Özet

Bu çalışma, Parkinson hastalarında gövde ve alt ekstremiteye yönelik geliştirilen görsel bildirimli egzersiz protokolünün vücut işlevleri ve yapıları, aktivite ve katılıma etkisini araştırmak amacıyla gerçekleştirildi. Çalışmaya dahil edilen 32 Parkinson hastası; 8 hafta, haftada 3 gün egzersiz eğitimi almak üzere iki gruba randomize edildi. Klasik grup (n=16) konvansiyonel egzersiz eğitimi aldı. Çalışma grubu (n=16) konvansiyonel egzersizlere ek gövde ve alt ekstremiteye yönelik lazer imleç ile görsel bildirimli egzersiz eğitimi aldı. Tedavi öncesi ve sonrasında Birleşik Parkinson Derecelendirme Ölçeği (BPDÖ), propriyosepsiyon ölçümleri, Görsel Analog Skalası, New York Postür Değerlendirmesi, Parkinson Yorgunluk Ölçeği, dinamik ve statik postüral salınım değerlendirmesi, Berg Denge Ölçeği, Fonksiyonel Uzanma Testi, L testi, Aktiviteye Özgü Denge Güven Ölçeği, Yürürken Donma Ölçeği, 10 Metre Yürüme Testi, Beş Kez Otur Kalk Testi ve Parkinson Hastalığı Ölçeği-39 uygulandı. Çalışma grubunun tüm BPDÖ skorları (p<0,05), ağrı (p=0,005) ve postür (p=0,026) bulgularında iyileşme görülürken, klasik grupta değişim olmadı. Çalışma grubunda sol ayak bileği dışında tüm eklemlerdeki propriyosepsiyon ölçümlerinde iyileşme elde edilirken (p<0,05); klasik grupta yalnızca sağ ayak bileği dorsi fleksiyonunda (p=0,028) iyileşme gözlendi. Çalışma grubunda dinamik ve statik postüral salınımlarda iyileşme elde edilirken (p<0,05); klasik gruptaki gelişim daha sınırlıydı (p<0,05). Her iki grupta da denge, denge güveni, yürüyüş hızı ve mobilitede iyileşme bulundu (p<0,05). Çalışma grubunda yaşam kalitesinde artış görülürken (p=0,026), klasik grupta değişim olmadı. Klasik grupta yorgunlukta azalma görülürken (p=0,025), çalışma grubunda değişim olmadı. Her iki grupta da yürüyüş donmasında değişim olmadı (p>0,05). İki grup karşılaştırıldığına çalışma grubunun motor semptomlarda (p<0,05), sağ ayak bileği ve sağ diz propriyosepsiyonunda (p<0,05), postürde (p=0,016), dinamik ve statik postüral kontrolde mediolateral yöndeki parametrelerde (p<0,05), yürüyüş hızında (p=0,044) ve mobilitede (p=0,018) üstünlük sağladığı bulundu. Sonuç olarak; görsel bildirimli egzersiz eğitimi, vücut işlevleri ve aktivite durumuna olumlu etki ederek Parkinson semptomlarında fayda sağlamaktadır. Bu kolay uygulanabilir ve ulaşılabilir yöntemin, özellikle postüral kontrolde mediolateral yönde gelişimin hedeflendiği rehabilitasyon programları için tercih edilebilir bir yöntem olarak klinik uygulamalara entegre edilebileceği görüşündeyiz.

This study was conducted to investigate the effect of a visual feedback exercise protocol developed for the trunk and lower extremities on body functions and structures, activity, and participation in patients with Parkinson's disease. Thirty-two patients with Parkinson's disease included in the study were randomized into two groups to receive exercise training 3 days a week for 8 weeks. The conventional group (n=16) received conventional exercise training. The study group (n=16) received visual feedback exercise training using a laser pointer targeted at the trunk and lower extremities in addition to conventional exercises. Before and after treatment, the Unified Parkinson's Disease Rating Scale (UPDRS), proprioception measurements, Visual Analog Scale, New York Posture Rating, Parkinson Fatigue Scale, dynamic and static postural sway assessment, Berg Balance Scale, Functional Reach Test, L-Test, Activities-specific Balance Confidence Scale, Freezing of Gait Questionnaire, 10-Meter Walk Test, Five Times Sit-to-Stand Test, and Parkinson's Disease Questionnaire-39 were administered. While improvements were observed in all UPDRS scores (p<0.05), pain (p=0.005), and posture (p=0.026) in the study group, no change occurred in the conventional group. In the study group, improvements were achieved in proprioception measurements across all joints except the left ankle (p<0.05); in contrast, only right ankle dorsiflexion showed improvement in the conventional group (p=0.028). Improvements in dynamic and static postural sway were obtained in the study group (p<0.05), whereas progress in the conventional group was more limited (p<0.05). Both groups demonstrated improvements in balance, balance confidence, gait speed, and mobility (p<0.05). Quality of life increased in the study group (p=0.026), while no change was observed in the conventional group. A reduction in fatigue was noted in the conventional group (p=0.025), whereas no change occurred in the study group. Neither group showed a change in freezing of gait (p>0.05). When comparing the two groups, the study group demonstrated superiority in motor symptoms (p<0.05), right ankle and right knee proprioception (p<0.05), posture (p=0.016), mediolateral parameters in dynamic and static postural control (p<0.05), gait speed (p=0.044), and mobility (p=0.018). In conclusion, visual feedback exercise training provides benefits in Parkinson's symptoms by positively affecting body functions and activity status. We believe that this easy-to-apply and accessible method can be integrated into clinical practice as a preferred approach, particularly for rehabilitation programs aiming to improve postural control in the mediolateral direction.
