Özet
Diyabet, sadece periferik sistemde değil, merkezi sinir sisteminde de yapısal ve işlevsel değişikliklere yol açan sistemik bir hastalıktır. Diyabetin merkezi sinir sistemi üzerindeki etkileri, nörodejeneratif süreçler açısından daha fazla ilgi görmektedir. Son yıllarda yapılan çalışmalar, özellikle tip 1 diyabet modeli oluşturmak amacıyla streptozotosin (STZ) ile indüklenen diyabetin, hipokampus gibi öğrenme ve hafıza ile ilişkili beyin bölgelerinde nörodejeneratif değişikliklere neden olabileceğini göstermektedir. Bu bağlamda, hipokampus gibi öğrenme ve bellekle ilişkili beyin bölgeleri diyabetin nöropatolojik etkilerine karşı hassas yapılardır. Gap junction proteinleri olan aynı zamanda hipokampusun fizyolojik işleyişinde önemli rol oynayan konneksinler, özellikle konneksin 43 gibi alt tipler, nöronlar arası iletişim, hücresel bütünlük ve sinaptik plastisite açısından kritik roller üstlenmektedir. Yapılan çalışmalar, diyabetin konneksin ekspresyonunu değiştirebileceğini ve bu değişikliklerin bilişsel işlev bozukluklarıyla ilişkili olabileceğini ortaya koymaktadır. Bu çalışmada deneysel olarak streptozotosin (STZ) ile diyabet oluşturulmuş sıçanların hipokampüslerinde konneksinler ve etki mekanizmalarının araştırılması amaçlanmıştır. Deneyde 48 adet Wistar albino suşu erkek sıçan (180-200 gr, 2 aylık) kullanıldı. Sıçanlar rastgele ve eşit olarak; diyabet grubu (50 mg/kg streptozotosin i.p. n: 16), sham grubu (sitrat tamponu ph; 6 i.p n: 16) ve kontrol grubu (n: 16) olmak üzere 3 gruba ayrıldı. STZ uygulaması yapılarak Tip I diyabet oluşturuldu. Hayvanlar aç bırakılarak kuyruk venalarından kan alındı ve kan glukoz seviyeleri ölçüldü. Deney 40 günde tamamlandı. Akabinde açık alan ve Y-Maze Davranış Testleri uygulandı. İşlemlerin tamamlanmasının ardından hayvanlar sakrifiye edildi. Bouin tespit solüsyonu ile perfüzyon yapılarak beyinleri çıkarıldı. Beyin dokularından 4 µm kesitler lam üzerine alındıktan sonra dokuları detaylı görüntüleyebilmek amacıyla peroksidaz immünohistokimya boyama yöntemi uygulanarak CX26, CX29, CX32, CX36, CX43, CX45, CX46 antikorları kullanıldı. Kontrol, sitrat ve diyabet gruplarındaki hayvanların hipokampüslerinden elde edilen boyanmış kesitler araştırma mikroskobu ile incelendi. Cx43'te glial hücrelerde (özellikle astrositlerde) belirgin bir ekspresyon (p<0.05) görülmüş olup gruplar arasında bir fark gözlemlenmedi. CX46, CX26'da gruplar arasında herhangi bir ekspresyon tespit edilmedi (p>0.05). CX32'de kontrol grubunda hipokampüs damar çevresinde, sitrat grubunda sinir liflerinde zayıf ekspresyon (p<0.05); diyabet grubunda kan damarları etrafı ve sinir liflerinde ekspresyon gözlemlenmedi (p>0.05). CX29'da kontrol grubu dentate gyrus hücre duvarları sitoplazmasında az miktarda ekspresyon (p<0.05); sitrat grubunda oligodentrosit ve kan damarlarında pozitif reaksiyon (p<0.05); diyabet grubunda ise diğer gruplara kıyasla daha az ekspresyon gözlemlenmedi (p>0.05). CX45'te gruplar arasında belirgin bir fark görülmezken (p>0.05) sitrat ve diyabet grupları hipokampüs hücrelerinde zayıf nükleer pozitif reaksiyon gösterdiği gözlemlendi (p<0.05). CX36' da hipotalamus CA4 bölgesi ve sitoplazmada belirgin bir ekspresyon gözlemlendi (p<0.05). Sonuç olarak yapılan çalışmada diyabetin hipokampusta hem konneksinlerin ekspresyonu üzerinde hem de gruplar arasında bir negatif etkisinin olmadığı belirlendi. Yapılan ağırlık ve glıkoz ölçümü neticesinde; diyabetli sıçanların ağırlıklarının zamanla azaldığı ve kan glukoz seviyelerinin arttığı, kontrol ve sitrat grubunun ise ilk ölçümlere paralel seyrettiği anlaşılmıştır. Uygulanan davranış testlerinde; gruplar arası farklılıkların ve verilerin dengeli bir dağılım gösterdiği anlaşılmıştır. Y-maze testinde grupların spontan alternasyon yüzdeleri karşılaştırıldığında, grup değişkenine bağlı anlamlı bir fark saptanmamıştır. Açık alan testinde kaydedilen orta kare sayısı değerleri gruplar arasında anlamlı düzeyde farklılık göstermektedir. Elde edilen bulgular, diyabetin konneksin ifadesi üzerindeki etkilerini ortaya koyarak, diyabete bağlı nörodejenerasyonun altında yatan mekanizmaların daha iyi anlaşılmasına katkı sağlayabilir. Bu sonuçlar, diyabetin hipokampal konneksinler aracılığıyla nörodejeneratif süreçleri nasıl etkileyebileceğine dair literatüre katkı sağlamakta ve ileride yapılacak çalışmalara yönelik nöroprotektif yaklaşımlar için önemli bir zemin oluşturmaktadır.

