Özet

Amaç: Çalışmadaki amacımız lokalize prostat kanseri nedeniyle açık veya robot yardımlı laparoskopik radikal prostatektomi uygulanan hastalarda, preoperatif hastalığın evrelenmesi amaçlı çekilen ⁶⁸Ga-PSMA PET/BT'nin postoperatif PSA nüksü ve biyokimyasal nüksü ön görmedeki prognostik rolünü değerlendirmek ve intraprostatik PSMA tutulumunun SUVmax değeri baz alınarak bir cut-off değer tanımlamaktır. Gereç ve Yöntem: Kliniğimizde 1 Haziran 2023 ile 12 Eylül 2024 tarihleri arasında radikal prostatektomi operasyonu yapılan ve operasyon önce hastanemiz nükleer tıp kliniğinde ⁶⁸Ga PSMA PET/BT çekilen ve radikal prostatektomi spesmenlerinde pozitif cerrahi sınır olmayan 78 hastanın verileri retrospektif olarak değerlendirildi. Seçilen bu hasta grubunun preoperatif PSA değerleri, PSA dansiteleri, klinik evreleri, biyopsi patolojileri, radikal prostatektomi patolojileri ve patolojik evreleri incelendi. Bu hastaların preoperatif yapılan ⁶⁸Ga-PSMA PET/BT görüntülemeleri tek bir nükleer tıp uzmanı tarafından retrospektif olarak tekrar değerlendirildi ve yorumlandı. Hastalar, biyokimyasal nüks açısından hastane otomasyon sisteminde kayıtlı ardışık serum PSA değerleri ile değerlendirildi. Biyokimyasal nüks, ardışık iki ölçümde PSA seviyesinin ≥ 0.2 ng/mL olarak görülmesi şeklinde tanımlandı. Bulgular: Çalışmaya dahil edilme ve dışlanma kriterlerine göre toplam 78 hasta analiz edildi. Hastaların ortalama yaşı 65,52 ± 5,59 yıl, ortalama PSA değeri 12,31 ± 9,1 ng/mL ve prostat bezi hacmi 48,08 ± 25,79 mL olarak bulundu. Hastaların tamamına transrektal ultrasonografi eşliğinde 12 kor prostat biyopsisi uygulandı ve ortalama 5,27 ± 2,66 korda prostat kanseri tespit edildi. Prostat biyopsisi sonuçlarına göre 37 hastada ISUP grade 2, 16 hastada ISUP grade 3, 18 hastada ISUP grade 4 ve 7 hastada ISUP grade 5 adenokarsinom saptandı. Radikal prostatektomi sonrası yapılan patolojik değerlendirmede 8 hastada ISUP grade 1, 33 hastada ISUP grade 2, 18 hastada ISUP grade 3, 8 hastada ISUP grade 4 ve 11 hastada ISUP grade 5 hastalık tespit edildi. pT evrelemesine göre 9 hastada pT2a, 8 hastada pT2b, 44 hastada pT2c, 5 hastada pT3a ve 12 hastada pT3b hastalık izlendi. Preoperatif ⁶⁸Ga PSMA PET/BT incelemesinde tüm hastalarda prostat bezinde PSMA tutulumu olduğu gözlendi ve ortalama SUVmax değeri 11,66 ± 8,53 olarak hesaplandı. Ayrıca, 23 hastada pelvik lenf nodlarında PSMA tutulumu saptandı ve bu hastalarda ortalama lenf nodu SUVmax değeri 2,52 ± 1,17 olarak bulundu. Hiçbir hastada uzak metastaz ile uyumlu bulgu görülmedi. Postoperatif dönemde 21 hastada biyokimyasal nüks (BKN) gelişti. BKN gelişmeyen (BKN-) ve gelişen (BKN+) gruplar karşılaştırıldığında, PSA dansitesi ve prostat biyopsisi ISUP derecesi açısından anlamlı fark saptanmadı (p>0,05). Ancak, ⁶⁸Ga-PSMA PET/BT'de prostat bezindeki SUVmax değeri BKN(+) grubunda BKN(-) grubuna göre istatistiksel olarak anlamlı derecede yüksek bulundu (AUC: 0,798). ROC analizi sonucunda, BKN'yi öngörmede ⁶⁸Ga-PSMA PET/BT'de prostat bezinde SUVmax cut-off değeri 9,91 olarak hesaplandı (sensitivite: 0,81; spesifite: 0,298). Sonuçlar, preoperatif dönemde ⁶⁸Ga-PSMA PET/BT'de prostat bezindeki SUVmax değeri 9,91'in üzerinde olan hastalarda postoperatif BKN gelişme riskinin arttığını göstermektedir. Sonuç: Çalışmamızda RP öncesi ⁶⁸Ga-PSMA-PET/BT'de prostat bezindeki SUVmax değerinin biyokimyasal nüksü ön görmede bağımsız olarak prognostik öneminin olduğu saptadık. Lokalize ve lokal ileri PK nedeniyle RP yapılacak hastalarda preoperatif çekilen ⁶⁸Ga PSMA-PET/BT'de prostat bezindeki SUVmax değeri 9,91'den büyük olan hastaların nükssüz sağkalım açısından daha kötü prognostik olabileceğini ve bu hastaların postoperatif PSA nüksü ve biyokimyasal rekürrens açısından daha yakın takip edilmesi gerektiğine inanmaktayız

