Özet
Periodontal hastalık, oral biyofilm ile konak yanıtı arasındaki dengenin bozulması sonucu gelişen kronik inflamatuar bir hastalıktır. Oksidatif stres ve antioksidan savunma yetersizliği, periodontal doku yıkımını artırabilmektedir. Bu nedenle antioksidan özellik gösteren mikrobesinlerin konak yanıtını düzenleyerek hastalığın şiddetini azaltabileceği düşünülmektedir. Selenyum ise antioksidan savunmada görev alan, inflamasyonun düzenlenmesine ve doku homeostazının sürdürülmesine katkı sağlayan temel bir mikrobesindir. Güncel çalışmalar, selenyumun oksidatif stresi azaltabildiğini ve çeşitli dokularda koruyucu etkiler gösterebildiğini bildirmektedir. Bu çalışmada, deneysel periodontitis modelinde sistemik olarak farklı dozlarda uygulanan selenyumun alveolar kemik kaybı üzerindeki etkilerinin değerlendirilmesi amaçlandı. Bu kapsamda, periodontal dokularda immünohistokimyasal olarak nükleer faktör kappa B'nin reseptör aktivatör bağlayıcısı (RANKL), osteoprotogerin (OPG) ve nükleer faktör kappa B (NF-κB) ekspresyonları; histolojik olarak osteoblast, osteoklast ve inflamatuar hücre sayıları; histomorfometrik olarak alveolar kemik kaybı; biyokimyasal olarak ise sistemik total oksidan seviye (TOS), total antioksidan seviye (TAS) ve oksidatif stres indeksi (OSİ) değerlendirildi. Ayrıca selenyumun olası sistemik etkilerini belirlemek amacıyla karaciğer ve böbrek dokuları histolojik olarak incelendi. Araştırmada toplam 32 adet, 16 haftalık, 300–330 g ağırlığında erkek Wistar Albino rat kullanıldı ve hayvanlar her grupta 8 rat bulunacak şekilde 4 gruba ayrıldı. Çalışma grupları; kontrol grubu (K), deneysel periodontitis grubu (P), düşük doz selenyum uygulanan periodontitis grubu (P+DDS) ve yüksek doz selenyum uygulanan periodontitis grubu (P+YDS) olarak oluşturuldu. Deney süresi 14 gün olarak planlandı ve periodontitis gruplarında ligatür uygulamasını takiben belirlenen dozlarda selenyum uygulandı. Başlangıçta düşük doz 0,3 mg/kg, yüksek doz ise 0,6 mg/kg olarak planlanmış olmakla birlikte, 0,6 mg/kg doz uygulanan grupta erken dönemde mortalite gözlenmesi üzerine doz protokolü revize edildi. Bu doğrultuda düşük doz 0,1 mg/kg, yüksek doz ise 0,3 mg/kg olarak yeniden düzenlendi ve deney bu yeni doz protokolü ile sürdürüldü. Biyokimyasal bulgular, periodontitis grubunda oksidatif stresin arttığını ve antioksidan kapasitenin azaldığını göstermiştir. Selenyum uygulanan gruplarda TOS ve OSİ'de azalma eğilimi, TAS düzeyinde ise anlamlı artış izlenmiştir. Bu sonuçlar, selenyumun deneysel periodontitis modelinde oksidan–antioksidan dengeyi konak lehine modüle edebileceğini düşündürmektedir. Histolojik değerlendirmede, P grubunda inflamatuar sürece bağlı olarak osteoblast aktivitesinin baskılandığı, buna karşılık osteoklast sayısının ve inflamatuar hücre infiltrasyonunun arttığı görülmüştür. Selenyum uygulanan gruplarda osteoblast skorlarının P grubuna kıyasla yükselmesi, özellikle P+YDS grubunda değerlerin kontrol grubuna yaklaşması, selenyumun baskılanmış osteoblastik aktivitenin yeniden kazanılmasına katkı sağlayabileceğini düşündürmektedir. Osteoklast bulguları açısından P+DDS grubunda skorların periodontitis grubuna benzer seyretmesi rezorptif yanıtın yeterince baskılanamadığını, buna karşın P+YDS grubunda osteoklast skorundaki azalma osteoklastik aktivitenin kısmen modüle edilebildiğini göstermektedir. İnflamatuar hücre infiltrasyonunun selenyum uygulanan gruplarda anlamlı biçimde azalması ise selenyumun periodontal