Özet
Saglık hizmetlerinde kalitenin saglanmasında en önemli unsurlardan biri olan hasta güvenligi, hastalara yardım ederken olusabilecek zararlardan kaçınmak, kaza eseri yaralanmaları, hata olasılıgını azaltmak ve önlemektir. Ancak hasta güvenligi nitelikli saglık çalısanı kaynagının azalması halinde saglanamaz. Dünya çapında yapılmıs arastırmalar, hemsirelik is gücünün giderek azalmakta oldugunu ve bunun sonucu olarak saglık hizmetinde kalitenin saglanmasının zorlasacagına dikkat çekmektedir. (22, 29, 34) Bir serviste çalısan hemsire sayısı ve niteliginin belirlenmesi verilecek saglık hizmetinin kalitesini etkilemektedir. Çalısmalar hemsirelik is yükünün hasta bakımı sonuçlarına etkisini kanıtlamıstır. (30, 36) Bu çalısma bir hastanedeki hemsire is yükü ve niteliginin hasta güvenligi saglanmasına etkisini incelemek amacıyla yapılan analitik bir arastırmadır. Arastırma 15 Eylül 2006- 15 Mart 2007 tarihleri arasında bir askeri hastanenin cerrahi, nöroloji-nörosirurji, dahiliye servislerinde gerçeklestirilmistir. Bu servislerdeki hemsirelerin nitelikleri, meydana gelen olaylar ve hemsire is yüküne ait veriler literatür ve hakem görüsleri dogrultusunda hazırlanan ?olay bildirim formu?, ?hemsire nitelikleri kayıt formu?, ?hasta sınıflandırma skalası? ve ?risk sınıflama matriksi? ile toplanmıstır. Verilerin istatistiki analizi Anova Tek Yönlü Varyans Analizi, Kruskall- Wallis Varyans Analizi, Ki-Kare ve ki Ortalama Arasındaki Farkın Önemlilik (t testi) testleri ile yapılmıstır. Sonuç olarak: Çalısmanın yürütüldügü kamu hastanesinde, ? Çalısma süresi on iki saati geçen hemsirelerin hastaları daha fazla riskli olaylarla karsılasmıslardır. ? Refakatsiz hastalarda daha riskli olaylar ile karsılasılmaktadırlar. ? Servislerdeki yatan hasta sayıları hemsirelik bakımına gereksinim seviyelerine göre farklıdır. Hemsirelik hizmetinin daha kaliteli verilmesini saglamak, hataları ve istenmeyen olayları önlenmek için, hastaların hemsirelik bakımına gereksinimlerine göre hemsire sayısı belirlenmelidir. Hemsirelerin çalısma saatlerinin on iki saati geçmeyecek sekilde düzenlenmesi gerekmektedir. (

Patient safety is an important issue on providing quality of health services. The mean of patient safety is to avoid damage which occurs when helping patients, to prevent being injured by accident and to decrease the probability of making an error related to patient care. Decreasing the number of qualified health staff causes the increase of risk in patient safety. The studies in the literature state that nursing workforces decreasing and as a consequence of this, it is getting difficult to provide the quality of health services. (22, 29, 34) The number and the quality of nurses affect the quality of health services. The studies in the literature have proved the effects of the nursing workload on the outcome of patient care. (30, 36) This study is an analytic study and in this study it was aimed to investigate the effects of nursing workload and quality on providing the patient safety in a hospital. The sample of the study is the nurses who work in surgical, neurological and medical services in ?a military hospital?. The data were collected during the period of September 15th, 2006 till March 15th, 2007. The data about the qualification of nurses, nursing workload and incidents that causes risks on patient safety were collected by using incident report form, scale of patient classification and risk classification matrix and recording qualification of nurses. The analysis of One-way ANOVA, Kruskall-Wallis, Chi-Square and the difference between the two means was compared through the Independent Samples T-Test were conducted to interpret the data. The analyses in the study showed that, ? nurses experience more adverse events (risks about patient care) when they work more than 12 hours in the hospital. ? more risky adverse events occur with the patients who do not have an accompaniment. ? the number of patients in services have to be differentiated according to the needs of nursing care As a conclusion of the study; the number of nurses in a hospital should be determined according to the needs of nursing care of patients to prevent the errors and adverse events and to provide more qualified nursing services in hospitals. In addition to this, it may be said that the work hours of nurses should be planned as not more than 12 hours (
