Özet
ÖZET Kasım 2001 - Mayıs 2003 tarihleri arasında gece idrar kaçırma yakınması ile polikliniğimize müracaat eden 48 erişkin erkek hasta çalışmaya alındı. Ayrıntılı biyokimyasal, radyolojik ve nörolojik incelemeden sonra tüm hastalara dolum sistometrisi uygulandı. Hastaların ortalama mesane kapasiteleri 400 mi. bulundu. Aşırı aktif mesane sendromu tespit edilen 8 hastaya antikolinerjik tedavi başlanarak çalışmadan çıkarıldı. Kırk hastaya Beck depresyon testi uygulandıktan sonra 0.2 mg desmopressin tedavisi başlandı. İki hafta sonra, haftalık ıslatma sıklığı %50 ve üzeri azalmayan hastalarda doz 0.4 mg'a yükseltildi. Bir ayın sonunda ilaca yanıtsız olan 5 (%12) hastada tedaviye devam edilmedi. Desmopressin tedavisine yanıt veren 35 (%88) hastanın 30'una 0.2 mg, 5 hastaya da 0.4 mg lık dozlarda Desmopressin Asetat 6 ay boyunca verildi. İlaç kullanım öncesi haftalık ortalama ıslatma sıklığı 5.7±1.5 olarak saptandı, ikinci ay sonunda 7(%20) hastada tam kuruluk sağlandı. Haftalık ortalama ıslatma sıklığı 1.4±1 bulundu. Dördüncü ayda 17(%48) hasta tam kuru ve haftalık ortalama ıslatma sıklığı 0.7±0.8, altıncı ay sonunda ise 21(%60) hastada tam kuru ve haftalık ortalama ıslatma sıklığı 0.6±0.8 bulundu. Tedavisiz 1 ay sonunda 7(%20) hasta tam kuru, haftalık ortalama ıslatma sıklığı 1.1±0.7 olarak tespit edildi. Tedavi öncesi Beck depresyon ortalama skoru 19.3 olan hastaların tedavi sonrası ortalama skoru 1 1.4 olarak bulundu. Tedavi öncesi değerlerle kıyaslandığında 2.,4., 6. ay ve tedavisiz 1 ay sonundaki ortalama ıslatma sıklıkları ve 6. ay ortalama Beck depresyon skoru istatistiksel olarak belirgin farklı bulundu (p=0.001). Aile öyküsü varlığı ile ilaca yanıt arasında anlamlı ilişki bulunamadı (p=0.513). Islatma sıklıklanndaki düzelmenin hangi parametreden etkilendiğini saptamak amacı ile yapılan çok değişkenli analizde ilaca yanıt ile parametreler arasında anlamlı ilişki saptanmadı. Çalışmamız sonucunda tek semptomlu erişkin enüretik olguların ürodinamik olarak normal popülasyona benzer özellikler gösterdiği ve bu olgularda ürodinami yapmanın pratik bir yarar sağlamadığı düşünüldü. Birinci ayın sonunda tedaviye yanıt veren olgular oral DDAVP tedavisine tedavi süresi ile artan oranda yanıt vermekte, ancak bu yanıt tedavinin kesilmesi ile belirli ölçüde azalmaktadır. 26

SUMMARY From November 2002 to May 2003, 48 adult male patients referring for nocturnal enuresis were included in the study. All subjects had a detailed biochemical, radiological and neurological evaluation and filling cystometric analysis was performed to all of them. Their mean bladder capacity was 400 ml and eight patients with overactive bladder were excluded from the study and treated with anticholinergics. Following the application of the Beck depression test these 40 patients were administered a daily dose of 0.2 mg desmopressin. In subjects, with less than %50 reduction in weekly wetting frequency the dose was adjusted to 0.4 mg. The patients were followed-up for 1 months and at the end of that period 30 subjects receiving 0.2 mg desmopressin and 5 subjects receiving 0.4 mg. desmopressin were found responsive to the treatment. The treatment was discontinued in 5 nonresponsive patients (%12). 35 of the patients (%88) have received their treatment during 6 months. The weekly wetting frequency of these subjects was 5.7±1.5 before the treatment at the 2 nd, 4 nd, and 6 nd month of the follow-up the weekly wetting frequencies and the total dryness rates were found 1.4±1 and %20, 0.7±0.8 and %48, 0.6±0.8 and %60 respectively. When they were followed for 1 month without treatment the wetting frequency was found 1.1± 0.7 and the dryness rate was %20. The patients had a pretreatment mean Beck depression score of 19.3, which was reduced to 1 1.4 by 6 months. The two monthly reduction in the wetting frequencies, increase in a 1 month period without treatment and reduction in the Beck depression score following the treatment were found statistically significant (p=0.001). Positive family history and the response to the treatment did not correlate (p= 0.513). In a multivariate analysis, none of the parameters was found to affect the improvement rate of the wetting frequency. Our monosymptomatic, adult enuretic patient group was found similar to the normal population as to their urodynamics and it was thought that there was no need to a urodynamic evaluation for these patients.
