Özet

SUMMARY ARTERIEL DISTENSIBILITY AND MICROALBUMINURIA RELATION IN PATIENTS WITH PRIMARY HYPERTENSION Systematic arteriel hypertension which is commonly seen in society is one of the most important reasons increasing cardiovascular morbidite and mortality microalbuminuria is known to be an indicator of renal damage in diabetic patients studies carried out indicate that the excretion of increased uriner albumin increases together with the cardiovascular risk in not only diabetics but also hypertensives and even in general population. It's been indicated that Pulse Wave velocity (PWV) and pulse wave analysis which are indicators of arteriel stiffness, the major hemodynamic factor affecting pulse pressure, predict the cardiovascular mortality independently. For the study, 67 hypertensive patients and 73 normotensive control cases who applied to without Diabetes Mellitus and secondary hypertension, not receiving antihypertensive treatment were taken. 24 hours ambulatuar blood pressure observation and PWV measurement was done microalbuminuria levels were observed in 24 hours urine. In the cases, noticable differences regarding ambulatuar blood pressure and measured day systolic blood pressure, diastolic blood pressure, pulse pressure, mean blood pressure, microalbuminuria and pulse wave velocity was observed between the two groups. At the end of the study, as the independent variable of microalbuminuria, PWV (p<0,001) and sistolic blood pressure (p<0,01) were determined; and as the independent variables of PWV, age (p<0,000), systolic blood pressure (p<0,000) and microalbuminuria (p<0,006) were determined. In our study, we found out a close relation between microalbuminuria, an early finding of renal damage, and aortic PWV, an indicator of stiffness of elatic veins. We think that abnormalities observed inthe high blood pressure and function and structure of the kidney are not 26only limted with resistance arteriols and that big vein pathologies should be taken into consideration.
