Özet

Amaç: Büllöz pemfigoid (BP) sıklıkla yaşlı popülasyonda gözlenir. Bu nedenle eşlik eden komorbiditeler ve bu komorbiditelere bağlı ilaç kullanımları mevcuttur. Biz çalışmamızda büllöz pemfigoid tanısı alan hastaların demografik özelliklerini, eşlik eden komorbiditelerini, ilaç kullanımını ve atopi öyküsünü inceleyerek olası büllöz pemfigoid tetikleyicilerini saptamayı amaçladık. Gereç ve Yöntem: Aydın Adnan Menderes Üniversitesi Deri ve Zührevi Hastalıkları kliniğine 1 Ocak 2015-1 Eylül 2022 tarihleri arasında başvuran 151 büllöz pemfigoid hastası çalışmaya alındı. Kontrol grubu 1 Ocak 2015- 1 Eylül 2022 tarihleri arasında Aydın Adnan Menderes Üniversitesi Aile Hekimliği polikliniğine başvuran, aktif hastalığı olmayıp genel muayene tanısıyla değerlendirilmiş hastalardan belirlendi. Hasta grubu ile uyumlu yaş ve cinsiyette 156 kontrol hastası çalışmaya dahil edildi. Her iki grupta da vefat eden hastalar eski verilere ulaşım zorluğu nedeniyle analizlere dahil edilmedi. Hastaların demografik özelikleri (yaş, cinsiyet, yaşadığı bölge), eşlik eden komorbidite varlığı, malignite öyküsü, kullandığı ilaçlar, atopi öyküsü (alerjik rinit, astım, atopik dermatit), periferik kanda eozinofilisi olup olmadığı kaydedildi. Bulgular: Büllöz pemfigoid hasta popülasyonunu oluşturan 104 hastanın 62'si (%59.6) kadın, 42'si (%40.4) erkekti. Hastaların yaş ortalaması 72.7±14.09'du. Oral mukoza tutulumu 23 (%22.1) hastada mevcuttu. BP hasta grubunda sık gözlenen komorbiditeler 76 (%73) hastada hipertansiyon (HT), 63 (%60.6) hastada diyabetes mellitus (DM), 28 (%26.9) hastada astım, 23 (%22.1) hastada hiperlipidemi 22 (%21.2) hastada osteoporozdu. Kontrol grubu ile karşılaştırıldığında DM, HT, astım nörojenik mesane, BP hasta grubunda daha sık eşlik etmekteydi (p<0.05). Kontrol grubunda ise hipotiroidi ve gastroözefagial reflü görülme sıklığı BP'li hasta grubuna kıyasla anlamlı olarak daha yüksekti (p<0.05). BP hastalarında atopi öyküsü, periferik kan eozinofilisi kontrol grubuna kıyasla daha fazlaydı (p<0.05). 104 BP hastasının 50 (%48.0)' sinde en az bir nörolojik hastalık öyküsü mevcuttu. BPli hasta grubunda sık görülen nörolojik hastalıklar 21 (%20.2) hastada serebrovasküler hastalık (SVH), 20 (%19.2) hastada Alzheimer hastalığı, 10 (%9.6) hastada vertigoydu. Kontrol grubu ile karşılaştırıldığında, hasta grubunda Alzheimer hastalığı ve SVH görülme sıklığı anlamlı olarak daha yüksek bulundu (p<0.05). Malignite görülme sıklığında artış açısından iki grup karşılaştırıldığında istatistiksel anlamlı fark saptanmadı. İlaç kullanımı karşılaştırıldığında antidiyabetik ilaçlardan vildagliptin, linagliptin, pioglitazon, insülin aspartat ve insüllin glarjin büllöz pemfigoid gelişimi ile ilişkiliydi (p<0.05). Antihipertansiflerden amlodipin, diltiazem, nebivolol; alzehimer tedavisinde kullanılan donepezil, rivastigmin, memantin kullanım sıklığı da BP hastalarında anlamlı olarak daha yüksekti (p<0.05). Sonuç: Büllöz pemfigoid (BP), hastalarında hipertansiyon, diyabetes mellitus, astım ve nörolojik hastalıklardan SVH, Alzheimer hastalığı tanısı sıktır. Bu komorbiditelere sahip kişiler BP gelişimi açısından dikkatli takip edilmelidir. Kişilerde atopi öyküsü olması, mevcut periferik kan eozinofilisi BP gelişimi açısından ipucu olabilir. İlaç kullanımı ayrıntılı biçimde sorgulanmalı, antidiyabetik ilaçlardan vildagliptin, linagliptin, pioglitazon, insülin aspartat ve insüllin glarjin, antihipertansiflerden amlodipin, diltiazem, nebivolol, alzehimer tedavisinde kullanılan ajanlardan donepezil, rivastigmin, memantin kullanımı varsa mümkünse alternatif bir ajanla değiştirilmelidir.

