Özet

Nörodejenerasyon, sinir sistemindeki nöronların ve sinapsların ilerleyici kaybı ile karakterize edilen karmaşık bir patofizyolojik durumdur. Bu hastalıkların patogenezinde genetik, çevresel ve yaşa bağlı faktörlerin birleşimi etkili olurken, protein yanlış katlanması, oksidatif stres, mitokondriyal disfonksiyon ve kalsiyum homeostazının bozulması gibi süreçler önemli roller üstlenmektedir. Kalsiyum homeostazındaki bozukluk, Alzheimer ve Huntington gibi nörodejeneratif hastalıkların ilerlemesini hızlandırarak mitokondriyal disfonksiyon ve apoptoz gibi süreçleri tetiklemektedir. Ancak, mevcut tedavi yöntemleri genellikle semptomatik rahatlama sağlamakla sınırlı kalmakta, hastalığın ilerlemesini durduramamaktadır. Bu bağlamda, son yıllarda eksozomlar, nörodejeneratif hastalıkların tedavisinde umut verici bir terapötik strateji olarak öne çıkmıştır. Hücreler arası iletişimi düzenlemede kritik bir rol oynayan eksozomlar, taşıdıkları biyomoleküller aracılığıyla hücre davranışlarını modüle etme potansiyeline sahiptir. Düşük immünojenisiteye sahip olmaları ve kan-beyin bariyerini aşabilmeleri sebebiyle nörolojik hastalıkların tedavisinde biyobelirteç ve taşıyıcı olarak kullanılabilmektedir. Bu çalışmada SH-SY5Y nöroblastoma ve U87 glioblastoma hücrelerinden elde edilen eksozomların, kalsiyum iyonları ile dejenere edilmiş nöronal hücrelerdeki nöroprotektif etkileri araştırılmıştır. Hedefimiz bu eksozomların anti-apoptotik (nöroprotektif) etkilerini; hücre canlılık testi, TUNEL apoptoz analizi ve qRT-PCR teknikleri ile incelemekti. MTS ve TUNEL Apoptoz Assay testi, sadece dejenere edilen nöronal hücrelere kıyasla beklediğimiz sonuçları vermiş MTS testinde % canlılık artmış TUNEL Apoptoz Assay'de ise dejenere hücre sayısı eksozom uygulanan iki grupta da anlamlı olarak düşük bulunmuştur. SH-SY5Y ve U87 hücrelerinden elde edilen eksozomların, dejenere edilmiş nöronal hücrelerde pro-apoptotik genlerden Bak'ın ekspresyonlarını ikisi de azaltırken U87 eksozomlarının pro-apoptotik genlerden Bax ve P53'ün ekspresyon seviyesini de azalttığı görülmüştür. SH-SY5Y eksozomlarının ise anti-apoptotik genlerden Bcl-xl'in ekspresyonunu artırdığı gözlemlenmiştir. MTS, TUNEL apoptoz analizi ve qRT-PCR dahil tüm bulgular, SH-SY5Y ve U87 kökenli eksozomların nöronal hücrelerde anti-apoptotik bir profil oluşturduğunu göstermektedir. Literatüre dayanarak, 10 µg/ml'lik konsantrasyonda eksozomların etkili olduğu ve en uygun uygulama süresinin 8 saat olduğu belirlenmiştir. Ancak, eksozomların farklı hücresel durumlar üzerindeki etkileri ve miRNA'lar, proteinler ve mRNA'lar gibi içeriklerinin anti-apoptotik etkiye katkıları hakkında daha fazla araştırma yapılması gerektiği anlaşılmaktadır. SH-SY5Y ve U87 hücreleri dahil tüm eksozom türlerinde bulunan çeşitli biyomoleküller ve bunların alıcı hücre tarafından işlenmesi durumu bu düşünceyi oluşturan önemli noktadır. Sonuç olarak, bu çalışma ile eksozomların dejenere edilmiş nöronal hücrelerde terapötik araçlar olarak kullanılabileceği, nörodejeneratif hastalıkların tedavisinde önemli bir potansiyel taşıdığı anlaşılmaktadır. Bu potansiyelin daha derinlemesine incelenebilmesi için farklı doz ve sürelerle eksozom uygulamaları gerekmektedir. Gelecek çalışmalar, eksozomların hücreler üzerindeki etkilerini uzun vadeli araştırmalarla ayrıntılı bir Şekilde inceleyerek nörodejeneratif hastalıkların tedavisine yönelik terapötik stratejilerin geliştirilmesine katkı sağlayacaktır.

Neurodegeneration is a complex pathophysiological condition characterized by the progressive loss of neurons and synapses within the nervous system. The pathogenesis of these diseases involves an interplay of genetic, environmental, and age-related factors, while processes such as protein misfolding, oxidative stress, mitochondrial dysfunction, and disruption of calcium homeostasis play pivotal roles. Dysregulation of calcium homeostasis accelerates the progression of neurodegenerative diseases such as Alzheimer's and Huntington's by triggering processes including mitochondrial dysfunction and apoptosis. However, current therapeutic approaches are generally limited to symptomatic relief and fail to halt disease progression. In this context, exosomes have recently emerged as a promising therapeutic strategy for the treatment of neurodegenerative disorders. Playing a critical role in intercellular communication, exosomes possess the potential to modulate cellular behaviors through the biomolecules they carry. Due to their low immunogenicity and ability to cross the blood-brain barrier, exosomes can serve as both biomarkers and delivery vehicles in the treatment of neurological diseases. In this study, the neuroprotective effects of exosomes derived from SH-SY5Y neuroblastoma and U87 glioblastoma cells were investigated on neuronal cells (SH-SY5Y) degeneratively damaged by calcium ions. Our aim was to evaluate the anti-apoptotic (neuroprotective) effects of these exosomes using cell viability assays, TUNEL apoptosis analysis, and qRT-PCR techniques. The MTS and TUNEL apoptosis assays yielded results consistent with our expectations when compared to degenerative neuronal cells alone: cell viability increased by approximately % in the MTS assay, while the number of apoptotic cells detected by the TUNEL assay was significantly lower in both exosome-treated groups relative to the degeneration-only group. Both SH-SY5Y- and U87-derived exosomes reduced the expression of the pro-apoptotic gene Bax in degenerative neuronal cells, with U87 exosomes additionally decreasing the expression levels of pro-apoptotic genes Bax and P53. Conversely, SH-SY5Y exosomes increased the expression of the anti-apoptotic gene Bcl-xL. Collectively, the findings from the MTS, TUNEL apoptosis analyses, and qRT-PCR demonstrate that exosomes derived from SH-SY5Y and U87 cells establish an anti-apoptotic profile in neuronal cells. Consistent with the literature, a dose of 10 µg/mL exosomes was found to be effective, with an optimal incubation time of 8 hours. Nevertheless, further investigation is required to elucidate the effects of exosomes on different cellular states, and to understand how their contents—such as miRNAs, proteins, and mRNAs—contribute to their anti-apoptotic effects. Particularly, the diverse biomolecules present in various exosome types, including those derived from SH-SY5Y and U87 cells, and their processing by recipient cells, represent critical aspects warranting deeper exploration. In conclusion, this study suggests that exosomes could be employed as therapeutic tools in degenerative neuronal cells, highlighting their significant potential in the treatment of neurodegenerative diseases. However, to fully realize this potential, further studies involving varied doses and durations of exosome administration are necessary. Future research aimed at long-term assessment of exosome effects on cells will provide more detailed insights and contribute to the development of therapeutic strategies targeting neurodegenerative disorders.
