Introduction: Diabetes is a chronic metabolic disease that is associated with elevated blood glucose levels. This is mainly due to the body's inefficient use of insulin and the inability of pancreatic beta cells to compensate for the increased insulin demand. Sulfonylureas (SUs) are among the most widely used oral hypoglycemic drugs 1,2. They work by stimulating insulin secretion from pancreatic B cells. Most SUs are highly metabolized in the liver and cytochrome P450 CYP 2C9 (a major enzyme mediated by SU metabolism has been shown to be associated with a better glycemic response to sulfonylureas and lower treatment failure rates. Purpose: the aim of this study was To investigate the genetic association of rs1057910 polymorphism of CYP2C9 gene with response to sulfonylurea in patients with type 2 diabetes. Methods: First, blood samples were taken from 30 patients with type 2 diabetes using sulfonylurea and DNA extraction was performed by SALTING OUT method, and then the desired fragment was PCR and observed on gel, then samples were taken to determine the genotype Were sequenced. Results: Although FBS in patients with AA, CA and CC genotypes decreased by 46.9%, 4.8% and 9.7% on average during treatment, in addition, although HbA1C in patients with previous AA, CA and CC carriers There was a significant difference between (p = 0.001) and after (p = 0.018) treatment period, our study showed that the therapeutic response to SFUs was not related to rs1057910 type of CYP2C9 gene. Discussion & Conclusion: Due to the lack of significant correlation between the genotypes of the common variant of CYP2C9 gene with the response to sulfonylurea treatment in patients with type 2 diabetes, it is recommended that this study be performed again with a larger statistical population.