Objective: Bullous pemphigoid (BP) is frequently observed in the elderly population. Hence, accompanying comorbidities and medication use related to these comorbidities are not rare. In our study, we aimed to identify possible triggers of this dermatosis by examining the demographic characteristics, accompanying comorbidities, medication use and atopy history of bullous pemphigoid patients. Materials and Methods: One hundred fifty-one bullous pemphigoid patients who had applied to Aydın Adnan Menderes University Skin and Venereology Clinic between January 1, 2015 and September 1, 2022 were included in the study. The control group was determined from patients who applied to Aydın Adnan Menderes University Family Medicine outpatient clinic between January 1, 2015 and September 1, 2022, did not have an active disease, and were evaluated with a general examination diagnosis. One hundred fifty-six patients with age and gender match with the patient group were included in the study. In both groups, deceased patients were not included in the analyzes due to the inability of accessing medical records. Demographic characteristics of the patients (age, gender, region of residence), presence of accompanying comorbidities, history of malignancy, medications used, history of atopy (allergic rhinitis, asthma, atopic dermatitis), and whether there was eosinophilia in the peripheral blood were recorded. Results: Of the 104 patients who made up the patient population, 62 (59.6%) were female and 42 (40.4%) were male. The mean age of the patients was 72.7±14.09 years. Oral mucosa involvement was present in 23 (22.1%) patients. The most frequently observed comorbidities in the BP patient group were hypertension (HT) in 76 (73.1%), diabetes mellitus (DM) in 63 (60.6%), asthma in 28 (26.9%), hyperlipidemia in 23 (22.1%) and osteoporosis in 22 (21.2%). Compared to the control group, the patient group exhibited a higher frequency of DM, HT, asthma, neurogenic bladder (p<0.05). The incidence of hypothyroidism and gastroesophageal reflux was statistically significantly higher in the control group compared to the BP patient group. Among the BP patients, 48% had at least one neurological disease. The most common neurological comorbidities in the BD patient group were cerebrovascular disease (CVD) in 21 (20.2%), Alzheimer's disease in 20 (19.2%), and vertigo in 10 (9.6%). The patient group showed a significantly higher incidence of Alzheimer's disease and CVD compared to the control group (p<0.05). There was no statistically significant difference in the incidence of malignancy between the two groups. The use of antidiabetic drugs vildagliptin and linagliptin, pioglitazone, insulin aspartate, and insulin glargine were found to be associated with the development of bullous pemphigoid (p<0.05). Among antihypertensives, amlodipine, diltiazem, and nebivolol were found to be used more frequently in patients with high blood pressure (p<0.05). Additionally, BP patients had a higher prevalence of atopy history and peripheral blood eosinophilia compared to the control group. Conclusion: Diagnosis of hypertension, diabetes mellitus, asthma, and neurological diseases such as CVD and Alzheimer's disease is common in patients with bullous pemphigoid (BP). People with these comorbidities should be allert for the development of BP. A history of atopy and existing peripheral blood eosinophilia may be clues to the development of BP. Drug use should be questioned in detail, and if possible, the use of antidiabetic drugs vildagliptin, linagliptin, pioglitazone, insulin aspartate and insulin glargine, antihypertensives amlodipine, diltiazem, nebivolol, and agents used in the treatment of Alzheimer's disease such as donepezil, rivastigmine and memantine should be replaced with an alternative
