Özet
Amaç: Kolorektal kanser erkeklerde ve kadınlarda en sık teşhis edilen ve mortaliteye neden olan üçüncü kanserdir. Rektum kanseri için radikal rezeksiyon güvenilir ve etkili bir tedavidir ancak post-operatif komplikasyonlar görülmektedir. Total mezorektal eksizyon (TME), rezektabl rektum kanseri için altın standart cerrahi prosedürdür. Rektum kanseri cerrahisi sonrası kolorektal anastomozla ilgili en sık görülen komplikasyon anastomoz kaçağıdır. Kolorektal anastomozlarda en sık ikinci ciddi komplikasyonu anastomoz darlığıdır ve insidansı %3-30'dur. Anastomoz darlığının mortalite oranı çok yüksek olmasa da, hastaların yaşam kalitesini etkiler. Ayrıca hastalarda sık dışkılama, bağırsak tıkanıklığı ve fekal inkontinasa neden olabilir. Bu nedenle, anastomoz darlığının riski yüksek olan hastaları belirlemek ve görülme sıklığını azaltmak için önleyici tedbirler almak özellikle gereklidir. Ancak, az sayıda çalışma anastomoz darlığının risk faktörlerini araştırmış olup, çalışmalar arasında değişken sonuçlar elde edilmiştir. Bu nedenle, bu araştırmanın amacı rektum kanseri nedeniyle total mezorektal eksizyon yapılan post-operatif benign anastomoz darlığını etkileyen risk faktörleri incelemektir. Gereç- Yöntem: Çalışmamıza, 1 Ocak 2019 - 31 Aralık 2024 tarihleri arasında Ankara Bilkent Şehir Hastanesinde rektum kanseri nedenli TME yapılan 1034 hasta arasından hasta dosyaları retrospektif olarak incelenerek, 18 yaşından küçük, hartman prosedürü veya miles prosedürü uygulanan, palyatif cerrahi yapılan, rektum kanseri neden olduğu komlikasyonlar (kanama,tıkanıklık ve perforasyon) nedeniyle acil cerrahi yapılan, aynı anda birden fazla primer tümörü olanlar, operasyon sırasında karın için tespit edilen yaygın implantların olması, ameliyat öncesi tanı ve tedavi amaçlı testleri ve tetkikleri hastanemiz hasta takip sistemi vasıtası ile ulaşılamayan, preoperatif ve postoperatif değerlerine erişilemeyen 375 hasta çalışma dışı bırakıldı. Çalışma kapsamına alınan 659 hastanın dosyaları retrospektif olarak incelendi ve yaş, cinsiyet, preoperatif albümin ve hemoglobin değeri, preoperatif kemoterapi ve radyoterapi varlığı, diyabetis mellitus (DM) varlığı, sigara kullanım öyküsü, preoperatif klinik evre, ameliyat notları, postoperatif anastomoz komlikasyonları ve patoloji raporu gibi parametreler değerlendirildi. Propensity skor eşleştirme (PSM) sonrası 47 darlık gelişen ve 141 gelişmeyen olmak üzere toplam 188 hastayla analiz gerçekleştirildi. Bulgular: Çalışmamızda PSM sonrası darlık gelişen 47 hasta ile gelişmeyen 141 hasta karşılaştırıldığında; yaş, cinsiyet, preoperatif albümin ve hemoglobin düzeyleri, sigara kullanımı, diyabet varlığı, klinik evre, patolojik evre, koruyucu ostomi, operasyon türü (açık/laparoskopik/robotik/açığa konverte) ve sirküler boyutu ile darlık gelişimi arasında istatistiksel olarak anlamlı bir ilişki bulunmamıştır. Ancak; neoadjuvan radyoterapi ve/veya kemoterapi alan hastalarda, intersfinkterik rezeksiyon uygulananlarda, postoperatif anastomoz kaçağı/fistül gelişenlerde, el ile anastomoz yapılanlarda, alt ve orta rektum lokalizasyonlu tümörlerde ve splenik fleksura mobilizasyonu yapılmayanlarda darlık gelişme oranı anlamlı olarak daha yüksek bulunmuştur. Çok değişkenli analizde ise en güçlü bağımsız risk faktörleri postoperatif anastomoz kaçağı/fistül varlığı (OR: 5,259; p=0,001) olarak saptanmıştır. Sonuç: Rektum kanseri nedeniyle TME yapılan hastalarda anastomoz darlığı gelişiminde başlıca risk faktörleri; neoadjuvan tedavi, anastomoz kaçağı/fistülü, intersfinkterik rezeksiyon, elle anastomoz, tümörün alt-orta rektum yerleşimi ve yetersiz kolon mobilizasyonudur. Bu risk faktörlerine sahip hastalarda darlık gelişme olasılığı daha yüksek olduğundan, dikkatli cerrahi teknikler uygulanmalı ve postoperatif takip özenle planlanmalıdır. Çalışmamız, bu risk faktörlerini ortaya koyarak literatüre katkı sağlamış, anastomoz darlığını önlemeye yönelik stratejilere ışık tutmuştur.

