Özet
GİRİŞ VE AMAÇ: Demodex cinsi akarlar, insan derisinin kalıcı mikrobiyotasının bir parçası olan ve en sık bulunan zorunlu ektoparazitlerdir. Fototerapinin neden olduğu lokal immünsupresyon ve sebase bezlerdeki olası morfolojik değişimlerin, demodex popülasyonunda değişime yol açıp açmadığını saptamak ve bu süreçte fotoproteksiyon amaçlı güneş koruyucu kullanımının parazit yoğunluğu üzerindeki etkisini incelemek hedeflenmiştir. YÖNTEM: Çalışmaya, kliniğimizde çeşitli tanılarla fototerapi planlanan 71 hasta dahil edildi. Demodex yoğunluğu ölçümü için tedavi öncesi (0. seans) ve tedavi sonrası (20. seans) dönemlerde sağ yanak, sol yanak, alın ve burun bölgelerinden Standart Yüzeysel Deri Biyopsisi (SSSB) yöntemiyle örnekler alındı. Verilerin istatistiksel analizinde Statistical Package for Social Science (SPSS) versiyon 26.0 yazılımı kullanıldı. BULGULAR: Çalışmaya katılan 71 hastanın toplam yüz parazit yoğunluğu medyan değeri tedavi başında 3,00 iken, 20 seanslık fototerapi sonunda 4,00'e yükselmiş ve bu artış istatistiksel olarak anlamlı bulundu (p=0,010). Bölgesel analizlerde sol yanak (p=0,008) ve burun (p=0,006) bölgelerinde parazit yükünde anlamlı artış saptandı. Güneş koruyucu kullanan (n=25) ve kullanmayan (n=46) gruplar arasında hem bölgesel yoğunluklar hem de seanslar arası değişim miktarları (delta değerleri) açısından istatistiksel olarak anlamlı bir fark saptanmadı (p>0,05). SONUÇ: Çalışma sonuçları, 20 seanslık NB-UVB uygulamasının yüzdeki toplam demodex yoğunluğunu artırdığını göstermektedir. Güneş koruyucu kullanımının bu artış üzerinde engelleyici bir etkisinin saptanmaması, fototerapinin deri üzerindeki immünolojik ve fizyolojik etkilerinin topikal fotoproteksiyonun ötesinde olduğunu düşündürebilir. Bu bulgular ışığında, fototerapi alan hastaların tedavi sürecinde fırsatçı demodikozis gelişimi açısından klinik olarak izlenmesi önerilmektedir.

Evaluation of demodex density in patients receiving narrowband uvb phototherapy AIM: Demodex mites are obligate ectoparasites that represent the most common constituents of the permanent human skin microbiota. This study aimed to determine whether local immunosuppression and potential morphological changes in sebaceous glands induced by phototherapy lead to alterations in the Demodex population, and to evaluate the impact of sunscreen use for photoprotection on mite density during this process. MATERIALS AND METHODS: A total of 71 patients scheduled to receive phototherapy for various diagnoses in our clinic were included in the study. To assess Demodex density, samples were obtained from the right cheek, left cheek, forehead, and nose using the Standardized Skin Surface Biopsy (SSSB) method before treatment (session 0) and after treatment (session 20). Statistical analyses were performed using Statistical Package for the Social Sciences (SPSS) version 26.0 software. RESULTS: The median total facial parasite density of the 71 patients included in the study increased from 3.00 at baseline to 4.00 after 20 sessions of phototherapy, and this increase was statistically significant (p=0.010). Regional analyses revealed a significant increase in parasite density in the left cheek (p=0.008) and nose (p=0.006). No statistically significant differences were observed between sunscreen users (n=25) and non-users (n=46) in terms of either regional parasite densities or the magnitude of change between sessions (delta values) (p>0.05). CONCLUSION: The results of this study demonstrate that 20 sessions of NB-UVB application increase the total facial Demodex density. The lack of an inhibitory effect of sunscreen use on this increase suggests that the immunological and physiological effects of phototherapy on the skin extend beyond topical photoprotection. In light of these findings, clinical monitoring of patients undergoing phototherapy for the development of opportunistic demodicosis during the treatment process is recommended.
