Özet

Psikososyal işlevsellikteki bozulma, şizofreni tedavisinde karşılaşılan en önemli zorluklardan biridir. Sanal Gerçeklik (SG), bu bozulmaları iyileştirmek için umut verici bir araç olarak ortaya çıkmıştır. Bu araştırmanın amacı, sinematik SG Psikososyal Tedavi Programının (sSG-PTP), şizofreni hastalarının psikososyal işlevselliğine olan etkisini incelemektir. Çalışmaya, DSM-5 kriterlerine göre şizofreni tanısı almış 37 gönüllü dahil edilmiş ve katılımcılar seçkisiz atama ile haftalık sSG-PTP seansları alan deney grubu (n=19) ve yüz yüze müdahale içermeyen kontrol grubu (n=18) olarak ikiye ayrılmıştır. Çalışma, deney grubunda 14, kontrol grubunda 12 katılımcıyla tamamlanmıştır. Değerlendirmeler müdahale öncesinde, 12 haftalık müdahale sonrasında ve 3 aylık izlemde gerçekleştirilmiştir. Gönüllülerin psikososyal işlevsellik (Bireysel ve Sosyal Performans Ölçeği, Sosyal İşlevsellik Ölçeği), sosyal bilişsel beceri (Dokuz Eylül Zihin Teorisi Ölçeği, Gözlerden zihin Okuma Testi, Yüzde Dışa Duran Duyguların Tanınması/Ayırt Edilmesi Testleri, Şizofrenide Gözlenebilir Toplumsal Bilişi Değerlendirme Ölçeği), nörobilişsel performans (Psikiyatride Bilişsel Belirti Tarama Test) ve klinik belirti (Pozitif ve Negatif Sendrom Ölçeği) düzeyleri üç zaman noktasında değerlendirilmiştir. Müdahale etkisi 2x2 ve 3x2 karışık desen ANOVA ile analiz edilmiştir. Sanal ortamda buradalık düzeyi, Sanal Ortamda Buradalık Ölçeği ile ölçülmüş, değişkenler arası ilişki Pearson korelasyon analizi ve yordayıcı faktörler basit doğrusal regresyon analizi ile incelenmiştir. Bulgular, sSG-PTP'nin şizofrenide psikososyal işlevselliği iyileştirmede tedavi etkisi olmadığını göstermiştir. Ancak, psikososyal işlevsellik [F(1,24)=6.1, p=0.021, ηp²=0.20], sosyal biliş [F(1,24)=10.75, p=0.003, ηp²=0.31], nörobiliş [F(1,24)=11.27, p=0.003, ηp²=0.32] ve klinik semptomlar [F(1,24)=54.3, p<0.001, ηp²=0.69] için zaman ana etkisi saptanmıştır. Gönüllüler, müdahale sırasında görece yüksek düzeyde buradalık algısı bildirmiştir. Ayrıca, başlangıç değerlendirmesinde daha düşük negatif semptom düzeyleri ve daha yüksek psikososyal işlevsellik düzeylerinin, artmış buradalık algısını öngördüğü saptanmıştır. Sinematik SG-PTP'nin standart yüz yüze görüşmelerden üstün olmadığı görülmüştür. Sanal gerçeklik tabanlı müdahalelerin psikososyal işlevsellikte anlamlı iyileşmeye olan etkisini incelemek için daha geniş örneklem gruplarıyla ve farklı müdahale protokolleriyle yapılan çalışmalara ihtiyaç vardır. Buradalık algısının tedavi sürecindeki rolü ve bu algıyı artırabilecek faktörlerin incelenmesi, SG uygulamalarının etkinliğini artırmak için önemli bir araştırma alanı sunmaktadır

Impairments in psychosocial functioning is one of the most significant challenges in the treatment of schizophrenia. Virtual Reality (VR) has emerged as a promising tool to address these impairments. This study aimed to investigate the effects of the sinematic VR Psychosocial Treatment Program (cVR-PTP) on the psychosocial functioning of individuals with schizophrenia. The study included 37 patients diagnosed with schizophrenia according to DSM-5 criteria. Participants were randomly assigned to either an experimental group receiving weekly cVR-PTP sessions (n=19) or a control group (n=18) that received non-interventional face-to-face sessions. The study was completed with 14 participants in the cVR-PTP and 12 in the control group. Assessments were conducted at three time points: before the intervention, after 12 weekly intervention sessions, and at a three-month follow-up. Participants' psychosocial functioning (measured with the Personal and Social Performance Scale and the Social Functioning Scale), social cognitive abilities (measured with the Dokuz Eylül Theory of Mind Scale, Reading the Mind in the Eyes Test, Emotion Identification/Discrimination Tests, and the Observable Social Cognition Rating Scale in Schizophrenia), neurocognitive performance (measured with the Screening for Cognitive Impairment in Psychiatry), and clinical symptoms (measured with the Positive and Negative Syndrome Scale) were evaluated. The effects of the intervention were analyzed using 2x2 and 3x2 mixed-design ANOVA. Presence in the virtual environment was measured using the Presence Questionnaire. Correlations between variables and presence questionnaire were examined through Pearson correlation analysis. Predictors of presence were analyzed using simple linear regression. The findings indicated that the cVR-PTP had no treatment effect on improving psychosocial functioning in schizophrenia. However, significant main effects of time were observed for psychosocial functioning [F(1,24)=6.1, p=0.021, ηp²=0.20], social cognition [F(1,24)=10.75, p=0.003, ηp²=0.31], neurocognition [F(1,24)=11.27, p=0.003, ηp²=0.32] and clinical symptoms [F(1,24)=54.3, p<0.001, ηp²=0.69]. Participants reported relatively high levels of presence in VR sessions. Furthermore, lower levels of negative symptoms and higher levels of psychosocial functioning at baseline were found to predict increased level of sense of presence. The findings suggest that cVR-PTP is not superior to standard face-to-face sessions. Future research with larger sample sizes and diverse intervention protocols is needed to explore the effects of VR-based interventions on psychosocial functioning more comprehensively. Investigating the role of presence in the treatment process and the factors that enhance this sense offers a valuable research field for improving the effectiveness of VR applications.
