Özet
Amaç: Güçlendirme modeli temelli tele-hemşirelik girişimlerinin KOAH hastalarını güçlendirmede etkisini test etmektir. Gereç ve Yöntem: Randomize kontrollü deneysel tasarım ile yürütülen bu araştırmaya bir kamu üniversite eğitim ve araştırma hastanesine başvuran 90 KOAH hastası (45 deney, 45 kontrol) alındı. Araştırmanın verileri Hasta Tanıtıcı Bilgi Formu, KOAH Güçlendirme Ölçeği (KOAH-GÖ), KOAH Değerlendirme Anketi (CAT), Modifiye Medikal Araştırma Kurulu Skalası (MMRC), Modifiye Barthel İndeksi, İnhaler Cihaz Kullanım Beceri Skalası ve Solunum Fonksiyon Testi (SFT) ile ön test (0.ay) son test (3. ay) ve izlem testte (6.ay) toplandı. Deney grubuna yüz yüze dört oturum KOAH güçlendirme eğitimi verildi. Ardından telefon aramaları ve bilgilendirici-hatırlatıcı kısa metin mesajları ile 12 hafta takip edildi. Kontrol grubu standart bakım aldı. Verilerin değerlendirilmesinde tanımlayıcı istatistiksel yöntemler, Ki-Kare ve Fisher exact testleri, bağımsız gruplar t-testi, ANOVA testi ve bonferroni testi kullanıldı. İstatistiksel anlamlılık düzeyi p<0,05 alındı. Bulgular: Deney grubunun son test ve izlem testte SFT değeri, KOAH alevlenmesi nedeniyle hastaneye başvuru ve yatış sayısı, KOAH-GÖ toplam ve alt boyut puanları, CAT, MMRC Dispne Skalası, inhaler cihaz kullanım beceri skalası toplam puanları kontrol grubuna göre anlamlı olarak iyileşti (p<0.05). Modifiye Barthel İndeksi toplam puanda anlamlı fark bulunmadı (p>0.05). Sonuç: Güçlendirme modeli temelli tele-hemşirelik girişimleri KOAH hastalarını güçlendirmede etkili olduğu bulundu.

Objective: The aim of this study is to test the effectiveness of empowerment model-based tele-nursing interventions in empowering patients with COPD. Materials and Methods: This randomized controlled trial included 90 COPD patients (45 in the experimental group, 45 in the control group) who applied to a public university training and research hospital. The data were collected through the Patient Demographic Information Form, COPD Empowerment Scale (COPD-ES), COPD Assessment Test (CAT), Modified Medical Research Council Scale (MMRC), Modified Barthel Index, Inhaler Device Use Skills Scale, and Pulmonary Function Test (PFT) at pre-test (0 month), post-test (3 months), and follow-up test (6 months). The experimental group received four face-to-face COPD empowerment training sessions. Subsequently, they were followed for 12 weeks with telephone calls and informative reminder text messages. The control group received standard care. Descriptive statistical methods, Chi-Square and Fisher's Exact Tests, independent samples t-test, ANOVA, and Bonferroni tests were used for data analysis. A statistical significance level of p<0.05 was accepted. Results: The experimental group showed significant improvements in the post-test and follow-up test for PFT, number of hospital admissions and admissions due to COPD exacerbations, total and subscale scores of the COPD-ES, CAT, MMRC Dyspnea Scale, and Inhaler Device Use Skills Scale compared to the control group (p<0.05). No significant difference was found in the total score of the Modified Barthel Index (p>0.05). Conclusion: Empowerment model-based tele-nursing interventions were found to be effective in empowering COPD patients.