dokudaki yıkıcı inflamatuar süreci hafifletebileceğini güçlü şekilde desteklemektedir. İmmünohistokimyasal değerlendirmede, periodontitis grubunda RANKL ekspresyonunun arttığı, buna karşılık OPG ekspresyonunun azaldığı gözlenmiştir. Bu bulgu, deneysel periodontitisin kemik rezorpsiyonunu destekleyen RANKL/OPG dengesini osteoklastik aktivite lehine bozduğunu göstermektedir. Selenyum uygulanan gruplarda RANKL düzeylerinde periodontitis grubuna göre azalma eğilimi izlenirken, özellikle OPG ekspresyonundaki artış daha belirgin bulunmuştur. Bu sonuçlar, selenyumun periodontal dokularda kemik yıkımını doğrudan baskılamaktan çok, RANKL/OPG dengesini daha koruyucu bir yöne kaydırarak alveolar kemik kaybının sınırlandırılmasına katkı sağlayabileceğini düşündürmektedir. P grubunda NF-κB ekspresyonunun belirgin olarak arttığı gözlenmiştir. Bu bulgu, deneysel periodontitis sürecinde inflamatuar sinyal yolaklarının aktifleştiğini ve dokudaki yıkıcı yanıtın güçlendiğini göstermektedir. Selenyum uygulanan gruplarda ise NF-κB düzeylerinin periodontitis grubuna göre azaldığı saptanmıştır. Bu sonuç, selenyumun periodontal dokularda inflamatuar yanıtı baskılayarak konak cevabını daha dengeli bir yönde modüle edebileceğini düşündürmektedir. Histomorfometrik değerlendirmede, MSS–AKT mesafesine dayalı alveolar kemik kaybı ölçümlerinde gruplar arasında istatistiksel olarak anlamlı fark saptanmıştır. Çoklu karşılaştırmalarda K, P ve P+DDS gruplarının aynı alt grupta yer aldığı, buna karşın P+YDS grubunun farklı bir alt grupta bulunduğu ve bu grupta alveolar kemik kaybının diğer gruplara göre beklenmedik biçimde daha yüksek olduğu ölçülmüştür. Bu bulgu, selenyumun immünohistokimyasal ve biyokimyasal düzeyde daha koruyucu bir profil göstermesine karşın, özellikle P+YDS grubunda histomorfometrik kemik kaybını azaltmadığını ortaya koymaktadır. Dolayısıyla selenyumun alveolar kemik yıkımı üzerindeki etkisinin; doz, uygulama süresi ve rezorpsiyonun zamana bağlı kümülatif doğası gibi değişkenlerden etkilenebileceği düşünülmektedir. Karaciğer ve böbrek dokularının histolojik incelemesinde, selenyum uygulanan gruplarda hafif ve orta düzeyde histopatolojik değişiklikler gözlenmiştir. Karaciğerde hepatosit vakuolizasyonu, sinüzoidal genişleme ve portal alanda inflamatuar hücre infiltrasyonu; böbrekte ise daha çok tubulointerstisyel düzeyde değişiklikler saptanmıştır. Bununla birlikte her iki organda da nekroz ve fibrozisin izlenmemesi, gözlenen bulguların ileri ve geri dönüşümsüz organ hasarından çok, doza bağlı gelişen ve büyük ölçüde geri dönüşümlü olabilecek erken dönem doku yanıtını yansıttığını düşündürmektedir. Selenyum deneysel periodontitis modelinde antioksidan kapasiteyi destekleyen, NF-κB aracılı inflamatuar yanıtı baskılayan ve OPG artışı üzerinden RANKL/OPG dengesini kemik koruyucu yönde modüle eden potansiyel bir konak modülatör ajan olarak değerlendirilmiştir. Buna karşın, histomorfometrik olarak yüksek doz grubunda alveolar kemik kaybının artmış bulunması, selenyumun kemik yıkımı üzerindeki etkilerinin doz ve doku mikroçevresine bağlı olarak değişebileceğini düşündürmektedir. Karaciğer ve böbrekte geri dönüşümsüz hasar saptanmamış olmakla birlikte, yüksek dozda gözlenen mortalite selenyumun dar terapötik aralığını ve doz bağımlı toksisite potansiyelini ortaya koymaktadır. Bu nedenle, klinik etkinlik ve güvenlilik profilinin netleştirilmesi için farklı doz, süre ve formülasyonları içeren ileri çalışmalara ihtiyaç duyulmaktadır.