Aim: Colorectal cancer is the third most commonly diagnosed cancer and the third leading cause of cancer-related mortality in both men and women. Radical resection is a reliable and effective treatment for rectal cancer; however, postoperative complications are frequently observed. Total mesorectal excision (TME) is the gold standard surgical procedure for resectable rectal cancer. The most common complication associated with colorectal anastomosis after rectal cancer surgery is anastomotic leakage. The second most common serious complication in colorectal anastomoses is anastomotic stricture, with an incidence rate ranging from 3% to 30%. Although the mortality rate of anastomotic stricture is not very high, it significantly affects patients' quality of life. In addition, it can lead to symptoms such as frequent bowel movements, bowel obstruction, and fecal incontinence. Therefore, it is essential to identify patients at high risk for anastomotic stricture and to take preventive measures to reduce its incidence. However, only a limited number of studies have investigated the risk factors for anastomotic stricture, and the results of these studies have been inconsistent. Therefore, the aim of this study is to investigate the risk factors affecting postoperative benign anastomotic stricture following total mesorectal excision for rectal cancer. Materials and Methods: In our study, the medical records of patients who underwent TME for rectal cancer at Ankara Bilkent City Hospital between January 1, 2019, and December 31, 2024, were retrospectively reviewed. Among 1,034 patients, a total of 375 patients were excluded based on the following criteria: patients younger than 18 years old, those who underwent Hartmann's procedure or Miles procedure, those who underwent palliative surgery, patients who required emergency surgery due to rectal cancer-related complications (such as bleeding, obstruction, or perforation), patients with multiple primary tumors detected simultaneously, patients with widespread intra-abdominal implants detected during surgery, and patients whose preoperative diagnostic and treatment-related tests and examinations could not be accessed through our hospital's patient follow-up system, as well as patients whose preoperative and postoperative data were unavailable.The records of the remaining 659 patients included in the study were retrospectively reviewed, and parameters such as age, gender, preoperative albumin and hemoglobin levels, presence of preoperative chemotherapy and radiotherapy, presence of Diabetes Mellitus (DM), smoking history, preoperative clinical stage, operative notes, postoperative anastomotic complications, and pathology reports were evaluated. After propensity score matching (PSM), a total of 188 patients 47 with and 141 without anastomotic stricture were included in the analysis Results: In our study, when comparing 47 patients who developed stenosis after PSM with 141 patients who did not, no statistically significant relationship was found between stenosis development and age, gender, preoperative albumin and hemoglobin levels, smoking status, presence of diabetes, clinical stage, pathological stage, presence of a protective stoma, type of surgery (open/laparoscopic/robotic/converted to open), or circular stapler size. However, the rate of stenosis development was found to be significantly higher in patients who received neoadjuvant radiotherapy and/or chemotherapy, those who underwent intersphincteric resection, those who developed postoperative anastomotic leakage/fistula, those who had hand-sewn anastomoses, patients with tumors located in the mid or lower rectum, and those in whom splenic flexure mobilization was not performed. In multivariate analysis, the most significant independent risk factor was the presence of postoperative anastomotic leakage/fistula (OR: 5.259; p=0.001). Conclusion: In patients undergoing TME for rectal cancer, the main risk factors for the development of anastomotic stricture were identified as neoadjuvant therapy, anastomotic leakage/fistula, intersphincteric resection, hand-sewn anastomosis, lower to mid-rectal tumor location, and inadequate colonic mobilization. Since patients with these risk factors have a higher likelihood of developing strictures, meticulous surgical techniques should be employed, and postoperative follow-up should be carefully planned. Our study has contributed to the literature by identifying these risk factors and has shed light on strategies to prevent anastomotic stricture
