Özet
Gastrointestinal sistem cerrahisinde sitoredüktif cerrahi ile kombine şekilde uygulanan hipertermik intraperitoneal kemoterapi tedavisi almış olan hastalarda ameliyat sonrası öz bakım gücünü değerlendirmek amacı bu çalışma ile nicel araştırma deseninden tanımlayıcı tipte planlandı. Ocak 2023- Temmuz 2024 tarihleri arasında, bir eğitim araştırma hastanesinin gastroenteroloji servisinde gerçekleştirildi. Hipertermik intraperitoneal kemoterapi uygulanan hastalar çalışmanın evrenini oluşturdu. Örneklem ise çalışma kriterlerine uygun olan 66 hastadan oluştu. Veri toplama aracı olarak, Demografik Bilgi Formları ve hasta grubu için Öz Bakım Ölçeği kullanıldı. Araştırmada elde edilen veriler free trial version of SPSS Statistics (Statistical Package for Social Sciences) for Windows 25.0 programı kullanılarak analiz edildi. Veriler değerlendirilirken (sayı, yüzde, min-maks değerleri, medyan, ortalama, standart sapma) tanımlayıcı istatistiksel metotlar kullanıldı. Verilerin normalliği Kolmogrov-Simirnov testi ile araştırıldı. Araştırma verileri sayı (n), yüzdelik dağılım (%), min-maks değerleri, ortalamalar (±standart sapma) ve tablolar ile ifade edildi. Çalışmaya katılan hastaların tanımlayıcı özellikleri ile HİPEK uygulaması sonrası öz bakım gücü arasındaki ilişkiler değerlendirildi. Bu doğrultuda Öz Bakım Gücü Ölçeği toplam puanı ile kategorik değişkenleri arasında, kronik hastalık varlığında“Evet”ve“Hayır”gruplarının toplam puan ortalamaları arasında anlamlı bir fark bulundu (p= .001). Bu sonuç, kronik hastalığı olmayan grubun toplam puanlarının, kronik hastalığı olan gruba göre istatistiksel olarak anlamlı derecede daha yüksek olduğunu göstermektedir. Sürekli ilaç kullanım durumu sonuçlarına göre,“Evet”ve“Hayır”gruplarının toplam puan ortalamaları arasında anlamlı bir ilişki bulundu (p= .006). Bu sonuç, ilaç kullanan grubun toplam puan ortalamasının kullanmayan gruptan daha düşük olduğunu göstermektedir. Çalışmaya katılan hastaların HİPEK uygulaması hakkında bilgi sahibi olup olmadığı ile ölçek toplam puanı arasında istatistiksel olarak anlamlı bir ilişki saptandı (p= .001). Bu fark,“Evet”grubunun toplam puanlarının“Hayır”grubuna göre istatistiksel olarak anlamlı derecede daha yüksek olduğunu göstermektedir. Sonuç olarak bu çalışma, sitoredüktif cerrahi ile kombine bir şekilde uygulanan hipertermik intraperitoneal kemoterapi tedavisinin hastalarda ameliyat sonrası öz bakım gücünü ne derece etkileyip etkilemediğini değerlendirmekte ve hasta üzerinde nasıl bir sonuca sebep olabileceğini göstermektedir.

The aim of this study was to evaluate postoperative self-care in patients undergoing hyperthermic intraperitoneal chemotherapy combined with cytoreductive surgery for gastrointestinal surgery. It was conducted in the gastroenterology department of a training and research hospital between January 2023 and July 2024. Patients who underwent hyperthermic intraperitoneal chemotherapy constituted the study population. The sample consisted of 66 patients who met the study criteria. Demographic Information Forms and the Self-Care Scale for the patient group were used as data collection tools. The data obtained in the study were analyzed using the free trial version of SPSS Statistics (Statistical Package for Social Sciences) for Windows 25.0 program. Descriptive statistical methods were used when evaluating the data (number, percentage, min-max values, median, mean, standard deviation). The normality of the data was investigated with the Kolmogorov-Smirnov test. Research data were expressed as number (n), percentage distribution (%), min-max values, means (±standard deviation) and tables. The relationships between the descriptive characteristics of the patients participating in the study and their self-care agency after HIPEC application were evaluated. In this regard, a significant difference was found between the total score of the Self-Care Ability Scale and its categorical variables, and the total mean scores of the“Yes”and“No”groups in the presence of chronic disease (p= .001). This result shows that the total scores of the group without chronic disease are statistically significantly higher than the group with chronic disease. According to the results of continuous drug use status, a significant relationship was found between the total mean scores of the“Yes”and“No”groups (p= .006). This result shows that the total mean score of the drug-using group is lower than the non-using group. A statistically significant relationship was found between whether the patients participating in the study had knowledge about HIPEC application and the total scale score (p= .001). This difference shows that the total scores of the“Yes”group are statistically significantly higher than the“No”group. In conclusion, this study evaluates to what extent hyperthermic intraperitoneal chemotherapy combined with cytoreductive surgery in oncological gastrointestinal system surgery affects the postoperative self-care ability of patients and shows what kind of results it may cause on the patient.