Diabetes is a systemic disease that causes structural and functional changes not only in the peripheral system but also in the central nervous system. The effects of diabetes on the central nervous system are of greater interest in terms of neurodegenerative processes. Studies conducted in recent years have shown that diabetes induced by streptozotocin (STZ), especially in order to create a type 1 diabetes model, can cause neurodegenerative changes in brain regions associated with learning and memory, such as the hippocampus. In this context, brain regions associated with learning and memory, such as the hippocampus, are sensitive structures to the neuropathological effects of diabetes. Gap junction proteins, connexins, especially subtypes such as connexin 43, which play an important role in the physiological functioning of the hippocampus, play critical roles in terms of communication between neurons, cellular integrity, and synaptic plasticity. Studies have shown that diabetes can alter connexin expression and that these changes may be associated with cognitive dysfunction. The aim of this study was to investigate connexins and their mechanisms of action in the hippocampus of rats experimentally induced with streptozotocin (STZ) for diabetes. 48 male Wistar albino rats (180-200 g, 2 months old) were used in the experiment. The rats were randomly and equally divided into 3 groups as diabetes group (50 mg/kg streptozotocin i.p. n: 16), sham group (citrate buffer pH; 6 i.p n: 16) and control group (n: 16). Type I diabetes was induced by STZ administration. The animals were fasted and blood was taken from their tail veins and blood glucose levels were measured. The experiment was completed in 40 days. Then, open field and Y-Maze Behavioral Tests were applied. After the procedures were completed, the animals were sacrificed. The brains were removed by perfusion with Bouin fixative solution. After 4 µm sections were taken from brain tissues on slides, peroxidase immunohistochemistry staining method was applied and CX26, CX29, CX32, CX36, CX43, CX45, CX46 antibodies were used in order to visualize the tissues in detail. Stained sections obtained from the hippocampi of animals in control, citrate and diabetes groups were examined with a research microscope. In Cx43, a significant expression was seen in glial cells (especially in astrocytes) (p<0.05), and no difference was observed between the groups. No expression was detected between the groups in CX46, CX26 (p>0.05). In CX32, weak expression was observed around the hippocampus vessels in the control group and in the nerve fibers in the citrate group (p<0.05); no expression was observed around the blood vessels and in the nerve fibers in the diabetes group (p>0.05). In CX29, there was a low amount of expression in the cytoplasm of the dentate gyrus cell walls in the control group (p<0.05); a positive reaction in oligodendrocytes and blood vessels in the citrate group (p<0.05); and no less expression was observed in the diabetes group compared to the other groups (p>0.05). While no significant difference was observed between the groups in CX45 (p>0.05), it was observed that the citrate and diabetes groups showed a weak nuclear positive reaction in hippocampus cells (p<0.05). In CX36, a significant expression was observed in the hypothalamus CA4 region and cytoplasm (p<0.05). As a result, it was determined in the study that diabetes had no negative effect on both the expression of connexins in the hippocampus and between the groups. As a result of the weight and glucose measurements, it was understood that the weights of diabetic rats decreased over time and their blood glucose levels increased, while the control and citrate groups were parallel to the first measurements. In the behavioral tests applied, it was understood that the differences and data between the groups showed a balanced distribution. When the spontaneous alternation percentages of the groups were compared in the Y-maze test, no significant difference was detected depending on the group variable. The values of the number of central squares recorded in the open field test showed significant differences between the groups. The findings obtained may contribute to a better understanding of the mechanisms underlying diabetes-related neurodegeneration by revealing the effects of diabetes on connexin expression. These results contribute to the literature on how diabetes may affect neurodegenerative processes via hippocampal connexins and provide important groundwork for future studies on neuroprotective approaches.
