Özet

ÖZET Atopik dermatitli hastalarda tip I hipersensitivitenin ve prick test yöntemi ile etkenlerin saptanması amacı ile yapılan bu çalışma sonucunda; Prick test pozitifliği açısından hasta ve kontrol grubu arasında istatistiksel olarak anlamlı bir fark olmasına rağmen hasta grubundaki negatif sonuçların ve kontrol grubundaki pozitif sonuçların beklenilen düzeyin üzerinde çıkması atopik dermatitin tanısında prick test pozitifliğinin yerini şüpheli kıldı. En sık allerjenler arasında bakıldığında ise yanlızca bazı çayır ve çimen polenlerine karşı pozitif sonuçlar elde edildi. Gıda allerjenlerine karşı elde edilen pozitif sonuçların literatürde belirtildiği gibi yüksek çıkmaması kayda değer bulundu. Atopik dermatitin diğer atopik hastalıklara (allerjik astım, allerjik rinit) eşlik ettiği olgularda prick test pozitifliği beklenildiği gibi yüksek çıktı. Serum total ile ilgili düzeyi hasta grubunda kontrol grubuna oranla yüksek bulundu. Pityrosporum ovale antijen solüsyonu ile yaptığımız prick test sonucunda hasta ve kontrol grubu arasındaki fark istatistiksel olarak anlamlı idi. Özellikle baş ve boyun tutulumu olan olgulardan pozitiflik oranı diğer olgulara göre daha yüksek bulundu. 39

SUMMARY. This study was done in patients with atopic dermatitis to find the causes of the disease by using the prick test. Although there was a statistically significant difference in the frequency of positive prick tests between the patients and the controls, the value of positive prick tests was doubtful in the diagnosis of atopic dermatitis because the frequency of negative results in the patients and the frequency of positive results in the controls were higher than the expected frequencies. It was a significant finding that the majority of allergens found by the prick test were some grass pollens and the frequency of positive results against to the food allergens was not as high as those reported in the literature. The frequency of positive prick tests was high in atopic dermatitis patients with other atopic disorders (allergic asthma, allergic rhinitis), as expected. The level of serum total IgE in the patients was higher than that in the controls. In the prick test with the allergen solution of Pityrosporum ovale, the difference between the frequencies of positivity in the patients and in the controls was statistically significant. The frequency of positivity was especially higher in the patients with the involvement of head and neck than in the others. 40
