Özet
Amaç: Hodgkin lenfoma (HL), genç erişkinlerde daha sık görülen ve yüksek tedavi başarı oranlarına rağmen klinik seyri bireyler arasında değişkenlik gösterebilen bir hematolojik malignitedir. Erken evre hastalarda standart tedavi ile sağkalım oranları %90'a ulaşsa da hastalığın seyri ve prognozunu etkileyebilecek faktörlerin belirlenmesi önemlidir. Son yıllarda malignite hastalarında nütrisyonel durumun hastalık prognozuna etkisi üzerinde çalışmalar artmıştır. Kontrollü Nütrisyonel Durum (CONUT) skoru ve Prognostik Nütrisyonel İndeks (PNI) nütrisyon durumunu değerlendirmede ön plana çıkmaktadır. Ancak, HL'de bu parametrelerin prognostik değerine dair veriler sınırlıdır. Çalışmamız HL hastalarında CONUT skoru ve PNI değerlerinin hastalık seyri ve sağkalım üzerindeki etkisini değerlendirmeyi amaçlamıştır. Gereç ve Yöntem: Bu çalışmada 2005-2024 yılları arasında Ege Üniversitesi Hematoloji kliniğinde HL tanısıyla izlenen 326 hastanın verileri retrospektif olarak incelendi. Tanı patolojik bulgularla doğrulandı ve evreleme için Ann Arbor evreleme sistemi-Cotswolds modifikasyonu kullanıldı. İleri evre hastalar evre IIB bulky, evre III ve IV olarak sınıflandırıldı ve bu hastalar için IPS 3-7 skorları hesaplandı. Erken evre hastalar ise EORTC kriterlerine göre favorable ve unfavorable olarak ayrıldı. CONUT skoru verisi bulunan 188 hasta çalışmaya dahil edildi. Bulgular: Çalışmada 326 hastadan oluşan genel popülasyon verileri incelendi. Genel popülasyon hasta grubumuzun medyan yaşı 36 ve %57,4'ü erkekti. Klasik Hodgkin lenfoma (cHL) hastaların %89,9'unu; nodüler lenfosit predominant HL (NLPHL) %9,8'unu oluştururken cHL alt tiplerinden nodüler sklerozan (NScHL) %52,9 oran ile en sık görüldü. Ortalama genel sağkalım süresi 71,28±49,69 ay olarak hesaplandı. Genel popülasyonumuzda IPS-7 ve IPS-3 skorları ile genel sağkalım arasında anlamlı ilişki (p<0,001 ve p:0,002) görüldü. CONUT skoru için cut-off değeri ≤4 olarak yüksek sensitivite (%92) oranı ile çalışmamız için cut-off değer olarak alınmıştır. PNI için belirlenen >56 cut-off değeri düşük sensitivite ve spesifite nedeniyle kullanılmamıştır. CONUT skoru hesaplanan hastaların medyan yaşı 38'dir. Kronik hastalığı olan 85 (%45,2) hastanın 30 (%35,3)'unda hipertansiyon mevcut olup hipertansiyon CONUT skoru >4 olan hastalarda anlamlı şekilde daha sık saptanmıştır (p:<0,034). CONUT grupları arasında kemik iliği ve ekstranodal tutulum olması anlamlı saptandı (p<0,001). IPS-7 ve IPS-3 skorları ile CONUT skoru genel sağkalımı ön görmede anlamlı saptandı (p:<0,001). Evrelere göre ve erken-ileri evre olma durumu ile hastaların genel sağkalım durumu arasında anlamlı fark saptanmıştır (p<0,05). CONUT ≤4 ve >4 grupları arasında 1,3, ve 5 yıllık genel sağkalım oranları arasında istatistiksel anlamlılık saptanmış olup düşük CONUT skorunun genel sağkalımı artırdığı gözlemlendi (p<0,001). Sonuç: HL hastalarında CONUT skoru ile tanı evresi, sağkalım durumu, HT varlığı, genel sağkalım oranları, prognostik belirteçler (IPS 3-7) ve biyokimyasal değerler (serum albümin düzeyi, total kolesterol, HDL, LDL, lenfosit sayısı ve hemoglobin düzeyi, ALP, GGT, nötrofil/lenfosit oranı, CRP, sedimentasyon ve ferritin düzeyi) arasında anlamlı ilişki saptandı. CONUT skoru ile genel sağkalım ve 1,3,5 yıllık genel sağkalım oranları arasında anlamlı ilişki saptanması düşük CONUT skorunun olmasının genel sağkalım olasılığını arttırdığı ve prognostik olarak kullananılabileceğini göstermiştir.

