operating room, the amount of blood loss, the amount of irrigation fluid, the difference between the average fluid volume and irrigation volume in the aspirator, the drainage volume in the aspirator, the incision and A statistically significant difference was found between the time to primary closure and the time to anesthesia induction (p<0.05). A statistically significant difference was found for HT, thyroid dysfunction, CCI score of 4 and above, ambient temperature of the postoperative care unit, status of blood product use, amount of blood loss the duration of anesthesia induction (p<0.05). Intraoperative IPH development was found to be 1.016 times higher with a 1 min increase in anesthesia induction time and 1.001 times higher with a 1000 ml increase in the amount of IV fluid administered. It was found that a 1°C increase in the operating room temperature decreased the risk of developing IPH during surgery by 0.804 times. The risk of developing IPH after surgery was 3.856 times higher in patients with HT and 3.181 times higher in patients with thyroid dysfunction. An increase of 1 unit in preoperative globulin level was 2.869 times higher (p<0.05). Conclusion: IPH was found to be high during and after knee arthroplasty surgeries. Ambient temperature in the operating room was determined to be the most important risk for IPH.
