Özet
Osteoporotik proksimal humerus kırığı olan hastalarda cerrahi tedavi sonuçlarımız, Gülhane Askeri Tıp Akademisi Haydarpaşa Eğitim Hastanesi Ortopedi ve Travmatoloji Servisinde Ekim 2009 - Aralık 2013 tarihleri arasında osteoporotik proksimal humerus kırığı tanısı ile cerrahi uygulanan 25 hastanın sonuçları değerlendirildi. Hastaların ortalama takip süresi 32 ay (3-54 ay) idi. Hastaların 23'ü (%92) kadın, 2'si (%8) erkekti. Yaş ortalaması 75'ti (60-92). 10 hastada sağ, 15 hastada sol proksimal humerus kırığı vardı. Kırık nedenleri 1'i araç içi trafik kazası, 1'i araç dışı trafik kazası, kalan 23 hasta ise basit düşme ile meydana gelmişti. Hastaların kırıkları Neer sınıflaması ile değerlendirildi ve 10 hasta 2 parçalı, 13 hasta 3 parçalı, 2 hasta 4 parçalı humerus kırığı mevcuttu. Hastaların başvurularında hepsine omuz ön-arka ve lateral grafileri çekildi. Eşlik eden yaralanmaları olan ve parçalı kırığı olan hastalarda bilgisayarlı tomografi çekildi. Hastalar travma sonrası ortalama 18,48 (3-153) gün sonra opere edildiler. 2 hastaya k-telleri ile fiksasyon sağlanırken, 16 hastada plak osteosentezi ve 7 hastada hemiartroplasti uygulandı. Hastaların yapılan son kontrol muayenelerinde Constant-Murley skoru ortalaması 65,52, ASES skoru ortalaması 62,16 olarak hesaplandı. Cerrahi teknikler değerlendirildiğinde k-teli ile opere edilenlerde ASES skoru 82,5, Constant-Murley skoru 82,5. Plak osteosentezinde ASES skoru 65,125, Constant-Murley skoru 71,56. Hemiartroplasti uygulanan hastalarda ASES skoru 49,57, Constant-Murley skoru 46,86 olarak hesaplandı. Hemiartroplasti ile kötü sonuçlar alınırken, minimal invaziv tekniklerle ve plak osteosentezi ile iyi sonuçlar alındı. Sonuç olarak literatürle karşılaştırıldığında cerrahi sonuçlarımızın uyumluluk göstermektedir. Cerrahi kararı verilen osteoporotik proksimal humerus kırıklarında öncelikle minimal invaziv teknikler veya kilitli plaklar ile osteosentez sağlanmalıdır. Hemiartroplasti çok parçalı kırıklarda ve diğer tekniklerle başarılı olunamaz ise tercih edilmelidir.

Our results of surgical treatment for patients with osteoporotic proximal humerus fractures The results of 25 patients who underwent surgery with a diagnosis of osteoporotic proximal humerus fractures between the date of October 2003 and December 2013 were evaluated in orthopedics and traumatology service, Gulhane Military Medical Academy Haydarpasa Training Hospital. The average length of follow-up of the patients was 32 months (3-54 months). 23 of the patients (92%) were female and 2 of them were male. The average of age was 75. 10 patients have right proximal humerus fractures and 15 patients have left proximal humerus fractures. One of the reason for fractures is in-vehicle traffic accident, one of those is out of the vehicle accident and others are falling into place. The fractures of patients are considered with Neer classification and 10 of the patients have 2-piece humeral fractures, 13 of them have 3-piece humeral fractures and others have 4-piece humeral fractures. All patients had anterior and posterior shoulders and lateral radiography scan after they applied to the hospital. Patients with associated injuries and segmental fracture had a computed tomography scan. After trauma, Patients were operated 18-48(3-153) days later on average. 16 patients were treated with osteosynthesis and 7 patients were treated with hemiarthroplasty while 2 patients were treated with k-wires fixation. In the last examination of patients, the average of Constant-Murley score was calculated as 65,52 and the average of ASES score was calculated as 62,16. When compared on the basis of surgical techniques, ASES score of those operated with k-wires is 82,5 and the Constant-Murley score of those is 82,5. ASES score of those operated with Plate osteosynthesis is 65,125 and the Constant-Murley score of those is 71,56. The ASES score of those operated with hemiarthroplasty was calculated as 49,57 and the Constant-Murley score of those was calculated as 46,86. Bad results were gotten with hemiarthroplasty, while plate osteosynthesis and minimally invasive techniques resulted in good results. Consequently, our surgery results are consistent with the literature. In the osteoporotic proximal humerus fractures, firstly osteosynthesis treatment with minimally invasive techniques or locked plates should be applied. If other techniques does not work on multi-part fractures, hemiarthroplasty should be applied.
