Özet

GİRİŞ ve AMAÇ: Hodgkin lenfoma (HL), çocukluk çağı lenfomalarının yaklaşık %40'ını oluşturan önemli bir malignitedir ve sıklıkla erkeklerde, bimodal yaş dağılımı göstererek izlenir. Etiyolojisinde enfeksiyonlar, genetik ve immünolojik faktörler rol oynamaktadır. Klinik olarak ağrısız lenfadenopati, mediastinal kitle ve B semptomları ile başvurabilir. Histopatolojik olarak klasik HL alt tipleri en sık gözlenirken, tanı eksizyonel biyopsi ile konur ve evreleme Ann Arbor sistemine göre yapılır. Tedavide kemoterapi ve gerektiğinde radyoterapi uygulanır; relaps veya refrakter olgularda kök hücre nakli gerekebilir. Bu çalışmada, merkezimizde takip edilen HL tanılı çocuk hastaların klinik, laboratuvar ve histopatolojik özellikleri, uygulanan tedavi protokollerine verdikleri yanıt, sağkalım analizleri ve Epstein-Barr virüs (EBV) varlığı ile bu unsurların prognoz üzerindeki etkileri değerlendirilmiştir. MATERYAL METOD: Bu retrospektif çalışmada, 2002–2024 yılları arasında İstanbul Üniversitesi İstanbul Tıp Fakültesi'nde Hodgkin Lenfoma tanısı almış 18 yaş altı 93 çocuk hastanın dosya kayıtları demografik, klinik, laboratuvar ve tedavi yanıtları açısından incelenmiştir. Ann Arbor evreleme sistemi kullanılmış; hastalara tanı yılına göre GPOH-HD-2002 ve ABVD protokolüne dayalı kemoterapi ve radyoterapi uygulanmıştır. Tedaviye yanıt değerlendirmesi PET/BT ile Deauville kriterlerine göre yapılmış; tam ve kısmi yanıt, relaps, refrakter hastalık gibi tanımlar doğrultusunda sınıflandırılmıştır. Elde edilen veriler SPSS ile analiz edilerek sağkalım analizleri Kaplan-Meier yöntemi ve log-rank testi ile yapılmış; p<0,05 anlamlı kabul edilmiştir. BULGULAR: Bu çalışmada, 2002–2024 yılları arasında Hodgkin Lenfoma (HL) tanısı alan 93 çocuk hasta retrospektif olarak değerlendirilmiştir. Olguların %66,7'si erkek olup, ortalama tanı yaşı 11,3±4,1 yıl bulunmuştur. En sık tanı yaşı grubu 10–14 yaş aralığı (%37,6), en yaygın evre ise ANN-ARBOR evre II (%45,2) olarak saptanmıştır. Histopatolojik olarak olguların %95,7'si klasik HL olup, alt tip olarak nodüler sklerozan (%49,5) ve mikst selüler (%35,5) formlar baskındır. En sık lenf nodu tutulumu servikal, mediastinal ve supraklavikular bölgelerde gözlenmiş; ekstranodal tutulumlar arasında dalak (%23,7), akciğer ve kemik (%11,8) tutulumu öne çıkmıştır. Ailede solid tümör öyküsü %20,4, hematolojik malignite öyküsü %4,4 oranında saptanmış, akraba evliliği oranı %15,1'dir. Başvuruda olguların %41,9'unda B semptomu, %53,8'inde mediastinal tutulum ve %20,4'ünde bulky hastalık mevcuttur.Tedavi açısından hastaların %41,9'una ABVD rejimi uygulanmış, kalan hastalara ABVD dışı kemoterapi protokolleri verilmiştir. Her iki tedavi grubunda evre, histolojik alt tip, yanıt oranları ve nüks sıklığı açısından anlamlı fark izlenmemiştir. İnterim PET-BT değerlendirmesi, tedaviye erken yanıtı göstermesi açısından anlamlı bulunmuş; ikinci kür sonrası tam yanıt gösterenlerde nüks oranı %5,8 iken, kısmi yanıt gösterenlerde bu oran %33,3'tür (p=0,039). Genel sağkalım oranı tüm kohortta %97,8, progresyonsuz sağkalım %91,1 olarak belirlenmiş, ABVD ve ABVD dışı protokoller arasında sağkalım farkı gözlenmemiştir. EBV pozitifliği %25,8 oranında olup, mikst selüler alt tip ve 5–9 yaş grubu ile ilişkili bulunmuştur (p=0,032 ve p=0,004); EBV durumu ile sağkalım arasında anlamlı fark saptanmamıştır.Yüksek riskli veya refrakter olgulara hedefe yönelik ajanlar uygulanmış; brentuksimab vedotin verilen hastalarda olaysız sağkalım oranı %83,3 olarak tespit edilmiştir. Nüks oranı %7,1 olup, en sık evre II ve III hastalarda izlenmiştir. Geç dönem komplikasyonlar arasında hipotiroidi (%9,7) ve tiroid karsinomu (%4,3) öne çıkmıştır. Bulgular, erken tanı, uygun kemoterapi protokolü ve gerektiğinde hedefe yönelik tedavi ile çocukluk çağı HL olgularında yüksek sağkalım oranlarına ulaşılabildiğini göstermektedir. SONUÇ: Çocukluk çağı Hodgkin Lenfoma olgularının analiz edildiği bu çalışmada sağkalım (genel sağkalım %97.8 ve progresyonsuz sağkalım%91.1)literatür ile uyumlu olarak yüksek düzeylerdedir. Risk grubuna uygun tedavi yöntemleri ile mortalitenin yanında morbiditenin de azaltılması hedeflenmektedir.

