Özet
Çalışmamızın amacı, propofolün intravenöz enjeksiyonu sırasında oluşan ağrının azaltılması amacıyla kullanılan deksametazonun etkinliğinin araştırılmasıdırBu çalışma, fakültemiz etik kurul onayı alındıktan sonra prospektif, randomize olarak planlandı. Her grupta 40 olgunun bulunduğu 3 grupta toplam 120 olgu çalışmaya dahil edildi. Tüm olgularda standardizasyon amacıyla 20G i.v kanül sol el sırtına yerleştirildi. Grup I ( n=40 ) de 5 mL volümde % 0,9 NaCl , Grup II ( n=40 ) de toplam 5 mL volüm içerisinde 0,15 mg kg-1 dozda deksametazon, Grup III ( n=40 ) de ise 1 dakika süre ile uygulanan turnikenin ilk 10 saniyesinde, yine toplam 5 mL volüm içerisinde 0,15 mg kg-1 dozda deksametazon kullanıldıGruplar arasında demografik özellikler, ASA skorları, kullanılan propofol ve deksametazon dozları bakımından istatistiksel olarak anlamlı farklılık saptanmadı ( p>0,05Ancak gruplar arasında VAS skorları değerlendirildiğinde istatistiksel olarak anlamlı farklılıklar saptandı. Grup I'in VAS ölçümleri Grup II ve Grup III'den anlamlı derecede yüksek bulundu ( p<0,01 ). Grup II'nin VAS ölçümleri Grup I'den anlamlı derecede düşük bulunurken ( p<0,01 ), Grup III'ün VAS ölçümleri diğer iki gruptan anlamlı derecede düşük bulundu ( p<0,01Elde ettiğimiz bu sonuçlara dayanarak; propofol enjeksiyon ağrısının giderilmesinde deksametazonun etkinliğinin bulunduğu ve bu etkinliğin turnike kullanılarak daha da artırılabileceği kanısına vardık.

The purpose of this study is to determine the efficacy of dexamethasone in the alleviation of propofol injection painAfter approval from the Institutional Ethics Committee, a prospective, randomized study was conducted. A total number of 120 patients were divided to take a part in one of the three groups of 30 patients each. A 20G intravenous cannula was inserted to all patients in the left dorsum of the hand. Group I ( n = 40 ) received 5 ml of 0,9 % NaCl , Group II ( n = 40 ) received 0,15 mg kg-1 dose of dexamethasone in 5 ml volume and Group III ( n = 40 ) received 0,15 mg kg-1 dose of dexamethasone in 5 ml volume in the first 10 seconds of venous occlusion produced by a rubber tourniquet for 1 minuteThere was no significant difference in terms of patient charecterisitcs data, mean of ASA scores and the doses of propofol and dexamethasone among the three groupsOn the other hand, there was significant difference in the mean VAS scores of the groups. The mean VAS score of the Group I was significantly higher than the scores of Group II and III ( p<0,01 ). The mean VAS score of Group II was significantly lower than Group I ( p<0,01 ), whereas the score of Group 3 was lower than the other two groups ( p<0,01In our study, we found that dexamethasone has an efficacy on the alleviation of the propofol injection pain and this effect of dexamethasone can be improved by the occlusion of the vein by a rubber tourniquet.
