Özet
Minimal invaziv cerrahi yöntemlerin gelişmesiyle birlikte, özellikle robotik cerrahi uygulamaları, kolorektal kanser cerrahisinde daha sık tercih edilen yaklaşımlar haline gelmiştir. Bu yöntemler postoperatif ağrı açısından avantaj sağlasa da, ağrı yönetimi hala cerrahi sonrası morbiditeyi ve hasta konforunu etkileyen önemli bir faktördür. Postoperatif ağrı kontrolünde multimodal analjezi stratejileri önerilmekte olup, rejyonel anestezi teknikleri bu yaklaşımların önemli bir parçasını oluşturmaktadır. Erektör spina plan bloğu (ESPB), uygulama kolaylığı ve hem somatik hem de viseral analjezi sağlaması nedeniyle ön plana çıkmıştır. Literatürde ESPB'nin çeşitli abdominal cerrahilerde etkinliğini destekleyen veriler mevcut olmakla birlikte, robotik kolorektal cerrahilerdeki analjezik etkinliği ile ilgili veriler sınırlıdır. Bu nedenle çalışmamızda, robotik kolorektal cerrahi geçirecek hastalarda ESPB'nin postoperatif ağrı kontrolüne ve opioid tüketimine etkilerini değerlendirmeyi amaçladık. Gereç ve Yöntem Etik kurul onayını aldıktan sonra ESPB grubunda 36, kontrol grubunda 34 olma üzere toplamda 70 hastanın verileri prospektif olarak değerlendirildi. Tüm hastaların demografik verileri, anestezi, cerrahi ve hastanede kalış süreleri, postoperatif derlenme ünitesinde 0, 15, 30, 60. dk ve servis takiplerinde 2, 6, 12, 24, 48 ve 72. saatlerdeki istirahat ve derin nefes alma/öksürme NRS skorları, kurtarıcı analjezik gereksinimleri, HKA cihazından ilk narkotik analjezik talep zamanı, toplam talep sayısı ve ilk 24 saatte tüketilen toplam narkotik analjezik miktarı, bulantı-kusma ve gelişen komplikasyonlar değerlendirilerek kaydedildi. Bulgular Çalışmamızda; robotik kolorektal cerrahilerde ESPB ile kontrol grubuna göre, ilk 24 saatte toplam opioid tüketiminin, HKA cihazından toplam talep sayısının daha az olduğu, ilk analjezik talep zamanının anlamlı şekilde daha uzun olduğu, postoperatif ilk 72 saat istirahat halinde ve derin nefes alma/öksürme sırasında NRS ağrı skorlarının anlamlı şekilde daha düşük olduğu, postoperatif derlenme alanında uygulanan VIII analjezik miktarının daha düşük olduğu tespit edildi. Postoperatif bulantı-kusma oranlarının da ESPB ile daha düşük görüldüğü belirlendi. Gruplar arasında hasta mobilizasyon ve yatış süreleri benzer bulundu. Sonuç Çalışmamız, ESPB'nin robotik kolorektal cerrahi geçiren hastalarda multimodal analjezi stratejilerine etkili bir katkı sağlayabileceğini ve opioid tüketimini azaltarak opioid ilişkili bulantı-kusma gibi yan etkileri azaltabileceğini göstermektedir. Gruplar arasında mobilizasyon ve hastanede kalış süreleri açısından fark bulunmaması, cerrahi tekniğin minimal invaziv özellikleri ve standart perioperatif protokollerle ilişkili olabilir. ESPB; robotik kolorektal cerrahi uygulamalarında, güvenli, uygulanabilir ve etkili bir rejyonel analjezi yöntemi olarak görülmekle birlikte bu bulguların daha geniş hasta gruplarında ve çok merkezli randomize kontrollü çalışmalarla doğrulanması gerekmektedir.

With the advancement of minimally invasive surgical techniques, robotic surgery has become an increasingly preferred approach in colorectal cancer surgery. Although these techniques offer advantages in terms of postoperative pain, pain management still remains a critical factor influencing postoperative morbidity and patient comfort. Multimodal analgesia strategies are recommended for postoperative pain control, and regional anesthesia techniques constitute an essential component of these approaches. Among them, the erector spinae plane block (ESPB) has gained attention due to its ease of application and its ability to provide both somatic and visceral analgesia. While there is growing evidence in the literature supporting the effectiveness of ESPB in various abdominal surgeries, data regarding its analgesic efficacy in robotic colorectal surgery remain limited. Therefore, the aim of this study was to evaluate the effects of ESPB on postoperative pain control and opioid consumption in patients undergoing robotic colorectal surgery. Material and Method Following approval from the institutional ethics committee, data from a total of 70 patients were prospectively evaluated, including 36 patients in the ESPB group and 34 patients in the control group. Demographic data, anesthesia and surgical durations, length of hospital stay, postoperative rest and deep inspiration/coughing Numeric Rating Scale (NRS) pain scores at 0, 15, 30, and 60 minutes in the post-anesthesia care unit and at 2, 6, 12, 24, 48, and 72 hours during ward follow-up were recorded. In addition, the requirement for rescue analgesics, the time to first analgesic request from the patient-controlled analgesia (PCA) device, the total number of demands, the total amount of analgesics administered, postoperative nausea and vomiting, and any complications were evaluated and documented. X Results In our study, compared with the control group, patients who received ESPB during robotic colorectal surgery demonstrated significantly lower total opioid consumption within the first 24 hours. The total number of demands from the PCA device was reduced, while the time to first analgesic request was significantly prolonged. NRS scores, both at rest and during deep inspiration or coughing, were consistently and significantly lower in the ESPB group throughout the first 72 postoperative hours. Additionally, the amount of analgesics administered in the post-anesthesia care unit was lower in the ESPB group. Postoperative nausea and vomiting rates were also reduced in patients receiving ESPB. There was no significant difference between the groups in terms of postoperative mobilization time or length of hospital stay. Conclusion Our study demonstrates that ESPB may provide an effective contribution to multimodal analgesia strategies in patients undergoing robotic colorectal surgery and may reduce opioid-related side effects, such as nausea and vomiting, by decreasing opioid consumption. The absence of differences between groups regarding mobilization and length of hospital stay may be associated with the minimally invasive nature of the surgical technique and standardized perioperative protocols. ESPB is considered a safe, feasible, and effective regional analgesia technique in robotic colorectal surgery; however, these findings need to be validated in larger patient populations and multicenter randomized controlled trials.