Periodontal disease is a chronic inflammatory disease that develops as a result of the disruption of the balance between oral biofilm and host response. Oxidative stress and antioxidant defense deficiency can increase periodontal tissue destruction. Therefore, it is thought that micronutrients exhibiting antioxidant properties can reduce the severity of the disease by regulating the host response. Selenium, on the other hand, is an essential micronutrient that plays a role in antioxidant defense, contributing to the regulation of inflammation and the maintenance of tissue homeostasis. Current studies report that selenium can reduce oxidative stress and show protective effects in various tissues. This study aimed to evaluate the effects of systemically administered selenium at different doses on alveolar bone loss in an experimental periodontitis model. Specifically, immunohistochemical measurements of nuclear factor kappa B receptor activator binder (RANKL), osteoprotogerin (OPG), and nuclear factor kappa B (NF-κB) expression were performed in periodontal tissues; histological measurements of osteoblast, osteoclast, and inflammatory cell counts were taken; histomorphometric measurements of alveolar bone loss were performed; and biochemical measurements of systemic total oxidant level (TOS), total antioxidant level (TAS), and oxidative stress index (OSI) were evaluated. Additionally, liver and kidney tissues were histologically examined to determine the potential systemic effects of selenium. A total of 32 male Wistar Albino rats, 16 weeks old and weighing 300–330 g, were used in the study and divided into 4 groups of 8 rats each. The study groups were: control group (K), experimental periodontitis group (P), low-dose selenium-treated periodontitis group (P+DDS), and high-dose selenium-treated periodontitis group (P+YDS). The experiment was planned for 14 days, and selenium was administered at the determined doses following ligation in the periodontitis groups. Although the initial dose was planned as 0.3 mg/kg (low) and 0.6 mg/kg (high), the dose protocol was revised due to early mortality observed in the 0.6 mg/kg group. Accordingly, the low dose was readjusted to 0.1 mg/kg and the high dose to 0.3 mg/kg, and the experiment was continued with this new dose protocol. Biochemical findings showed increased oxidative stress and decreased antioxidant capacity in the periodontitis group. In the selenium-treated groups, a tendency towards decrease in TOS and OSI was observed, while a significant increase in TAS levels was seen. These results suggest that selenium may modulate the oxidant-antioxidant balance in favor of the host in an experimental periodontitis model. Histological evaluation revealed that in group P, osteoblast activity was suppressed due to the inflammatory process, while osteoclast number and inflammatory cell infiltration increased. The increase in osteoblast scores in the selenium-treated groups compared to group P, especially the values approaching the control group in the P+YDS group, suggests that selenium may contribute to the recovery of suppressed osteoblastic activity. In terms of osteoclast findings, the similarity of scores in the P+DDS group to the periodontitis group indicates that the resorptive response was not sufficiently suppressed, while the decrease in osteoclast scores in the P+YDS group shows that osteoclastic activity was partially modulated. The significant decrease in inflammatory cell infiltration in the selenium-treated groups strongly supports the idea that selenium may alleviate the destructive inflammatory process in periodontal tissue. Immunohistochemical evaluation revealed increased RANKL expression and decreased OPG expression in the periodontitis group. This finding indicates that experimental periodontitis disrupts the RANKL/OPG balance, which supports bone resorption, in favor of osteoclastic activity. While selenium-treated groups showed a tendency towards decreased RANKL levels compared to the periodontitis group, the increase in OPG expression was particularly pronounced. These results suggest that selenium may contribute to limiting alveolar bone loss by shifting the RANKL/OPG balance in a more protective direction, rather than directly suppressing bone resorption in periodontal tissues. In group P, a significant increase in NF-κB expression was observed. This finding indicates that inflammatory signaling pathways are activated and the destructive response in the tissue is strengthened during the experimental periodontitis process. In the selenium-treated groups, NF-κB levels were found to be decreased compared to the periodontitis group. This result suggests that selenium may modulate the host response in a more balanced direction by suppressing the inflammatory response in periodontal tissues. In histomorphometric evaluation, statistically significant differences were found between groups in alveolar bone loss measurements based on the CNS–ACT distance. In multiple comparisons, the K, P, and P+DDS groups were found to be in the same subgroup, while the P+YDS group was in a different subgroup, and alveolar bone loss was unexpectedly higher in this group compared to the other groups. This finding reveals that although selenium shows a more protective profile at the immunohistochemical and biochemical levels, it does not reduce histomorphometric bone loss, especially in the P+YDS group. Therefore, it is thought that the effect of selenium on alveolar bone resorption may be influenced by variables such as dose, duration of administration, and the time-dependent cumulative nature of resorption. Histological examination of liver and kidney tissues revealed mild to moderate histopathological changes in the selenium-treated groups. In the liver, hepatocyte vacuolization, sinusoidal dilation, and inflammatory cell infiltration in the portal space were observed; in the kidney, changes were primarily seen at the tubulointerstitial level. However, the absence of necrosis and fibrosis in either organ suggests that the observed findings reflect a dose-dependent, largely reversible, early tissue response rather than advanced and irreversible organ damage. Selenium has been evaluated as a potential host modulator agent in an experimental periodontitis model, supporting antioxidant capacity, suppressing NF-κB-mediated inflammatory response, and modulating the RANKL/OPG balance in a bone-protective direction via increased OPG. However, the increased alveolar bone loss observed histomorphometrically in the high-dose group suggests that the effects of selenium on bone resorption may vary depending on the dose and tissue microenvironment. Although irreversible liver and kidney damage was not detected, the mortality observed at high doses highlights selenium's narrow therapeutic range and dose-dependent toxicity potential. Therefore, further studies involving different doses, durations, and formulations are needed to clarify its clinical efficacy and safety profile.