Aim: Hodgkin lymphoma (HL), is a hematologic malignancy more prevalent in young adults, exhibits variable clinical courses despite high treatment success rates. While standard therapy achieves survival rates of up to 90% in early-stage patients, identifying factors that may influence disease progression and prognosis remains crucial. In recent years, studies have increasingly focused on the impact of nutritional status on disease prognosis in malignancy patients. The Controlling Nutritional Status (CONUT) score and Prognostic Nutritional Index (PNI) are prominent in assessing nutritional status. However, data regarding the prognostic value of these parameters in HL remain limited. Our study aims to evaluate the effects of CONUT scores and PNI values on disease course and survival in HL patients. Materials and Method: In this study, data from 326 patients diagnosed with Hodgkin lymphoma (HL) and followed at the Ege University Hematology clinic between 2005 and 2024 were retrospectively analyzed. Diagnosis was confirmed by pathological findings, and the Ann Arbor staging system-Cotswolds modification was used for staging. Advanced-stage patients were classified as stage IIB bulky, stage III, and stage IV, and IPS 3-7 scores were calculated for these patients. Early-stage patients were categorized as favorable and unfavorable according to the EORTC criteria. A total of 188 patients with available CONUT scores were included in the study. Results: In this study, data from a general population of 326 patients were analyzed. Classical Hodgkin lymphoma (cHL) accounted for 89.9% of cases, while nodular lymphocyte-predominant HL (NLPHL) comprised 9.8%. Among cHL subtypes, nodular sclerosis (NScHL) was the most prevalent, observed in 52.9% of patients. The mean overall survival was calculated as 71.28±49.69 months. A significant correlation was found between IPS-7 and IPS-3 scores and overall survival in the general population (p<0,001 and p:0,002, respectively). The CONUT score cut-off value of ≤4 was selected for this study due to its high sensitivity (92%). The PNI cut-off value of >56 was not utilized due to low sensitivity and specificity. The median age of patients with calculated CONUT scores was 38 years. Among the 85 patients (45.2%) with chronic diseases, 30 (35.3%) had hypertension, which was significantly more frequent in patients with a CONUT score >4 (p<0,034). Significant associations were observed between CONUT groups and the presence of bone marrow and extranodal involvement (p<0,001). IPS-7 and IPS-3 scores, along with the CONUT score, were significant predictors of overall survival (p<0,001). A significant difference in overall survival was observed based on disease stage and early- versus advanced-stage status (p<0,05). Statistically significant differences were found in 1, 3, and 5-year overall survival rates between the CONUT ≤4 and >4 groups, indicating that a lower CONUT score was associated with improved overall survival (p<0,001). Conclusions: A significant relationship was found between the CONUT score and the stage at diagnosis, survival status, presence of hypertension, overall survival rates, prognostic markers (IPS 3-7), and biochemical values (serum albumin level, total cholesterol, HDL, LDL, lymphocyte count and hemoglobin level, ALP, GGT, neutrophil/lymphocyte ratio, CRP, sedimentation rate, and ferritin level) in HL patients. The significant relationship between the CONUT score and overall survival, as well as 1, 3, and 5-year overall survival rates, indicates that having a low CONUT score increases the likelihood of overall survival and suggests that it can be used as a prognostic marker.
