Özet

Amaç: Bu araştırma yoğun bakım hastalarında doğa temelli müzik dinletisinin ağrı ve fizyolojik parametreler üzerindeki etkisini incelemek amacıyla deneysel randomize kontrollü bir çalışma olarak gerçekleştirildi. Yöntem: Araştırma, İstanbul'da bir devlet hastanesi'nin yoğun bakım ünitesinde gerçekleştirildi. Araştırma evrenini Mart 2024 ile Mayıs 2024 tarihleri arasında yoğun bakım ünitesine yatışı yapılan hastalar örneklemini ise araştırmaya dahil edilme kriterlerini karşılayan 108 hasta oluşturdu. Hastalar randomizasyon yöntemi ile doğa sesi deney grubu (54) ve rutin uygulamaların yapıldığı kontrol grubuna (54) ayrıldı. Araştırma verileri“Hasta Tanıtıcı Bilgi Formu”,“Sayısal Ağrı Skalası”ve“Fizyolojik Parametreler Takip Formu”ile toplandı. Deney grubuna doğa temelli müzik dinletisi uygulanırken kontrol grubuna herhangi bir müdahale yapılmadı. Verilerin analizinde SPSS 22.0 programı kullanıldı ve tanımlayıcı istatistikler, t-testi ki kare testi ile değerlendirildi. Bulgular: Araştırmaya katılan deney grubundaki hastaların yaş ortalaması 58,92±16,46, kontrol grubundakilerin 73,64±5,79 yıl, deney grubunun %51,9'unun kadın, kontrol grubunun %53,7'sinin evli, deney grubunun %24,1'nin çalışmadığı, kontrol grubunun %25,9'nun lise mezunu olduğu saptandı. Kontrol grubundaki hastaların %75,9'da, deney grubundaki hastaların ise %72,2'ninde kronik hastalık bulunduğu, deney ve kontrol grubundaki hastaların %25,9'nun solunum sıkıntısı tanısı ile hastane yatışları olduğu belirlendi. Ön test sonuçlarına göre deney ve kontrol grupları arasında ağrı puanları açısından anlamlı bir fark bulunmazken (p=0,639), sistolik kan basıncı deney grubunda daha yüksek ölçüldü (p<0,05), diğer fizyolojik parametreler açısından gruplar arasında belirgin bir farklılık gözlemlenmedi. Son test sonuçlarına göre, deney grubunda ağrı skoru, sistolik ve diyastolik kan basıncı, nabız hızı ve oksijen satürasyonu açısından olumlu değişimler gözlenirken (p<0,05), kontrol grubunda ağrı skoru, sistolik ve diyastolik kan basıncı, nabız hızı ve solunum hızının arttığı, oksijen satürasyonunun ise azaldığı tespit edildi (p<0,05). Sonuç: Doğa temelli müzik dinletisinin, yoğun bakım hastalarında ağrıyı azaltmada ve fizyolojik parametreleri düzenlemede etkili bir non-farmakolojik yöntem olduğu belirlendi. Bu bulgular doğa temelli müzik uygulamalarının yoğun bakım ünitelerinde hasta bakım süreçlerine dahil edilmesi gerektiğini gösterir.

Objective: This study was conducted as an experimental, randomized controlled trial to examine the effect of nature-based music on pain and physiological parameters in intensive care patients. Method: The mean age of the patients in the experimental group participating in the study was 58.92±16.46, and those in the control group were 73.64±5.79 years, 51.9% of the experimental group was female, 53.7% of the control group was married, 24.1% of the experimental group was unemployed, and 25.9% of the control group was high school graduate. It was determined that 75.9% of the patients in the control group and 72.2% of the patients in the experimental group had chronic diseases, and 25.9% of the patients in the experimental and control groups had been hospitalized with a diagnosis of respiratory distress. According to the pre-test results, no significant difference was found between the experimental and control groups in terms of pain scores (p=0.639), while systolic blood pressure was measured higher in the experimental group (p<0.05), and no significant difference was observed between the groups in terms of other physiological parameters. According to the post-test results, positive changes were observed in the experimental group in terms of pain score, systolic and diastolic blood pressure, pulse rate and oxygen saturation (p<0.05), while in the control group, pain score, systolic and diastolic blood pressure, pulse rate and respiratory rate increased, and oxygen saturation decreased (p<0.05). Conclusion: It has been determined that nature-based music listening is an effective non-pharmacological method in reducing pain and regulating physiological parameters in intensive care patients. These findings indicate that nature-based music practices should be included in patient care processes in intensive care units.