INTRODUCTION and AIM: Hodgkin lymphoma (HL) is a significant malignancy accounting for approximately 40% of childhood lymphomas, and it is more commonly observed in males, showing a bimodal age distribution. In its etiology, infectious, genetic, and immunological factors are implicated. Clinically, patients may present with painless lymphadenopathy, mediastinal mass, and B symptoms. Histopathologically, classical HL subtypes are the most frequently observed forms. Diagnosis is typically established by excisional biopsy, and staging is performed using the Ann Arbor system. Treatment includes chemotherapy and, when necessary, radiotherapy; hematopoietic stem cell transplantation may be required in relapsed or refractory cases. This study aimed to evaluate the clinical, laboratory, and histopathological features of pediatric patients diagnosed with HL and followed in our center, along with their responses to applied treatment protocols, survival outcomes, and the presence of Epstein-Barr virus (EBV), as well as the impact of these factors on prognosis. MATERIALS AND METHODS: This retrospective study included 93 pediatric patients under the age of 18 who were diagnosed with Hodgkin Lymphoma at Istanbul University, Istanbul Faculty of Medicine, between 2002 and 2024. Medical records were reviewed for demographic, clinical, laboratory, and treatment response data. Staging was performed according to the Ann Arbor system. Based on the year of diagnosis, patients received chemotherapy and radiotherapy per the GPOH-HD-2002 and ABVD protocols. Treatment responses were assessed by PET/CT using Deauville criteria, and patients were classified as having complete or partial response, relapse, or refractory disease accordingly. Data were analyzed using SPSS software. Survival analyses were conducted using the Kaplan-Meier method and log-rank test, with p<0.05 considered statistically significant. RESULTS: This study retrospectively evaluated 93 pediatric patients diagnosed with Hodgkin Lymphoma (HL) between 2002 and 2024. Among the cases, 66.7% were male, with a mean age at diagnosis of 11.3±4.1 years. The most common age group at diagnosis was 10–14 years (37.6%), and the most frequent stage was Ann Arbor stage II (45.2%). Histopathologically, 95.7% of the cases were classified as classical HL, with nodular sclerosis (49.5%) and mixed cellularity (35.5%) being the most prevalent subtypes. Cervical, mediastinal, and supraclavicular lymph node involvement were most commonly observed, while extralymphatic involvement included spleen (23.7%), lung, and bone (11.8%). A family history of solid tumors was identified in 20.4% and hematologic malignancies in 4.4% of cases; the rate of consanguineous marriage was 15.1%. At presentation, B symptoms were observed in 41.9%, mediastinal involvement in 53.8%, and bulky disease in 20.4% of the patients. Regarding treatment, 41.9% of patients received the ABVD regimen, while the remaining received non-ABVD chemotherapy protocols. There were no statistically significant differences between the two groups in terms of disease stage, histological subtype, treatment response, or relapse rates. Interim PET-CT was found to be a significant predictor of early treatment response; the relapse rate was 5.8% among patients with complete metabolic response after the second chemotherapy cycle, compared to 33.3% in those with partial response (p=0.039). The overall survival rate was 97.8%, and progression-free survival was 91.1%, with no significant survival differences based on treatment regimen, stage, histological subtype, or EBV status. EBV positivity was identified in 25.8% of patients and was more commonly associated with the mixed cellularity subtype and younger age group (p=0.032 and p=0.004), though it did not significantly affect survival outcomes. Targeted therapies were administered to high-risk or refractory cases; among patients receiving brentuximab vedotin, the event-free survival rate was 83.3%. The overall relapse rate was 7.1%, most frequently occurring in stage II and III cases. Late complications included hypothyroidism (9.7%) and thyroid carcinoma (4.3%) as the most common findings. These results indicate that early diagnosis, appropriate chemotherapy protocols, and, when necessary, targeted therapies can achieve high survival rates in childhood HL cases. CONCLUSION: In this study analyzing childhood Hodgkin lymphoma cases, survival rates were high and consistent with the literature (overall survival: 97.8%, progression-free survival: 91.1%). Treatment strategies tailored to risk groups aim not only to reduce mortality but also to minimize morbidity.
