Özet

Kanser, görülme sıklığı ve mortalite bakımından dünya genelinde önemli sağlık sorunları arasında üst sıralarda yer almaktadır. Beyin kanserleri, toplumda görülme sıklığı az olan fakat kısa sağkalım süresi ve agresif gelişimi ile karakterizedir. Glioblastoma ise beyin tümörleri arasında en sık görülen ve agresif olan kötü huylu beyin tümörü olup, kemoterapiye direnci tedaviyi zorlaştırmaktadır. Glioblastomanın kemoterapiye karşı dirençli olması tanı alan hastaların yaşam süresini oldukça kısaltan bir faktördür. Temozolomid, glioblastoma tedavisinde en sık kullanılan kemoterapötik ajanlardan biri olmasına rağmen, glioblastomanın bu ajana karşı direnç göstermesi nedeniyle terapötik etkisi sınırlanmaktadır. Ayrıca, temozolomid ve benzeri kemoterapötik ajanların sağlıklı dokularda meydana getirdiği sitotoksisite sebebiyle de hastalarda yan etkiler meydana gelmektedir. Bu tez kapsamında, literatürde antikanser etkinliği kanıtlanmış olan steroid olmayan antiinflamatuvar ajan indometasinin, hedeflendirilmiş ve pH/redoks duyarlı glikotriblok polimerik ilaç taşıyıcı sisteme konjugasyonu ile antikanser etkinliğinin serbest ilaca göre arttırılabileceği ve biyouyumlu glikotriblok sistem sayesinde sağlıklı hücrelerde sitotoksisitenin azaltılabileceği hipotezi öne sürülerek, U87-MG insan glioma hücrelerinde antikanser etkisinin araştırılması amaçlanmıştır. Hücreler uygun doz aralıkları ve zamanlarda serbest indometasin ve indometasin konjuge glikotriblok ile muamele edilerek ilaç taşıyıcı sistemin antikanser etkinliği hücresel ve moleküler düzeyde in vitro çalışmalar ile değerlendirilmiştir. Sitotoksisite, hücre çevrimi ve apoptoz analizleri ile antikanser etkinlik incelenirken, hücresel alım ile hedeflendirme mekanizmasının avantajı, siklooksijenaz enzim seviyelerinin belirlenmesi ile de hedeflendirme ve ilaç salım etkinliği değerlendirildi. Elde edilen bulgular, uygun doz ve zaman noktasında indometasin konjuge glikotriblok polimer, serbest indometasin ve hedefsiz ve ilaçsız glikotriblok polimer ile muamele edilen hücrelerde indometasin konjuge glikotriblok polimerindeki şeker üniteleri (glukoz bloğu) ile GLUT reseptörlerini hedefleyebilmesi, TPGS ile çoklu ilaç direnci mekanizmasını baskılayarak hücre içi tutulumu arttırması ve disülfit bağı varlığında yüksek GSH konsantrasyonuna sahip tümör mikroçevresinde kontrollü ilaç salımı gerçekleştirebilmesinden dolayı antikanser etkinliğinin serbest ilaca oranla daha yüksek olduğu ve biyouyumlu yapısı sayesinde sağlıklı kontrol hücresinde serbest ilaca oranla daha düşük sitotoksisite gösterdiği belirlenmiştir. Sonuç olarak, hedeflendirilmiş ve redoksa duyarlı glikotriblok polimerik ilaç taşıyıcı sistemin glioblastoma tedavisinde etkili bir alternatif yaklaşım olabileceğini göstermektedir.

Cancer remains one of the leading global health concerns in terms of both incidence and mortality. Brain cancers are relatively rare within the population; however, they are characterized by aggressive progression and short survival times. Glioblastoma is the most common and aggressive malignant brain tumor, and its resistance to chemotherapy significantly complicates treatment. The intrinsic resistance of glioblastoma to chemotherapeutic agents is a major factor contributing to the poor prognosis and reduced survival of diagnosed patients. Although temozolomide is one of the most widely used chemotherapeutic agents in glioblastoma treatment, its therapeutic efficacy is limited due to the development of resistance. Furthermore, temozolomide and similar chemotherapeutic drugs can induce cytotoxic effects in healthy tissues, leading to various adverse side effects in patients. Within the scope of this thesis, it was hypothesized that the anticancer efficacy of indomethacin, a nonsteroidal anti-inflammatory drug with well-documented anticancer activity in the literature, could be enhanced through its conjugation to a targeted and pH/redox-responsive glyco-triblock polymeric drug delivery system. It was further proposed that the biocompatible glyco-triblock carrier could reduce cytotoxicity in healthy cells. Accordingly, the anticancer effects of this system were investigated in U87-MG human glioma cells. Cells were treated with free indomethacin and indomethacin-conjugated glyco-triblock polymer at appropriate concentrations and incubation periods, and the anticancer efficacy of the drug delivery system was evaluated through a series of in vitro cellular and molecular studies. Anticancer activity was assessed by cytotoxicity, cell cycle, and apoptosis analyses, while the advantages of the targeting mechanism were evaluated through cellular uptake studies. In addition, the targeting and drug-release performance of the system were investigated by determining cyclooxygenase enzyme levels. The findings demonstrated that, at the optimal dose and treatment duration, the indomethacin-conjugated glyco-triblock polymer exhibited greater anticancer efficacy than free indomethacin, as well as non-targeted and drug-free glyco-triblock polymer formulations. This enhanced activity was attributed to the glucose units within the polymer structure, which enabled targeting of GLUT receptors, the presence of TPGS, which suppressed multidrug resistance mechanisms and increased intracellular retention, and the disulfide linkage, which facilitated controlled drug release within the glutathione-rich tumor microenvironment. Moreover, owing to its biocompatible nature, the indomethacin-conjugated glyco-triblock polymer exhibited lower cytotoxicity in healthy control cells compared with free indomethacin. Overall, these results suggest that the targeted and pH/redox-responsive glyco-triblock polymeric drug delivery system represents a promising alternative therapeutic strategy for the treatment of glioblastoma.
