Özet
Amaç: Valsalva manevrası doğumun ikinci evresinde önemli rol oynamaktadır. Ancak pek çok gebe aktif olarak valsalva manevrası yapmayı bilmemektedir veya levator ani kasının kasılması ile karıştırmaktadır. Bu çalışmada gebelere antepartum süreçte transperineal ultrasonografi (TPUS) eşliğinde görsel geri bildirimlerle valsalva manevrası eğitimi verilmiş ve bu eğitimin doğum ikinci evre süresine etkisini değerlendirmek amaçlanmıştır. Gereç ve Yöntem: Çalışmamıza Haziran 2024 - Aralık 2024 tarihleri arasında hastanemize normal spontan doğum amaçlı interne edilen, 18-40 yaş arası, term ve primipar gebeler dahil edildi. Çalışmaya dahil edilen gebelere doğum için doğumhaneye interne edildiklerinde, doğumun henüz birinci evresindeyken, TPUS eşliğinde valsalva mekanizması görsel geri bildirimlerle anlatılarak; doğum süresinin kısalmasına katkı sağlanmaya çalışıldı. Çalışma boyunca gebelerimizin yaş, gebelik haftası, vücut kitle indeksi (VKİ), doğum ağrılığı, eğitim öncesi ve sonrası levator hiatus uzunlukları, gebelerin internasyondaki ve postpartum hemogram sonuçları, doğumun ikinci evre süresi, yenidoğan APGAR skorları, yenidoğan umblikal kord kan gazı pH ve baz açığı değerleri ve yenidoğanların yoğun bakım ünitesi (YYBÜ) 'ne gitme oranları kaydedildi. Kontrol grubu olarak da benzer yaş, gebelik haftası, VKİ ve doğum ağırlığına sahip primipar term gebelerin bilgileri toplandı. Bulgular: Çalışmaya dahil edilen 108 gebeden 57 kişi ultrason (USG) grubu, 51 kişi kontrol grubu olarak belirlendi. USG grubundaki toplam 6 gebe doğumun sezaryen ile sonuçlanması nedeniyle çalışma dışı bırakıldı. Yapılan istatistiksel analizler sonucunda valsalva manevrası eğitimi verilen grupta doğumun ikinci evre süresi, eğitim verilmeyen gruptan anlamlı olarak daha kısa bulundu (p<0.05). Sonuç: Çalışmamız sonucunda gebelere verilen ultrasonografik valsalva manevrası eğitiminin doğum ikinci evresi süresini anlamlı olarak kısalttığı bulunmuştur. Gebeler için doğuma yönelik böyle bir hazırlık yapmak; doğumun kolaylaşması, doğum ve hastanede kalış sürelerinin kısalması adına eğitimlerin yaygınlaşmasını sağlayabilir. Basit, ulaşılabilir ve tekrarlanabilir oluşuyla TPUS ve görsel geri bildirimler ile verilecek eğitimlerin özellikle obstetrisyen hekimlerin rutin bir uygulama yöntemi olabileceğini ve bu konuda farkındalığın arttırılması gerektiğini düşünmekteyiz.

Aim: The Valsalva maneuver plays an important role in the second stage of labor. However, many pregnant women can not know how to actively perform the Valsalva maneuver or confuse it with contraction of the levator ani muscle. In this study, the pregnant women were given Valsalva maneuver training with transperineal ultrasonographic visual feedback during the antepartum period and the aim was to evaluate the effect of this training. Materials and Methods: Our study included term, primiparous pregnant women between the ages of 18-40 who were interned to our hospital for normal spontaneous labor between June 2024 and December 2024. When the pregnant women included in the study were admitted to the delivery room for delivery, while they were still in the first stage of labor, the Valsalva mechanism was explained with visual feedback accompained under transperineal-ultrasound guidance; an attempt was made to contribute to shortening the delivery time. Throughout the study, the pregnants' age, gestational week, body mass index, birth weight, pre- and post-training levator hiatus lengths, prenatal and postpartum hemogram results of pregnant women, the duration of the second stage of labor, newborn APGAR scores, newborn umbilical cord blood gas pH and base deficit values and rates of newborns going to intensive care units were recorded. As a control group, information was collected from primiparous term pregnant women with similar age, gestational age, BMI and birth weight. Results: Of the 108 pregnant women included in the study, 57 were determined as the ultrasound group and 51 as the control group. A total of 6 patients in the USG group were excluded from the study because their delivery resulted in a cesarean section. As a result of the statistical analysis, the duration of the second stage of labor was found to be significantly shorter in the group that received Valsalva maneuver training than in the group that did not receive training (p<0.05). Conclusion: As a result of our study, it was found that the ultrasonographic Valsalva maneuver training given to pregnant women during the antepartum process significantly shortens the second stage of labor. Making such preparations for birth for pregnant women can help spread education to make labor easier and shorten the duration of labor and hospital stay. Nowadays, the availability of ultrasonography and the developments in ultrasound techniques can enable this modality to play a key role and be used for pelvic floor training in pregnant women.
