Özet
Araştırma Multiple Sklerozlu (MS) hastaların yaşadıkları cinsel problemler ve cinsel problemlerin yarattığı duygu ve düşünceleri belirlemek amacıyla tanımlayıcı olarak yapılmıştır. Araştırma Ocak-Haziran 2006 tarihleri arasında, Atatürk Üniversitesi Yakutiye Araştırma Hastanesi Nöroloji polikliniğinde kayıtlı Multiple Skleroz'lu hastalarla gerçekleştirilmiştir. Araştırmaya Nöroloji Polikliniğinde kayıtlı olan ve araştırmanın örneklem özelliğine uyan 21 Multiple Skleroz hastası dahil edilmiştir. Araştırma niceliksel ve niteliksel bölümlerden oluşmaktadır. Çalışmanın niceliksel bölümünde hastalara, araştırmacı tarafından hazırlanan `soru formu' ile Golombok ve Rust tarafından 1986 yılında geliştirilen, Tuğrul ve arkadaşlarının 1993 yılında geçerlilik ve güvenilirliğini yaptığı `Golombok-Rust Cinsel Doyum Ölçeği' uygulanmıştır. Araştırmanın niteliksel bölümünde ise, hastaların cinsel yaşamlarıyla ilgili yaşadıkları sorunlara yönelik her vaka ile birebir derinlemesine görüşmeler yapılmıştır. Araştırmanın niceliksel sonuçlarına göre Multiple Sklerozlu kadın hastaların Golombok Rust Cinsel Doyum Ölçeği (GRCDÖ)'nden aldıkları minumum puan 38, maximum puan ise 93, Multiple Sklerozlu erkek hastaların ise GRCDÖ'nden aldıkları minumum puan 20, maximum puan ise 62 olarak bulunmuştur. Araştırmanın niteliksel aşamasında MS'li hastaların cinsel fonksiyonlarına yönelik derinlemesine görüşmeler sonucunda, MS'li hastaların %66.6'sının hastalandıktan sonra cinsel yaşamlarında sorun yaşadıkları, %85'inin bu konuyu bir sağlık çalışanı ile paylaşmadıkları, %80.9'unun cinsel isteklerinde azalma olduğu, %9.5'inde cinsel isteklerinde artış olduğu, %4.7'sinin ilişki sırasında ağrı yaşadığı, %25'inin vajinal kuruluk yaşadığı, %44.4'ünün erken boşalma sorunu olduğu, %47.6'sının orgazm problemleri yaşadığı saptanmıştır. Araştırmada MS'li hastalarla yapılan derinlemesine görüşmeler sonucunda tanı aldıktan sonra hastaların %24'ünün beden imajlarının bozulduğu, %10'unun üreme yeteneğini kaybetme korkusu yaşadıkları, %38'inin benlik saygılarının azaldığı, %29'unun eş memnuniyeti kaygısı taşıdıkları ve %10'unun yetersizlik hissettikleri belirlenmiştir. Hastalarla cinsel yaşamlarına dair korku ve endişelerine yönelik yapılan derinlemesine görüşmeler sonucunda, hastaların %33'ünün terk edilme/kabul görmeme korkusu, %43'ünün aldatılma korkusu ve %24'ünün cinsel rol kaybı korkusu yaşadıkları saptanmıştır. Bu çalışma sonucuna MS'li hasta ve eşlerinin hastalığa ve cinsel sorunlara uyumunda hemşirelerin düzenli olarak hasta ve eşlerinin gereksinimlerine yönelik rehberlik yapmaları ve bu konuda bilgilendirilmeleri önerilebilir.

The study was carried out with the aim of determining of feelings and thoughts created by sexual problems and sexual problems experienced by the patients with Multiple Sclerosis (MS). The study was carried out in Neurology Clinic of Atatürk University Yakutiye Research Hospital betwen the dates January and June 2006. 21 patients with MS, compatible with the study sampling and enrolled to the Neurology Polyclinic were included in the study. The resarch consist of the sections of quantitative and qualitive. In the quantitative section of the study, question form prepared by researcher along with Golombok Rust Sexual Satisfaction Scale developed by Golombok and Rust in 1986 and proved its currency by Tuğrul et al in 1993 were applied. In the qualitive section of the study, the interviews were made to gether with each case individually as regards sexual lives of the patients. According to quantitative results of the study, minımum score which women patients with MS took from Golombok Rust Sexual Satisfaction Scale was 38, and maximum score was 93; and minumum score which men patients with multiple Sclerosis took from Golombok Rust Sexual Satisfaction Scale was 20, and maximum score was 62. At the qualitative phase of the study, in the results of interviews throughly orienting to their sexual functions of the patients with MS, it was detected that 66.6%of the patients with MS experienced problems in their sexual lives after acquiring the disease, and that 85% of then didn?t share this matter with health professionals, and that 80.9% of them noted a reduction on their sexual desires, and that 9.5% of them experienced on increasing of their sexual desires, and that 4.7% of them had pain during intercourse, and 25% of them experienced vaginal dryness, and that 44.4 % of of them had early discharge problem, and that 47.6% of them had orgasm problems. In the results of deep interviews made patients with Multiple Sclerosis, it was found out that 24% of them body images changed after dignosis was taken, and that 10% of them experienced the fear of loss of fertility ability, and that 38% of them respectfulness reduced, and that 29% of them they carried the anxiety of mate satisfaction and that 10% of them they felt themselves in sufficient. In the study, in the result of deep interviews about their fear and anxiety as to their sexual life, it was detected that 33% of them experienced the fear of being left, that 43% of them being deceived, and that 10% of them experienced the loss of sexual role. In the conclusion of this study, it may be suggested that the nurses shuold make quidance towards the needs of patients and their spouses, and they should be informed in this subject in the adjusting to the disease and sexual problems of the patients with Multiple Sclerosis and their spouses.
