Özet
Diyabetik hastalarda perioperatif morbidite ve mortalite 2-3 kat daha fazladır. Diyabetik hastaların anestezisinde ek risk yaratan etmenler; kardiyovasküler sistemi etkileyen otonom nöropati, mide boşalmasını geciktiren gastroparezi ve entübasyonu güçleştiren eklem hareket kısıtlılığı olarak sıralanabilir. Diyabetik olgular hem kardiyovasküler komplikasyonlar hem de asfiksi yönünden yüksek risk taşıyan özel bir hasta grubu olmasına karşın, genel anestezi sırasında resüsitasyona yanıt açısından yapılmış herhangi bir çalışmaya rastlanmamıştır. Bu çalışmada streptozotosinle diyabet ve otonom nöropati oluşturulan sıçanlarda asfiksiye bağlı dolaşım arresti ve resüsitasyona yanıt bakımından sevofluran anestezisi ile propofol-fentanil anestezisinin karşılaştırılması ve hangi yöntemin daha güvenli olabileceğinin belirlenmesi amaçlandı.Kurumsal Hayvan Deneyleri Etik Kurulu onayı alınarak 40 adet Sprague-Dawley cinsi sıçan rastgele 4 eşit gruba ayrıldı. Gruplar; sevofluran kontrol (Grup SK), propofol?fentanil kontrol (Grup PFK), sevofluran diyabetik (Grup SD) ve propofol?fentanil diyabetik (Grup PFD) olarak tanımlandı. Sıçanlarda diyabet oluşturmak için 60 mg/kg streptozotosin, 0,5 ml fosfat tampon çözeltisi içinde intraperitoneal verildi; 0, 3, 10. günlerde ve 6. hafta sonunda kan glukozu ölçülerek diyabet oldukları belirlendi. 6. hafta sonunda EKG kayıtları ile düzeltilmiş QT intervalleri hesaplanarak otonom nöropati geliştiği gösterildi. Daha sonra sevofluran (O2-hava karışımı içinde %2) ile inhalasyon anestezisi ya da propofol (15 mg/kg/sa) ve fentanil (25 µg/kg/sa) ile intravenöz anestezi altında asfiksi-resüsitasyon modeli uygulanarak dolaşım arresti gelişme süreleri ve resüsitasyona yanıtları kaydedildi.Asfiksiye bağlı dolaşım arresti gelişme süresinin diyabetik gruplarda (Grup SD: 221,6±21,2 sn, Grup PFD: 199,6±21 sn) kontrol gruplarından (Grup SK: 401,1±11,5 sn, Grup PFK: 315,2±36,1 sn); intravenöz anestezi verilen gruplarda (Grup PFK, Grup PFD) da inhalasyon anestezisi verilen gruplardan (Grup SK, Grup SD) daha kısa olduğu saptandı (sırasıyla p<0,0001, p<0,05). Ayrıca intravenöz anestezi ile resüsitasyona yanıt verme süresi daha uzun (Grup PFK'de 173,3±100,8 sn ve Grup PFD'de 267,5±10,7 sn'ye karşılık Grup SK'de 62,6±59,6 sn, Grup SD'de 107,6±62,3 sn) (p<0,05) ve sağkalım oranı da daha düşük (Grup PFK'de %60 ve Grup PFD'de %20'ye karşılık Grup SK'de %100, Grup SD'de %70) (p<0,05) bulundu.Sonuç olarak; otonom nöropati gelişmiş diyabetik olguların genel anestezisi sırasında asfiksiye bağlı kardiyak arrest gelişme riski ve resüsitasyona yanıt bakımından sevofluran ile inhalasyon anestezisi yönteminin, propofol-fentanil ile intravenöz anestezi yöntemine göre daha güvenli olduğu kanısına varıldı.

Perioperative morbidity and mortality are increased 2 to 3 fold in patients with diabetes. The factors that give rise to additional risks in anesthesia of diabetic patients are related to be; autonomic neuropathy that affects the cardiovascular system, gastroparesis that leads to delay in gastric emptying and limitations in joint mobility that might complicate intubation. Despite diabetics considered to be a special high risk group of patients in terms of asphyxia and cardiovascular complications, during general anesthesia any study has not been encountered concerning the response to resuscitation. In this study we aimed to compare sevoflurane and propofol-fentanyl anesthesia in streptozotocin induced diabetic rats with autonomic neuropathy in terms of circulatory arrest due to asphyxia and response to resuscitation, and to determine which method would be safer.Forty Sprague Dawley rats were randomly divided into 4 groups after the approval taken from the Institutional Ethics Committee on the Animal Experiments. The groups were defined as sevoflurane control group (Group SC), propofol-fentanyl control group (Group PFC), sevoflurane diabetic group (SD) and propofol-fentanyl diabetic group (PFD). Streptozotocin 60 mg/kg dissolved in 0.5 ml phosphate buffer solution was given intraperitoneally to the rats; blood glucose levels were measured on 0, 3, 10th day and at the end of 6 weeks to demonstrate they were diabetes. At the end of 6 weeks the ECG recordings of the rats were taken and the corrected QT intervals were calculated to demonstrate the presence of autonomic neuropathy. Then an asphyxia-resuscitation model was performed under inhalation anesthesia with sevoflurane (2% in O2-air mixture) or under intravenous anesthesia with propofol (15 mg/kg/h) and fentanyl (25 µg/kg/h) during which the arrest intervals and responses to resuscitation were recorded.The time needed for the onset of circulatory arrest due to asphyxia in diabetic groups (Group SD: 221.6±21.2 s, Group PFD: 199.6±21 s) were shorter than control groups (Group SC: 401.1±11.4 s, Group PFC: 315.2±36.1 s) (p<0.0001) and also in intravenous anesthesia groups (Group PFC, Group PFD) than inhalation anesthesia groups (Group SC, Group SD) (respectively p<0.0001, p<0.05). Besides the response interval to resuscitation with intravenous anesthesia was found longer (Group PFC 173.3±100.8 s and Group PFD 267.5±10.6 s versus Group SC 62.6±59.6 s and Group SD 107.6±62.3 s) (p<0.05) and also survival rate with intravenous anesthesia was found lower (Group PFC 60% and Group PFD 20% versus Group SC 100% and Group SD 70%) (p<0.05).It was concluded that inhalation anesthesia with sevoflurane is safer than intravenous anesthesia with propofol-fentanyl in cases with autonomic neuropathy due to diabetes in terms of the development of cardiac arrest due to asphyxia and response to resuscitation.
