Genel cerrahi servisinde yatan hastaların hemşirelik bakım kalitesi algılamaları ile memnuniyet durumları arasındaki ilişkinin incelenmesi

Özet
Amaç: Bu araştırma genel cerrahi servisinde yatmakta olan hastaların hemşirelik bakım kalitesini algılamaları ile memnuniyet durumları arasındaki ilişkinin incelenmesi amacıyla yapıldı. Gereç ve Yöntemler: Bu araştırma, 1 Aralık 2023 - 1 Mart 2024 tarihleri arasında Ege Üniversitesi Hastanesi Genel Cerrahi servisinde yürütüldü. Tanımlayıcı ve kesitsel tipte olan bu araştırmanın örneklemini; genel cerrahi servisinde yatmakta olan araştırmaya katılmaya gönüllü 132 hasta oluşturdu. Verilerin toplanmasında,“Hasta Tanıtım Formu”,“Bakım Davranışları Ölçeği - BDÖ24”ve“Newcastle Hemşirelik Bakımı Memnuniyet Ölçeği - NHBMÖ”kullanıldı. Araştırma verilerinin analizinde“Statistical Package For Social Science (SPSS) 25.0”paket programı kullanıldı. Verilerin dağılımı“tanımlayıcı analiz parametreleri”[ortalama, standart sapma, minimum, maksimum, frekans ve yüzde] ile gösterildi. Verilerin normal dağılıma uyup uymadığı“Shapiro Wilk testi”ile analiz edildi. Verilerin normal dağılıma uymaması nedeniyle analizlerde“Kruskal Wallis ve Mann Whitney U testi”kullanıldı. Farklılığın bulunduğu karşılaştırmalarda, farklılığın kaynak aldığı grubun tespit edilmesinde“Tamhane's T2 Post Hoc testi”kullanıldı. Verilerin analizinde“%95 güven aralığı”tercih edildi. Bulgular: Araştırmaya dahil olan hastaların yaş ortalaması 50,67±15,95'tir. Hastaların en küçüğü 21, en büyüğü ise 89 yaşındadır ve büyük çoğunluğu (%35,6) 35-49 yaş grubundadır. Hastaların %32,6'sı ortaokul mezunu, %22,8'i emekli, %50,8'i çalışmamakta, %91,7'si ilde yaşamakta, %100'ü sosyal güvence sahibi, %65,2'sinin kronik hastalığı yoktur. Hastaların %53,0'ının daha önce hastaneye yattığı; %59,8'inin daha önce cerrahi girişim geçirdiği ve %47,7'sinin şu ana kadar 1 defa hastanede kaldığı ve %25,0'ının 3 gece hastanede kaldığı belirlendi.“Newcastle Hemşirelik Bakımı Memnuniyet Ölçeği puan ortalaması”79,00±16,47'dir. Hastaların“Newcastle Hemşirelik Bakımı Memnuniyet Ölçeği toplam puanı”yla yaş, medeni durum ve bu yatışlarında hastanede kaç gece kaldıkları arasında“istatiksel olarak anlamlı fark”bulundu [p<0.05]. 65 yaş ve üzeri hastaların, diğer yaş gruplarındaki hastalara göre daha yüksek“NHBMÖ puan ortalaması”na sahip olduğu belirlendi. Hastanedeki gece kalış süresi 5 gece olan hastaların“ NHBMÖ puanları”daha yüksek bulundu.“Bakım Davranışları Ölçeği genel puan ortalaması”5,19 ±0,86 olarak saptandı. Dört alt boyutlu ölçeğin“bağlılık alt boyutu toplam puanı”5,13 ±0,92,“saygılı olma alt boyutu toplam puanı”5,18 ±0,92,“bilgi beceri alt boyutu toplam puanı”5,23 ±0,84 ve“güvence alt boyutu toplam puanı”5,22 ±0,87 olarak bulundu. Yaşlarına göre, 65 ve üzeri yaş hastaların“BDÖ”“bağlılık”,“bilgi-beceri”,“güvence”ve“BDÖ toplam puan ortalaması”diğer yaş gruplarından yüksek bulundu. Bekar hastalar,“BDÖ”“bağlılık”ve“saygılı olma alt boyutu”nda evli hastalara göre hemşirelik bakımından daha çok memnunlardı. Hastaların bu yatışlarındaki hastanede gece kalış süreleri yönünden,“BDÖ toplam puanı”,“bağlılık alt boyutu”nda,“bilgi-beceri alt boyutu”nda ve“güvence alt boyutu”nda, 4 gece kalan hastaların diğer hastalardan hemşirelik bakımı algılamaları daha yüksek oltuğu tespit edildi. Hastanede gece kalış süresi ile bakım davranışları arasında“istatistiksel olarak anlamlı bir fark”bulundu [p<0.05]. Hemşirelik bakımı algılamaları puanı en düşük olan hastalar;“bağlılık alt boyutu”nda 3 gece kalan hastalar,“saygılı olma alt boyutu”nda 2 gece kalan hastalar,“bilgi-beceri alt boyutu”nda 3 gece kalan hastalar ve“güvence alt boyutu”ndaysa 2 gece kalan hastalar olduğu tespit edildi. Sonuç: Araştırmaya katılan hastaların“NHBMÖ”den aldıkları“puan ortalaması”nın orta değerin üzerinde yüksek olduğu,“BDÖ toplam puan”ve“alt boyut puan ortalamaları”nınsa çok yüksek olduğu; yaş, medeni durum ve hastanede gece kalış süresinin“NHBMÖ”ve“BDÖ toplam”ve“alt boyutlarının puan ortalamaları”arasında“istatistiksel olarak anlamlı bir ilişki”olduğu saptandı.

