Özet

1. ÖZET Helicobacter pylori Antijen ve DNA' sının Dışkıda, IgG Antikorunun serumda Saptanması, İnvaziv ve İnvaziv Olmayan Tanı Yöntemlerinin Karşılaştırılması Helicobacter pylori (II. pylori) peptik ülser, gastrik kanser ve gaslrik lenfoma (MALT) etiyolojisinde önemli yeri olan, sınıl' I karsinojen olarak tanımlanmış, kronik enfeksiyon etkeni Gram negalif hakleridir. Çalışmamızda; H. pylori tanısında anti-//. pylori IgG F.LISA, Western blot, dışkı antijen testi, dışkı PZR altın standart yöntemler olan hızlı üreaz testi ve histopatoloji ile tanıdaki duyarlılık, seçicilik ve uygulama kolaylıkları yönünden araştırıldı. Mayıs-PC&sım 20071 tarihleri arasında dispeptik yakınmaları nedeniyle Dokuz Eylül Üniversitesi Tıp Fakültesi Gastroenteroloji Polikliniği'ne başvuran ve endoskopi endikasyonu konulan 54 olgu (37 kadın. 17 erkek yaş ortalaması 46.48 ± 13.12 SD; yaş aralığı 21-78) çalışmaya alındı. Hastalardan alınan iki antrum, iki korpus. dört biyopsi örneğine hızlı üreaz testi ve histopatolojik inceleme uygulandı. H. pylori enfeksiyonu histopatoloji ve/veya hızlı üreaz testi olumluluğu. H. pylori negatifliği ise altın standart testlerin her ikisininde olumsuzluğu ile belirlendi. Ilematoksilen-eozin. Alcian blue ve Giemsa boyaları kullanılarak morfolojik olarak“Helicobacter-lİks organisms (111. O) yönünden incelendi. Gastrik aktivite ve H. pylori yönünden bakteriyel yoğunluk güncelleştirilmiş Sydney sistemine göre değerlendirildi. F.ndoskopi ile eş zamanlı alınan serum örneklerinde anti-//. pylori IgG antikorları F.LISA (FUROİMMUN Medizinische Labordiagnostika. Lübeck). Western blot (FUROİMMUN Medizinische Labordiagnostika. Lübeck); dışkı örneklerinde//, pylori antijeni dışkı antijen FLİSA (PRİ Mil. R PLATINUM IlpSA. Meridian Bioscience, ine) testi ve yeni geliştirilen dışkı P/R yöntemi ile çalışıldı. Toplam 54 hastanın 4(>'sı (% 85.2) altın standart yöntemlere göre //. pylori enfeksiyonu olumlu, seki/i (''<' 14.8) olumsuz bulundu. Ellidört hastanın 47'sı (% 87) anti-//. pylori IgG FLİSA testi ile olumlu, 7'si (% 13) olumsuz saptandı. Duyarlılık % 'M.3: seçicilik % 37.5. olabilirlik oranı 1.46, olumluöngörü değeri % 89.36. olumsuz öngörü değeri ise % 42.85 olup. altın standart yöntemler ile istatistiksel anlamlı fark gözlenmedi (p~l ). Anti-H. pylori IgG Western blot testi ile 46'sı (% 85.2) olumlu. 3'ö (% 5.6) olumsuz. 5'i (% 0.3) belirsiz olarak saptandı. Belirsiz sonuçlar dahil edilmediğinde; duyarlılık % 07.61; seçicilik % 28.57 olabilirlik oranı 1.37, olumlu öngörü değeri % 80.13. olumsuz öngörü değeri ise % 66.66 bulundu. HpSA dışkı antijen ELISA testi ile 25'i (% 46.3) olumlu. 29'u (% 53.7) olumsuz olup. altın standart yöntemler ile istatistiksel olarak anlamlı fark gözlendi (p< 0.0001 ). Duyarlılık % 54.35, seçicilik % 100, olumlu öngörü değeri % 100, olumsuz, öngörü değeri ise % 27.50 bulundu. Dışkı PZR yöntemi ile 32”si (% 59.3) olumlu. 22“si (% 40.7) olumsuz olup, altın standart yöntemler ile arasında istatistiksel olarak anlamlı fark görüldü (p<0.001). Duyarlılık % 65.22, seçicilik % 75, olabilirlik oranı 2.61. olumlu öngörü değeri % 93.75, olumsuz öngörü değeri % 27.7 saptandı. Çalışmamızda anti-Hpylori IgG F.LISA testinde duyarlılığın literatür ile uyumlu, seçiciliğin düşük bulunması; tedavi edilmemiş hastalarda IgG antikorlarının persistansına, kullanılan serolojik kitin eşik değerinin hasta grubu için uygunsuzluğuna veya H. pylori suşlarınm genetik farklılığına bağlandı. Anti-Hpylori IgG Western blot testinin duyarlılık ve olumlu öngörü değeri literatür ile uyumlu, seçicilik ve olumsuz öngörü değeri düşük olup bu sonuç önceki H. pylori enfeksiyonunun ”serolojik skar“ı veya Helicobacter heilmannii gibi benzer antijenik yapıya sahip Helicobacter türü ile enfeksiyona bağlandı. H. pylori yama tarzında enfeksiyon oluşturduğundan F.LISA ve Western blot olumlu altın standart yöntemlerle olumsuz, hastaların gerçekte enfekte olabileceği düşünüldü. Dışkı antijen testinde seçiciliğin yüksek olmasına rağmen duyarlılığın düşüklüğü Hpylori'nin dışkıdan atılan miktarının saptama sınırının altında olmasına, belirlenen eşik değerin hasta grubumuz için uygun olmamasına, kitle tek bir H. pylori susunun kullanılmasına bağlandı. Alarm semptomları olmayan 4; yaş altı hastalarda Hpylori tanısında invaziv olmayan yöntemlerin kullanılması, seçilen serolojik kitin, dışkı antijen testinin veya serolojik kitin populasyon için uygunluğunun saptanması gerektiği sonucuna varıldı.H. pylori DNA”sının dışkı PZR yöntemi ile saptanmasında; filtrasyon. antikorla kaplanmış manyetik boncuk gibi ek PZR basamakları yerine uygulanımı kolay ve ucuz yeni bir protokol geliştirildi. DNA ekstraksiyonunda“QlAamp®DNA Stool Mini Kit”kullanıldı. Dışkı PZR yönteminin Hpylori enfeksiyonunun tanısının yanısıra: dışkıdan virulans genlerinin saptanmasında, ilaç direnci çalışmalarında, mutasyonların belirlenmesinde invaziv olmayan bir yöntem olarak klinik ve laboratuvar uygulamalarında kullanılabileceği sonucuna varıldı.

