Aim: The responses to treatment of patients who were treated with crystallized phenol after pit excision and sinus debridement in the microinvasive surgical treatment of pilonidal sinus (PS) disease and patients who were washed only with physiological serum after pit excision and sinus debridement were observed. Considering the local irritation and toxic effects of phenol, a chemical substance, it was aimed to apply the alternative method of washing with physiological serum at this stage of the microinvasive surgical method and compare its results. In addition, the factors affecting the recurrence of the disease were investigated. Material and Method: 130 patients who were diagnosed with pilonidal sinus at the General Surgery Clinic of S.B.Ü Ankara Education and Research Hospital between January 1, 2021 and December 31, 2023 and who had not previously undergone pilonidal sinus surgery were included in the study. Infected cases, cases with more than 3 pits on examination, and cases with pits more than 2 cm laterally were not included. Age, gender, body mass index, intergluteal sulcus depth, subcutaneous fat tissue thickness, pit number, history of previous abscess, and onset time of disease symptoms were evaluated and healing and recurrence status were recorded at the 3rd and 6th month controls after the applied treatments. The number of applied sessions and whether any complications developed due to the procedures were evaluated. Results: No significant difference was found between the patients who achieved healing in the 3rd and 6th month controls of the 1st patient group (n=60) who received crystallized phenol treatment after pit excision-debridement and the 2nd patient group (n=70) who received physiological serum (p=1.000, p=0.253). When the factors affecting the recurrence of the disease were evaluated with the multivariate logistic regression model, it was observed that recurrence developed 3.55 times more in patients treated with crystallized phenol (group 1) compared to the patient group treated with physiological saline (group 2), and that recurrence developed 3.82 times more in patients with an abscess history than in patients without an abscess history, x regardless of the groups. When the development of complications after treatment was evaluated, skin irritation was significantly seen in patients in group 1. There were no complications in patients in group 2. p<0.001) Conclusion: In our study comparing the two treatment groups, no superiority of crystallized phenol application was found over the group treated with physiological saline. It was revealed that effective hair removal and debridement are the most important factors of the treatment in the treatment of pilonidal sinus. As determined in the pilot study conducted previously, it was concluded that phenol, a chemical substance, is not necessary in the microinvasive surgical treatment of the disease, and that irrigation with physiological saline will be more beneficial in terms of avoiding chemical substance toxicity.
