Özet
ÖZET Amaç: Bu çalışmanın amacı; yoğun bakım ünitelerinde sıkça kullanılan benzodiazepin ve opioidlerin etkilerini antagonize eden flumazenil ve naloksonun, deksmedetomidinin antinosiseptif etkisi üzerine etkileşimini araştırmaktır. Materyal ve Metot: Çalışmada, ağırlığı 25-35 gram olan 50 adet Bulb-C türü albino fare kullanıldı. Denekler beş gruba ayrıldı ve gruplar her birinde 5 erkek ve 5 dişi olacak şekilde düzenlendi (n=10). Birinci grup kontrol grubuydu ve bu gruptaki farelere serum fizyolojik uygulandı. İkinci gruptaki farelere 200 ug/kg deksmedetomidin, üçüncü gruptaki farelere 1 mg/kg yobimbin ve 200 ug/kg deksmedetomidin, dördüncü gruptaki farelere 1 mg/kg flumazenil ve 200 ug/kg deksmedetomidin, beşinci gruptaki farelere ise 2 mg/kg nalokson ve 200 ug/kg deksmedetomidin uygulandı. Analjezi eşiğini ölçmek için tail-flick testi uygulandı. Ölçümler, ilaç verilmeden önce ve ilaç uygulandıktan sonra 20.,40.,60. ve 80.dakikalarda yapıldı. İstatistiksel analiz, grup içi karşılaştırmalar tek yönlü ANOVA testi ile, gruplar arası karşılaştırmalar ise bağımsız student's t testi ile yapıldı. Bulgular: Bu çalışmanın sonuçlarına göre, grup içi karşılaştırmalarda grup 2, 4 ve 5' te 20, 40, 60 ve 80. dakikalarda anlamlı analjezi izlenirken, grup 3' te analjezi yönünden anlamlı fark yoktu. Gruplar arası karşılaştırmalarda kontrol grubuna ve grup 3' e göre grup 2' de anlamlı fark mevcuttu. Ancak grup 2 ile grup 4 ve 5 arasında istatistiksel olarak fark yoktu. vıSonuç: Deksmedetomidin ile kontrol grubuna göre anlamlı antinosisepsiyon sağlanmıştır. Deksmedetomidinin bu antinosiseptif etkisi yohimbin ile antagonize edilmiştir. Bununla beraber deksmedetomidinin antinosiseptif etkisi üzerine flumazenil ve naloksonun herhangi bir etkisi bulunamamıştır.

ABSTRACT Aim The aim of this study was to search the effects of flumazenil and naloxane, which are frequently being used in intensive care units for benzodiazepine and opioid antagonism, on dexmedetomidine's antinociceptive effect. Material and method 50 Bulb-C albino mice, weight between 25-35 grams, were used in the study. Mice were randomized into 5 groups and groups were desined as 5 male and 5 female mice in each group (n=10). Group 1 was control group and only saline was injected to these mice. 200 ug/kg of dexmedetomidine was injected to group 2, 1 mg/kg of yohimbin and 200 jag/kg of dexmedetomidine were injected to group 3, 1 mg/kg of flumazenil and 200 (J.g/kg of dexmedetomidine were injected to group 4 and finaly 2 mg/kg of naloxane and 200 ug/kg of dexmedetomidine were injected to group 5. We used tail-flick test for analgesia triger point determination. Measurements were done before drug administration and at 20., 40., 60. and 80. minutes after the drug administration. One way ANOVA test was used for statistical analysis in groups and independent student's t test was used for statistical analysis between the groups. Results In groups 2, 4 and 5, we found statisticaly significant analgesia at 20., 40., 60. and 80. minutes. There was no significant analgesia difference in group 3. There was significant statistical difference in group 2 when compared with control group and group 3. But there was no significant statistical difference between group 2 and groups 4, 5. vmConclusion We found significant antinociceptive effect with dexmedetomidine when compared with control group. This antinociceptive effect of dexmedetomidine was antagonized with yohimbin. However, we could not find any effect of flumazenil and naloxane on antinociceptive effect of dexmedetomidine.
