Background Cardiovascular disease (CVD) is the major cause of mortality and morbidity across the world, causing one third all global deaths. However, secondary prevention plays a key role to reduce coronary mortality and morbidity. Mobile phone text messaging is a significant means of people's communication globally, therefore, it has the potential to improve secondary prevention in CVD. Objectives The aim of this review is to identify, retrieve, critically appraise and synthesize the existing mobile phone text messaging interventions that have been done for secondary prevention of CVD. Methods The systematic review has followed The Cochrane Handbook for Systematic Reviews of Interventions. The CONSORT statement checklist (2010) was used to assess the quality of reporting in the studies. The searching was conducted by using the following six databases: MEDLINE via OVID (1946 to July Week 2 2016), EMBASE via OVID (1974 to July Week 2 2016), PsychINFO (1806 to July Week 3 2016), CINAHL, PUBMED and ScienceDirect. Only Randomised Controlled Trials (RCTs) were selected. Results A total of nine RCTs were included for this review. The preventive factors measured among studies varied including medication adherence, blood pressure level (systolic and/or diastolic), smoking, physical activity, body mass index (BMI). Only medication adherence and blood pressure were taken into consideration for this review because they were the most common outcome assessed in the studies to prevent patients from existing CVD. Whereas, four of the six trials found improvement on medication adherence on patients with CVD, three of the six studies did not find reduction on BP level. Conclusion The results of this systematic review indicate that there is a moderate quality data on the effectiveness of mobile phone text messaging interventions for the secondary prevention of CVD and this review found that in overall, mobile phone text messaging is shown to benefit CVD patients on medication adherence which encourages and highlights the possibility of improving secondary prevention of CVD. However, mobile phone text messaging is shown to have a few benefit for CVD patients on BP improvement which does not encourage the possibility of improving secondary prevention of CVD.