Objective : Our study aims to evaluate the prognostic role of preoperative ⁶⁸Ga-PSMA PET/CT, performed for staging purposes, in predicting postoperative PSA recurrence and biochemical recurrence in patients undergoing open or robot-assisted laparoscopic radical prostatectomy for localized prostate cancer. Additionally, we aim to define a cut-off value based on the intraprostatic PSMA uptake SUVmax value. Materials and Methods : The data of 78 patients who underwent radical prostatectomy between June 1, 2023, and September 12, 2024, in our clinic, had preoperative ⁶⁸Ga-PSMA PET/CT performed in our hospital's nuclear medicine department, and had no positive surgical margins in their radical prostatectomy specimens were retrospectively analyzed. The preoperative PSA levels, PSA densities, clinical stages, biopsy pathologies, radical prostatectomy pathologies, and pathological stages of this selected patient group were examined. The preoperative ⁶⁸Ga-PSMA PET/CT imaging of these patients was retrospectively re-evaluated and interpreted by a single nuclear medicine specialist. Patients were assessed for biochemical recurrence using serial serum PSA values recorded in the hospital automation system. Biochemical recurrence was defined as a PSA level of ≥ 0.2 ng/mL in two consecutive measurements. Results : A total of 78 patients were analyzed based on inclusion and exclusion criteria. The mean age of the patients was 65.52 ± 5.59 years, the mean PSA level was 12.31 ± 9.1 ng/mL, and the mean prostate volume was 48.08 ± 25.79 mL. All patients underwent a 12-core transrectal ultrasound-guided prostate biopsy, with an average of 5.27 ± 2.66 cores positive for prostate cancer. Based on prostate biopsy results, adenocarcinoma was detected in 37 patients with ISUP grade 2, 16 patients with ISUP grade 3, 18 patients with ISUP grade 4, and 7 patients with ISUP grade 5. Pathological evaluation after radical prostatectomy revealed ISUP grade 1 disease in 8 patients, ISUP grade 2 in 33 patients, ISUP grade 3 in 18 patients, ISUP grade 4 in 8 patients, and ISUP grade 5 in 11 patients. According to pathological tumor (pT) staging, 9 patients had pT2a, 8 had pT2b, 44 had pT2c, 5 had pT3a, and 12 had pT3b disease. Preoperative ⁶⁸Ga-PSMA PET/CT revealed PSMA uptake in the prostate gland in all patients, with a mean SUVmax value of 11.66 ± 8.53. Additionally, PSMA uptake in pelvic lymph nodes was detected in 23 patients, with a mean lymph node SUVmax value of 2.52 ± 1.17. No findings suggestive of distant metastasis were observed in any patient.In the postoperative period, biochemical recurrence (BCR) occurred in 21 patients. When comparing the BCR-negative (BCR-) and BCR-positive (BCR+) groups, no significant differences were found in PSA density or prostate biopsy ISUP grade (p>0.05). However, the SUVmax value in the prostate gland on ⁶⁸Ga-PSMA PET/CT was significantly higher in the BCR(+) group compared to the BCR(-) group (AUC: 0.798). Based on ROC analysis, the cut-off SUVmax value for predicting BCR on ⁶⁸Ga-PSMA PET/CT was determined to be 9.91 (sensitivity: 0.81; specificity: 0.298). These results suggest that patients with a preoperative SUVmax value greater than 9.91 in the prostate gland on ⁶⁸Ga PSMA PET/CT have an increased risk of developing postoperative BCR. Conclusion : In our study, we found that the SUVmax value in the prostate gland on preoperative ⁶⁸Ga-PSMA PET/CT is an independent prognostic factor for predicting biochemical recurrence. We believe that patients undergoing radical prostatectomy for localized or locally advanced prostate cancer with a preoperative SUVmax value higher than 9.91 on ⁶⁸Ga-PSMA PET/CT may have a worse prognosis in terms of recurrence-free survival and should be monitored more closely for postoperative PSA recurrence and biochemical recurrence
