Aim: In this study, the harmful effects of nicotine on flap surgery of rats exposed tonicotine preoperatively were examined. The aim is to determine by trial the effectsof resveratrol and cilostasol thought to eliminate the harmful effect of nicotine.Methods: Rats were divided into 6 groups with 10 animals in each group withinrandomisation method. Groups were called: Control(Group 1), Nicotine(Group 2),Nicotine and Resveratrol(Group 3), Nicotine and Cilostazol(Group 4), Nicotine andPreresveratrol(Group 5), Nicotine and Precilostazol(Group 6), Subcutenous nicotineinjections of 2mg/kg/day were administered to animals in groups 2,3,4,5 and 6 fortwo weeks preoperatively. Animals in group 3 received orogastricly 40mg/kg/dayresveratrol for 7 days beginning with surgery. Animals in group 4 also receivedorogastricly 100mg/kg/day cilostazol for 7 days beginning on the same day ofsurgery. Animals in groups 5 and 6 received the same doses of drugs for 10 daysbeginning on day 3 preoperatively. No postoperative drugs were administered togroup 1 and 2. A dorsal peninsula flap(McFarlane flap) with 3x9 cm cranial basedwas planned on the rat dorsum. Before the elevation of flaps, laser Dopplerinscriptions were taken from the proximal and distal 1/3 midpoint of fixed areas.Immediately after surgery and on postoperative day 7, inscriptions were repeated.Flaps were photographed at postoperative day 7 and full thickness biopsies weretaken from necrotic borders for histopathological and biochemical examination.Results: As there was no significant difference between preoperative cilostasol andpreoperative resveratrol groups and the postoperative applications of the samesubstances, they were assessed as the same group. After examination of necroticareas, we found that the necrosis in nicotine group was larger compared to controlgroup and the necrosis in groups that received resveratrol and cilostasol wassmaller(p<0,01). Laser Doppler measurement demonstrated that bloodflow innicotine group was reduced(p<0,01). Adenosin deaminase acvitity values ofcilostasol and resveratrol group showed a statistically significant increase comparedto control group. Histopathologic assessment of the necrotic areas were the largest inthe group receiving only nicotine. The necrotic area in the control group was smallerthan the nicotine group but larger than other groups.Conclusion: We found that preoperative nicotine application in rats had reducedflap viability. The use of resveratrol and cilostazol in rats exposed to nicotinereduces the harmful effects of the nicotine on flap viability.