2. SUMMARY Detection of Helicobacter pylori antigen and DNA in stool specimens, IgG antibodies in sera; comparison of invasive and noninvasive diagnostic methods Helicobacter pylori (H. pylori) is a Gram negative bacteria, which is the major agent causing peptic ulcer, gastric cancer and MAI.T gastric lymphoma as a class I carcinogen. In this study anti H. pylori IgG ELISA. Western blot, stool antigen test, stool PCR as noninvasive diagnostic methods were compared with the rapid urease test and histopathologic examination as the gold standard techniques for their specificity, sensitivity, PPV, NPV and convenience for the diagnosis of//, pylori infection. The study population consisted of 54 H. pylori infected and non-infected adult patients (IX males, 36 females; mean age, 46.41 + 13.12 years; age range, 21 to 78 years) from Outpatient Gastroenterology Service at Dokuz Eylül University Hospital between May and November 2003. All patients had dyspeptic symptoms and were referred to a diagnostic upper endoscopy. Two antrum and two corpus biopsy specimens were taken from each patient. Infection by //. pylori was defined as positivity of histopathology and/or positivity of rapid urease test. A patient was classified as being H. pylori negative when histopathologic^ examination and urease test were all negative. Haematoxylin-eosin, Alcian blue and Gicmsa stains were used for morphologic examination of Helicohacter-liVs organisms (HLO). Updated Sydney system were also used for gastritis activity and to grade bacterial density of//, pylori. Sera and stool specimens of these patients were examined by anti-//. pylori IgG ELISA (EUROIMMUN Medizinischc Labordiagnostika. Liibeck). IgG Western blot (F.UROIMMUN Medizinischc Labordiagnostika. Liibeck), stool antigen test (PREMIER PLATINUM HpSA, Meridian Bioscience. Inc.) and newly developed stool PCR methods. Among these 54 patients, 46 of them (85.2 rî ) were diagnosed positive and eight of them ( 14.X %) were diagnosed negative for //. pylori infection by the gold standard methods. Among the 54 patients. 47 (87 %) were diagnosed positive and seven of them ( 13 '“i ) were diagnosed negative by anti-//. pylori IgG ELISA methods. No statisticaldifference was found belween the results of gold standard methods and anti- H. pylori IgG ELISA (p=l). The sensitivity, specificity, positive and negative predictive values and likelihood ratio were 91.3 %, 37.5 %, 89.36 %, 42.85 % and 1.46, respectively. Forty-six (85.2%) patients were positive, three (%5.6) were negative, and five of them were borderline (9.3%) by anti H. pylori IgG Western blot test. When we didn't take borderline results into consideration, the sensitivity, specificity, positive and negative predictive values and likelihood ratio were 97.6%, 28.57%, 89.13%, 66.66%, and 1.37, respectively. Twenty-five patients (46.3%) were positive, and twenty-nine patients (53.7%) were negative by HpSA stool antigen test. When we compared the gold standard methods with the HpSA stool antigen test, there was a statistical difference between these methods (p<0.0001). The sensitivity, specificity, positive and negative predictive values were 54.35%. 100%, 100%, 27.59%, respectively. Thirty-two patients were positive (59.3%), and 22 of them (40.7%) were negative by the stool PCR method. The stool PCR method and gold standard methods showed a statistical difference for the detection of H. pylori infection (p<0.()()01). Sensitivity, specificity, likelihood ratio, positive and negative predictive values were 65.22%, 75%, 2.61, 93.75%. 27.7%, respectively. The invasive diagnostic methods to detect //. pylori causes false negative results because of the bacterial patchy-like infection. According to the Maastrich 2-2000 consensus report, the detection of //. pylori infection should be done by the noninvasive diagnostic methods on patients who are <45 years old with no alarm symptoms. In this study, the sensitivity of the anti- H. pylori IgG ELISA was in accordance with the other studies on H. pylori. However, the specificity was lower than the previous studies. The reason for this finding was due to untreated patients who had had IgG antibodies for a long time. The cut-off value might not be suitable for these patients or the genetic heterogeneity among the strains. Sensitivity and PPV of anti-//. pylori IgG Western blot test were also in accordance with the literature although the specificity was lower than expected. The cause of these results may be the ”serological scar“ as the antigenic similarity of the oilier gastric helicobactcrs especially //. heillmanni.difference was found belween the results of gold standard methods and anti- H. pylori IgG ELISA (p=l).