Aim: This study was conducted to examine the relationship between the perception of nursing care quality and the satisfaction levels of patients hospitalized in the general surgery unit. Material and Methods: This study was conducted between December 1, 2023, and March 1, 2024, in the“General Surgery Unit of Ege University Hospital”. The study, which was descriptive and cross-sectional in design, included a sample of 132 patients hospitalized in the general surgery unit who voluntarily agreed to participate. Data were collected using the“Patient Demographic Form”,“Caring Behaviors Inventory - CBI24”and“Newcastle Satisfaction with Nursing Care Scale - NSNCS”. For data analysis, the“Statistical Package for Social Sciences (SPSS) version 25.0”was utilized. The distribution of the data was presented using“descriptive analysis parameters”[mean, standard deviation, minimum, maximum, frequency, and percentage]. The normality of the data distribution was assessed using the“Shapiro-Wilk test”. As the data did not follow a normal distribution,“Kruskal-Wallis and Mann-Whitney U tests”were employed in the analysis. For comparisons showing significant differences, the source of the difference was identified using the“Tamhane's T2 Post Hoc test”. A“95% confidence interval”was used for the analysis. Results: The average age of the patients participating in the study was 50.67±15.95 years. The youngest patient was 21 years old, while the oldest was 89 years old, with the majority (35.6%) falling into the 35-49 age group. Among the participants, 32.6% were middle school graduates, 22.8% were retirees, 50.8% were unemployed, 91.7% resided in the city, and 100% had social security coverage. Additionally, 65.2% of the patients reported having no chronic illness. It was also determined that 53.0% of the patients had been hospitalized before, 59.8% had undergone a surgical procedure in the past, 47.7% had been hospitalized only once until now, and 25.0% had stayed in the hospital for three nights during their most recent hospitalization. The mean score of the“Newcastle Satisfaction with Nursing Care Scale (NSNCS)”was 79.00±16.47. A statistically significant difference was found between the total“NSNCS score”and the patients' age, marital status, and the number of nights they stayed in the hospital during this admission [p<0.05]. It was observed that patients aged 65 and older had higher“NSNCS”mean scores compared to those in other age groups. Additionally, patients who stayed in the hospital for five nights had higher“NSNCS”scores. The overall mean score of the“Caring Behaviors Inventory (CBI)”was found to be 5.19 ± 0.86. The subscale scores were as follows: the“Commitment”subscale scored 5.13 ± 0.92, the“Respectfulness”subscale scored 5.18 ± 0.92, the“Knowledge and Skill”subscale scored 5.23 ± 0.84, and the“Assurance”subscale scored 5.22 ± 0.87. Patients aged 65 and older had higher mean scores in the“Commitment,”“Knowledge and Skill,”and“Assurance”subscales, as well as in the overall“CBI score”, compared to other age groups. It was observed that single patients reported greater satisfaction with nursing care in the“Commitment”and“Respectfulness”subscales compared to married patients. Regarding the number of nights spent in the hospital during this admission, patients who stayed for four nights had higher overall“CBI scores”and higher scores in the“Commitment,”“Knowledge and Skill,”and“Assurance”subscales than those who stayed for fewer or more nights.“A statistically significant difference”was found between the length of hospital stay and the perception of caring behaviors [p<0.05]. The lowest scores in the perception of nursing care were observed among patients who stayed for three nights in the“Commitment”subscale, those who stayed for two nights in the“Respectfulness”subscale, those who stayed for three nights in the“Knowledge and Skill”subscale, and those who stayed for two nights in the“Assurance”subscale. Conclusion: It was determined that the mean score of the“Newcastle Satisfaction with Nursing Care Scale (NSNCS)”among the patients participating in the study was above the median level, indicating a high level of satisfaction. Additionally, the overall and subscale mean scores of the“Caring Behaviors Inventory (CBI)”were found to be very high.“A statistically significant relationship”was identified between age, marital status, and the length of hospital stay, and the mean scores of both the“NSNCS”and the overall and“subscale scores of the CBI”.
