Özet
Bu araştırma, lösemi tanısı alan hastalarda yaşam kalitesi ve sosyal destek düzeylerini, bunları etkileyen faktörleri, yaşam kalitesi ve sosyal destek arasındaki ilişkileri belirlemek amacı ile planlanmış kesitsel bir çalışmadırAraştırmaya Ocak 2008- Haziran 2008 tarihleri arasında hematoloji polikliniğine başvuran lösemi tanısı almış, terminal dönemde olmayan, iletişim kurmaya engel teşkil edecek akli ya da bedensel bir özrü bulunmayan, okur-yazar olan, araştırmaya katılmaya istekli, 18 yaş ve üzeri 117 hasta yer almıştır. Veriler hasta bilgi formu, EORTC QLQ-C30 Yaşam Kalitesi ölçeği ve Çok Boyutlu Algılanan Sosyal Destek ölçeği kullanılarak toplanmıştırÇalışma grubundaki hastaların yaş ortalaması 49.86 olarak belirlenmiştir. Hastaların % 62.4'ü erkek, % 80.3'ü evli, % 47.9'u ilköğretim mezunu, % 34.2'si emekli, % 76.1'i çekirdek aileye mensup ve % 64.1'i SSK'ya bağlı olduğu belirlenmiştirHastaların % 53.9'u KML hastası olduğu, % 92.3'ü hastalığa uyum sağladığı, % 43.5'nin hastalığının 3 yıl ve üzeri süre önce teşhis edildiği, %88.9'nun lösemiyi uzun süre tedavi gerektiren bir hastalık olarak gördüğü, % 97.4'ünün ilaç tedavisine uyum sağladığını, % 49.6'sının ilaçların tedavi edeceğine inandığını, % 83.8'nin sigara ve %94'nün alkol kullanmadığını ifade ettiği belirlenmiştirHastaların genel sağlık durumu puan ortalaması 59.76 ± 24.01 ve algılanan sosyal destek puan ortalaması 58.04 ± 17.45 olarak bulunmuştur. Hastalar fonksiyonel ölçeklerden en yüksek puanı rol fonksiyondan alırken en düşük puanı da emosyonel fonksiyondan almıştır. Genel sağlık durumu erkeklerin kadınlara göre daha iyi olduğu bulunmuştur (p<0.05). Semptom ölçeklerinden puan ortalaması en yüksek mali zorluk maddesinde bulunmuş ve en yoğun görülen 3 semptom sırası ile yorgunluk, uykusuzluk ve ağrı olarak bulunmuştur. Alt ölçeklerden en yüksek puanı aile alt grubu alırken bunu sırası ile arkadaş, özel bir insan izlemiştirGenel sağlık durumu, fonksiyonel ölçekler ve semptom ölçekleri yaş, cinsiyet, BKİ, çocuk sayısı medeni durum, eğitim durumu, löseminin türü, hastalığa uyum sağlama durumu, hastalığın teşhis edilmesi ve tedavi süresi, hastalığın algılanması, ilaçlar hakkındaki düşünceler gibi kişisel ve hastalıkla ilgili özelliklerden etkilendiği belirlenmiştir (p<0.05Algılanan sosyal destek düzeyi ve alt grupları medeni durum, eğitim durumu, hastalığa uyum sağlayabilme, BKİ ve çocuk sayısından etkilendiği belirlenmiştir (p<0.05ÇBASD ölçeği toplam ve alt boyutları ile EORTC QLQ-C30 yaşam kalitesi ölçeğinin genel sağlık durumu ve fonksiyonel ölçekleri ile pozitif yönde anlamlı bir ilişki belirlenmiştir. Semptom ölçeklerinden iştahsızlık semptomu ve mali zorluk maddesi ile negatif yönde anlamlı bir ilişki belirlenmiştir (p<0.05Çalışmanın bu sonuçları doğrultusunda;?Hemşirelerin lösemi hastalarının yaşam kalitesini ve sosyal destek düzeylerini etkileyen kişisel (yaş, cinsiyet, medeni durum vb.) ve hastalığa ilişkin (teşhis ve tedavi süresi vb.) özellikleri göz önüne alarak hemşirelik bakımını planlaması ve uygulaması,?Hemşireler tarafından lösemili hastaların yaşam kalitesi ve sosyal destek düzeylerini belirlemeye yönelik düzenli aralıklarla geçerli ve güvenilir ölçeklerle değerlendirmelerin yapılması önerilebilir.

This cross-sectional study which aims to determine the quality of life and social support levels of patients diagnosed with leukemia, the factors that affect these, and the relationship between the quality of life and social support.The study included 117 voluntary leukemia patients aged 18 years and over who applied to the hematology polyclinic between January 2008 and June 2008, who are not in the terminal period, who have no mental or physical disabilities that will prevent communication, and who are literate. Data was collected using the patient information form, EORTC QLQ-C30 Quality of Life scale, and the Multi Dimensional Scale of Perceived Social Support.The age average of the patients included in the study group is identified as 49.86. 64.4% of patients are identified as male, 80.3% as married, 47.9% as primary school graduates, 34.2% as retired, 76.1% as a member of an immediate family, and 64.1% as insured under SSI (Social Security Institution).53.9% of patients are identified as Chronic Myeloid Leukemia (CML) patients, 92.3% to be attuned to the disease, 43.5% to be diagnosed with the disease 3 or more years ago, 88.9% to consider leukemia as a disease that requires long term therapy, 97.4% to show adherence to the drug therapy, 49.6% to believe that the drugs will treat them, 83.8% to state that they do not use cigarettes and 94% to state that they do not use alcohol.The average score for the general health condition of patients was found to be 59.76 ± 24.01and the average score perceived social support was found to be 58.04 ± 17.45. While patients achieved the highest score from the role function, they received the lowest score from the emotional function. Men were found to have a better general health condition in comparison to women (p<0.05). The highest average score of the symptom scales was found in the financial difficulty item, and the most frequently observed 3 symptoms are fatigue, sleeplessness, and pain, respectively. The highest subscale score was achieved in the family subgroup followed by friend and significant others.It was determined that the general health condition was affected by personal and disease-related characteristics such as functional scales and symptom scales, age, gender, BMI, number of children, marital status, academic status, type of leukemia, adherence to disease, time taken to diagnose and treat the disease, detection of the disease, and conceptions about the drugs (p < 0.05).A positively significant relationship was found with the total and dimensions of the multidimensional scale of perceived social support (MSPSS), and the general health condition and functional scales of the EORTC QLQ-C30 quality of life scale. A negatively significant relationship was found with the loss of appetite symptom and the financial difficulty item from amongst the symptom scales (p < 0.05).In accordance with these results of the study;?Nurses can be advised to plan and provide nursing care for patients by taking into consideration the personal (age, gender, marital status, etc.) and disease-related (duration of diagnose and treatment, etc) characteristics that affect the quality of life and social support levels of leukemia patients,?Nurses can be advised to make assessments at regular intervals by means of valid and reliable scales aimed at determining the quality of life and social support levels of leukemia patients.
