Background and aim: This study aimed to elucidate the molecular mechanism behind neurotoxicity in neonatal rats exposed to single and repeated sevoflurane anesthesia, while also evaluating the neuroprotective properties of preanesthetic melatonin administration. Methods: The research was carried out at the Laboratory of Experimental Animals, Faculty of Veterinary Medicine, Aydın Adnan Menderes University, following the approval of the Local Ethics Committee for Animal Experiments. The investigation encompassed four distinct groups: 1- the control group (CG), 2- the group subjected to anesthesia once (Sevo), 3- the group subjected to anesthesia three times consecutively at twenty-four-hour intervals (Sevo3), and 4- the group administered melatonin pretreatment before consecutive sevoflurane anesthesia three times at twenty-four-hour intervals (Sevo3+M). Thirty-two neonatal rats were utilized on the seventh postnatal day, with eight rats assigned to each group. Elisa and Real Time-PCR tests were employed to assess the protein expressions of GLT-1, GS, GDH, and GLS, along with the gene expressions of m-RNA, in the context of biochemical measurements. The data analysis involved descriptive statistics, chi-square, student-t, and Mann-Whitney U tests. Results: The expression of GLT-1 protein exhibited a statistically significant reduction in the Sevo, Sevo3, and Sevo3+M groups when compared to the control group; the most significant decrease was observed in the Sevo3 group, while the melatonin-pretreated group exhibited a comparatively smaller decrease in GLT-1 protein expression. Compared to the control group, a decrease in GS protein expression was found in the Sevo, Sevo3, and Sevo3+M groups. Nonetheless, only the Sevo3 group demonstrated a statistically significant decline. In the Sevo, Sevo3, and Sevo3+M groups, there were reductions in GDH protein expression compared to the control group; however, these differences did not reach statistical significance. Regarding GLS protein expression, there were increases in the Sevo, Sevo3, and Sevo3+M groups compared to the control group. However, only the Sevo3 group exhibited a statistically significant difference. Conclusion: The study's findings suggest that repeated sevoflurane anesthesia could potentially have neurotoxic effects in pediatric age groups. This phenomenon is believed to be due to its interaction with the brain's protective mechanisms, specifically those involving glutamate metabolism pathways. The administration of melatonin prior to exposure may exhibit protective properties against the toxic effects. It is advisable to exercise caution when administering repeated sevoflurane anesthesia to pediatric patients.
